OBJECTIVE: To determine the more effective dose of intramuscular (IM) Phenylephrine in preventing hypotension due to spinal anesthesia (SA) administered for cesarean section. METHODS: The study was conducted from January 1, 2020 to July 31, 2020, at Rehman Medical Institute, Peshawar, Pakistan. Sixty participants of American Society of Anesthesiologists (ASA) physical status 1 and 2, scheduled for elective cesarean section (CS) under SA were enrolled in this prospective double-blind study and randomized into two equal groups, P4-group and P8-group. P4-group received IM Phenylephrine 04 mg and P8-group received IM Phenylephrine 08 mg before SA. The incidence and intensity of hypotension, rescue doses of Phenylephrine and any adverse event, were recorded. Data collected was analyzed using Epi lnfo ™ version 7.2.5. RESULTS: Mean age of patients from P4-Group and P8-Group was 28.10±4.39 years and 28.33±4.95 years respectively. There were no statistically significant differences in the two groups with respect to demographic and clinical characteristics. Hypotension occurred in 33.3% (n=20/60) of participants. The incidence of hypotension in P4-group and P8-group were 46.7% (n=14/30) and 20% (n=6/30) respectively. In P8-group, risk of hypotension was 1.5 times less common compared with P4-group with 95% confidence interval of 1.02–2.19 (p=0.02). There was also no statistically significant difference between the onset of hypotension in the two groups. Those who developed hypotension had consumed a lesser amount of rescue Phenylephrine in P8-group compared with P4-group (p-value <0.01). CONCLUSION: Phenylephrine 08 mg IM preemptively controlled SA induced hypotension in a sustained way in the fit parturients.

OBJECTIVES: This scoping review aimed to appraise the existing literature on using the claims databases for epidemiological studies and to draw inferences for using data from Pakistan's health insurance databases. METHODS: We conducted a scoping review of literature focusing on health insurance databases, querying MEDLINE, EMBASE and Google Scholar. We used the frameworks proposed by the Joanna Briggs Institute and Arksy and O'Malley for mapping our results. RESULTS: There was a considerable chronological increase in studies published using data from health insurance databases. Most of the studies in our search were from economically developed countries. Most of the studies (n=84) focussed on chronic non-communicable diseases, while a limited number (n=09) focussed on communicable (infectious) diseases. Our findings suggest that insurance databases could be utilised to study rare diseases, prospects of prolonged follow-up, and minimal research costs. This is especially important for countries like Pakistan, having limited resources to conduct regular, population-level epidemiological studies. Several methodological approaches (for instance, disease, pharmacy or intervention classification codes) were presented in these studies to extract epidemiological data from the insurance database. CONCLUSION: Health insurance databases are utilised as sources for epidemiological studies, predominantly for chronic illnesses, in economically developed countries. Methodological approaches described in these papers could be used to extract data for epidemiological research from health insurance databases in Pakistan. This could be especially useful for following the patterns of infectious disease in the country.

OBJECTIVES: To compare the different smile variables amongst patients with varying vertical skeletal patterns, and to find whether there is a correlation between the dynamic contributing factors and the vertical smile parameters. METHODS: This cross-sectional study was conducted at department of Orthodontics, Nishtar Institute of Dentistry, Multan, Pakistan from 01-10-2020 to 01-04-2021. The posed smiles of 120 participants, aging 15-25 years, having well-aligned dental arches were videotaped for objective measurements and comparisons with their underlying vertical skeletal growth patterns. RESULTS: The evaluation of smile parameters showed a significantly decreased inter-labial gap in patients with skeletal low angle in males (8.92±3.6mm) and females (9.44±2.58 mm) [p<0.004]. Upper lip length was significantly decreased in low angle (16.23±1.59 mm) cases for males as compared to vertically high (18.57±1.47 mm) and normal angle (18.24±2.08 mm) cases. Inter commissural width was increased significantly in the low angle cases (63.39±8.14 mm) when compared with the high angle cases (58.24±5.44 mm) in females. Maximal Incisal Display was significantly increased in high angle cases (9.03±1.31 mm) p<0.001 as compared to the low angle cases (7.64±1.98 mm). A 65.8% (n=79) of the sample showed a flat smile arc with 26.7% (n=32) having consonant smiles while 7.5% (n=9) showed a reverse smile arc. CONCLUSION: Smile dynamics are influenced by underlying skeletal patterns. Individuals with skeletally high angles showed an increased incisal display and an increased upper lip elevation upon smiling. While the low angle patterns displayed a decreased inter-labial gap and majority showed a flat smile arc.

OBJECTIVE: To explore the mediating role of self-regulation in peer relationship, rejection sensitivity and positive development among adolescents. METHODS: This cross-sectional research was conducted at Department of Psychology from October, 2018 to September, 2019, comprised of two phases. In first phase, Self-Regulation Questionnaire (SRQ), Adolescent Peer Relations Instrument (APRI), Rejection Sensitivity Questionnaire (RSQ), and Positive Youth Development Inventory (PYDI), were translated into Urdu by forward-backward translation method. For pilot-testing, these scales were administered on 130 students and scales were found to be reliable. In second phase, Urdu versions of four instruments were st administered to a sample of 608 (males=330, females=278) college students of 1 nd and 2 years studying in various colleges of Sara-i-Alamgir by stratified proportionate random sampling technique.

OBJECTIVE: To assess and compare the cognitive functioning of post-Covid patient with non-Covid person among the general population of Khyber Pakhtunkhwa province of Pakistan. METHODS: This descriptive cross-sectional study was conducted from January to May 2021. Sample of 500 community members, including both post-Covid and non-Covid, from population of Khyber Pakhtunkhwa, Pakistan. Male and female with age of 18-64 years were included, age less than 18 years, having any nervous system or psychiatric illness or previous known cognitive impairment were excluded from the study. Validated Cognitive Functioning Self Reporting Scale (CFSS) questionnaire was used. Data was analyzed through SPSS V.25. RESULTS: Out of 600 approached, 500 (83.3%) participants responded to online questionnaire. Mean age of participants was 45+or-22 years, Majority (n=174: 34.7%) of participants were from 31- 40 years' age group. Hypertension (n=33;6.6%) and Diabetes Mellitus (n=30;6%) were the most common comorbid conditions;while 378 (75.6%) had no comorbidities. Mean CFSS values was 38.4+or-11.5 and 42.2+or-12.04 for male (n=230) and female (n=270) participants respectively. Based on standard cut off values of CFSS, mild, moderate and severe cognitive impairment was observed in 46 (24.8%), 127 (68.6%) and 12 (6.4%) cases of post-COVID and 19 (20.4%), 73 (78.4%) and 1 (1.0%) cases of non-COVID participants having no comorbidities. CONCLUSION: Cognitive performance was moderately affected among post COVID patients as compared to non-Covid population, compromising daily executive functioning as well as other elements of mental health such as attention, memory, spatial functioning & inhibition.
