Epidemiological Characteristics of SARS-CoV-2-Positive Samples in Japan: A Retrospective Study 日本sars - cov -2阳性样本流行病学特征的回顾性研究

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) causes coronavirus disease 2019 (COVID-19). In Japan, infections began in January 2020, and the number of infections has continued to increase. Specimens requested by medical institutions were measured for ORF1a/b, N-gene, and S-gene using TaqPath SARS-CoV-2 real-time polymerase chain reaction detection kits in our biomedical laboratory from January to August 2021. The distribution of threshold cycle (Ct) values of positive samples was analyzed separately every 2 months. The Ct values of saliva samples were normally distributed for all genes, except for the May/June S-gene. By contrast, nasopharyngeal swab samples showed a bimodal distribution from January to June, with a shift toward lower median values in July/August. The results suggest that viruses with different properties may be captured in saliva and nasopharyngeal samples.

Screening for Gastric Cancer 胃癌筛查

Gastric cancer remains one of the most lethal types of cancer. When examining the global distribution of gastric cancer, the highest incidences are observed in South-East Asia, as well as in China and Japan [1]. Histologically, most gastric cancers are adenocarcinomas. Known risk factors are H. pylori infection, chronic gastritis, tobacco use and heavy alcohol consumption. A small percentage of gastric cancers occur hereditary. The TNM-classification is used for gastric cancer staging. Imaging methods, blood tests with tumor markers, and gastroscopy with biopsies are employed for detection, diagnosis, grading and staging. Treatment options include operation, chemotherapy, immunotherapy and radiotherapy. There are non-invasive methods (such as H. pylori serology and plasminogen tests), invasive methods (endoscopy), and radiological methods (upper gastrointestinal series with barium, photofluorography) to screen for gastric cancer. Screening programs are most useful for countries with high incidence of gastric cancer, where the technical and economic conditions exist at the same time. That applies to most countries in East Asia, for instance for South Korea, and of course also for Japan and increasingly for China, where the overall costs are in a favorable relationship to the benefits [2]. In countries with lower incidence of gastric cancer, non–invasive screening programs are discussed controversially, although endoscopy is readily available almost everywhere, has few complications and is affordable for most health systems. In Europe and the United States gastric cancer screening programs and recommendations regarding screening are in place in some but by far not in all countries [3].

Although Osteoarthritis (OA) is a widespread type of arthritis, no cure or medication can halt its natural progression. Only weight loss and physiotherapy can help to relieve pain and preserve function. Chronic un-inflammatory synovitis is not uncommon in OA; however, Inflammatory Arthritis (IA) can also start in middle-aged adults affected by OA. Pain and swelling of the joints, especially in the knee joints, are usual complaints in rheumatological practice where the primary treatment for chronic inflammatory arthritis is disease-modifying antirheumatic drugs (DMARDs). At the same time, persistent chronic synovitis leads to secondary OA due to inflammation, aging, and other factors. Discrimination between chronic synovitis due to inflammation or degeneration poses a significant challenge, especially when specific markers for IA are negative. Interleukin-6 (IL-6) has been a research topic for many scientific publications over the past several years. Although IL-6 production and signaling have been observed in OA, a recently published article shows much more elevated IL-6 concentration in synovial fluid (SF) of symptomatic joints in different types of IA [1]. We decided to clarify the importance of IL-6 concentration in synovial fluid (SF) for diagnostic and treatment purposes.

Impact of COVID 19 on Hospitalization for Thromboembolic Diseases in Germany COVID - 19对德国血栓栓塞性疾病住院治疗的影响

Objectives: We compared hospitalization rates for thrombotic venous and arterial disease in 2020 and 2021 of the COVID-19 pandemic with the 3-year period from 2017 to 2019 before. Patients and Methods: Lists of hospitalizations with principal diagnoses of Pulmonary Embolism (PE), Deep Vein (DVT), Sinus Vein (SVT) and Portal Vein Thrombosis (PVT), Budd-Chiari-Syndrome (BCS), Myocardial Infarction (MI), Embolic Stroke (ES), and acute Peripheral Embolism and Thrombosis (PET) were provided by the Federal Statistical Office in Germany. Results: The number of hospitalizations was fairly constant before the COVID-19 pandemic for PE, PVT, BCS, MI, and ES; those for DVT and PET were trending downward; and those for SVT were trending upward. In 2020, absolute number of all hospitalizations was 13.8% lower than the average from 2017 to 2019. Contrary to this trend, hospitalized cases for SVT, BCS, and ES were within the 95%-CI of the 3 years before COVID-19, and they were lower for PE, DVT, PVT, MI, and PET. In 2021, absolute number of all hospitalizations was 14.6% lower. Contrary to this trend, the absolute number of hospitalizations for PE, SVT, and ES was above the upper limit of the 95%-CI of the 3 years before COVID-19. The increase in PE in 2021 occurred in the 50-to 80-year age groups, and the decrease in MI and PET in the 70-and 85-year age groups. The increase in SVT affected all age groups. Conclusion: The analysis shows that the COVID-19 pandemic does not affect all venous and arterial diseases equally.
