PSYCHOLOGICAL INTERVENTIONS IN THE TIME OF CRISIS 危机时刻的心理干预

Psychological interventions may improve patients’ motivation and adherence to various therapies, including those alleviating pain, depression, and fatigue. Evaluating patients’ psychological status and choosing the most appropriate intervention may improve combined treatment outcomes, particularly in the elderly and other vulnerable groups. Exposure to psychotraumatic events such as violence, torture, and limb amputation should be appropriately assessed to choose a psychotherapy method. Narrative exposure therapy and cognitive behavioural therapy are commonly used psychological interventions with variable level of evidence. Training first-contact health professionals and psychologists to use the most efficient evidence-based psychological interventions is thus becoming critical at the current stage.

TELEREHABILITATION AFTER HIP-FRACTURE SURGERY 髋部骨折手术后的远程康复

Hip fracture is one of the most common fractures among the elderly population with high rates of mortality and morbidity. The annual incidence of hip fractures is predicted to be increased by around 16 times by the year 2050. Due to the high cost of traditional rehabilitation after hip fracture surgery and the effect of pandemics, telerehabilitation after hip fracture is gaining more importance in recent years. This review is designed to examine the importance and progress of telerehabilitation after hip fracture surgery. PubMed/MEDLINE, Scopus, and Web of Science for English-language sources were searched from their inception using the keywords telerehabilitation and hip fracture between January 2014 and January 2023. The literature search resulted in 29 non-duplicate records and 8 of them were eligible and were included in this review. Telerehabilitation is a feasible, safe, and effective rehabilitation method after hip fracture surgery. However, the ability of elderly patients and their caregivers to use technology and apply what they see seems to be a big disadvantage. The literature lacks long-term studies with larger patient groups.

EPIGENETIC ALTERATIONS IN GENE EXPRESSION IN THE AGING IMMUNE SYSTEM 衰老免疫系统中基因表达的表观遗传改变

Immunosenescence or aging of the immune system is characterized by dysregulation of the innate and adaptive immune system mechanisms, associated with a systemic low-grade inflammation (i.e., «inflammaging»), non-effective protective humoral and cellular immune responses to a microorgamis or vaccine, autoimmunity, and cancer. In addition, genetic dysregulation and alterations across the immune system drive the cellular manifestations of immunosenescence. In this narrative review, we provide an overview of the recent advancements in immunosenescence research concerning epigenetics alterations in gene regulation and disturbances in gene expression in the aging immune system, focusing on DNA methylation, histone modifications, and microRNAs.

CAN MANUAL THERAPY AND MASSAGE BE USED AS COMPLEMENTARY METHODS FOR FEMALE INFERTILITY IN DIFFERENT AGE GROUPS? 手法治疗和按摩是否可以作为不同年龄段女性不孕症的补充方法?

Infertility is the inability to conceive after one year of unprotected intercourse, and about one in six couples deal with this issue. Nearly half of the factors contributing to a couple’s infertility are female-related, and the underlying cause is variable. Many treatment options can be used depending on the underlying cause of infertility. The available treatments often include in vitro fertilization, intrauterine insemination, and fertility-inducing drugs. Numerous couples favor complementary medicine alongside conventional treatments to enhance treatment efficacy and reduce side effects. Manual therapy and massage are among these complementary treatment approaches. Manual therapy is defined in various ways. One of them is manipulating and mobilizing soft tissue structures and related joints using the hands. Manual therapy can alleviate pain, reduce inflammation, prevent contracture formation, manage existing contractures, increase range of motion, aid mobilization, and improve overall health. It contains a variety of massage treatments, stretching, and mobilization applications. Manual therapy and massage can be administered to manage various disorders, from pediatric to geriatric populations. These are safe interventions. Manual therapy and massage have beneficial effects on the management of female infertility. These methods reduce adhesions and mobilize organs in the pelvic region. They have positive effects on circulatory system disorders, including lymphatic drainage abnormalities. Overcoming reproductive system lymphatic drainage and circulation difficulties can result in balanced hormone levels, regular menstrual cycles, and pregnancies. In addition, manual therapy and massage improve the psychological state of individuals in a way that will benefit the management of female infertility.

Systemic lupus erythematosus (SLE) is an autoimmune disease affecting multiple systems and presenting with a wide range of clinical symptoms. Disease phenotype alters from childhood to old age. The possible reasons for these differences are genetic factors, changes in the hormonal status, and the aging immune system. As a result of the disease itself or as a side effect of medications, SLE patients often experience manifestations associated with aging like cardiovascular disease and infection susceptibility. Furthermore, immunosenescence, which commonly affects the elderly subjects, may result from the continued immune system activation during the natural history of SLE. In this study, we aimed to analyze the immunosenescence in SLE, the differences and similarities in SLE phenotype at different age groups, and possible reasons for these changes.

SENILE CALCIFIC AORTIC VALVE STENOSIS: MODERN INTERVENTIONS IN UKRAINE 老年钙化主动脉瓣狭窄:乌克兰的现代干预措施

Background. Aortic valve stenosis remains the most dominant form of valvular heart disease. The aortic valve area below 1.0 cm2 is an assignment to the interventions. The modern senile aortic valve stenosis treatment options are mini-surgical valve replacement (mini-SVR), balloon aortic valvuloplasty (BAV), and transcatheter aortic valve implantation (TAVI). This study aims to inform readers about up-to-date interventions for patients with senile calcific aortic valve stenosis in Ukraine, based on the experience of the Cardiac surgery department in Lviv, Ukraine. Methods. From a single-centre retrospective registry (10/2015-02/2022), 204 patients were included. One hundred seven patients underwent mini-SVR, tree BAV, and four – TAVI. Diagnostic modalities used to assess the anatomy of the aortic valve were: ECHO, ECG-gated computer tomography with aortic valve calcic scoring, and Angiography. Interventional procedure techniques were BAV, TAVI; surgical: mini-SVR via upper ministernotomy or right-sided minithoracotomy. Results. Anatomical assessment of the aortic valve in senile aortic valve stenosis was based on the morphology of the aortic valve (bicuspid or tricuspid aortic valve), asymmetrical hypertrophy of the left ventricular outflow tract, and coronary ostia height. The age-related anatomical features were calcium deposits in the leaflet, coronary ostium and mitral annular calcification. Ministernotomy (in 67 cases) and a right-sided minithoracotomy (in 40 cases) were performed to secure the “heart step” without instability of the chest cage with the smaller valve size implantation than was expected before. Preserving the chest cage and avoiding aortic cross-clamp/cardio-pulmonary bypass were advantages of TAVI. Fragile patients expected TAVI risks: aortic root damage, paravalvular leak, moderate aortic insufficient, the risk of atrioventricular block and embolic stroke, and kidney dysfunction. Conclusions. Mini-surgical valve replacement and transcatheter aortic valve implantation are accessible procedures in Ukraine for senile calcific aortic valve stenosis treatment.

Aortic valve stenosis remains the most dominant form of valvular heart disease. The aortic valve area below 1.0 cm2 is an assignment to the interventions. The modern senile aortic valve stenosis treatment options are mini-surgical valve replacement (mini-SVR), balloon aortic valvuloplasty (BAV), and transcatheter aortic valve implantation (TAVI). This study aims to inform readers about up-to-date interventions for patients with senile calcific aortic valve stenosis in Ukraine, based on the experience of the Cardiac surgery department in Lviv, Ukraine. \u0000Methods. From a single-centre retrospective registry (10/2015-02/2022), 204 patients were included. One hundred seven patients underwent mini-SVR, tree BAV, and four – TAVI. Diagnostic modalities used to assess the anatomy of the aortic valve were: ECHO, ECG-gated computer tomography with aortic valve calcic scoring, and Angiography. Interventional procedure techniques were BAV, TAVI; surgical: mini-SVR via upper ministernotomy or right-sided minithoracotomy. \u0000Results. Anatomical assessment of the aortic valve in senile aortic valve stenosis was based on the morphology of the aortic valve (bicuspid or tricuspid aortic valve), asymmetrical hypertrophy of the left ventricular outflow tract, and coronary ostia height. The age-related anatomical features were calcium deposits in the leaflet, coronary ostium and mitral annular calcification. Ministernotomy (in 67 cases) and a right-sided minithoracotomy (in 40 cases) were performed to secure the “heart step” without instability of the chest cage with the smaller valve size implantation than was expected before. Preserving the chest cage and avoiding aortic cross-clamp/cardio-pulmonary bypass were advantages of TAVI. Fragile patients expected TAVI risks: aortic root damage, paravalvular leak, moderate aortic insufficient, the risk of atrioventricular block and embolic stroke, and kidney dysfunction. \u0000Conclusions. Comprehensive and multidisciplinary rehabilitation is gaining momentum as a useful strategy that aims to improve physical, psychological, and social components of health in subjects affected by violence, trauma, and mental distress. Previous and current wars have prioritized essential diagnostic and rehabilitative services to civil subjects and military servicepersons which can be delivered by skilled physiatrists and allied specialists. Stratifying subjects in need of various rehabilitative procedures and offering them psychological support, balanced nutrition, musculoskeletal care, and socialisation in a safe and relaxing atmosphere may improve their mental and functional capacities and resolve numerous health issues. The choice of comprehensive rehabilitative procedures depends on their availability and understanding of complementary effects of various interventions.

METFORMIN AS A PROMISING ANTI-AGING AGENT IN THE TREATMENT OF OSTEOARTHRITIS 二甲双胍作为一种有前景的抗衰老剂治疗骨关节炎

Osteoarthritis (OA) is traditionally considered an age-related disease. Therefore, repurposing drugs with the potential to reduce cell senescence is a justified therapeutic strategy. Such is the case of metformin, the most widely used antidiabetic medicine with well-known pharmacokinetics, acceptable toxicity, and beneficial metabolic effects. Metformin could significantly impact processes associated with aging and OA such as cellular senescence, infammaging, mitochondrial dysfunction and impaired nutrient sensing. The aim of the present narrative review is to unveil the potential of metformin to modify disease course in light of aging osteoarthritic joints. The drug has pleiotropic effects on chondrocyte and extracellular matrix metabolism and may provide through AMPK-dependent and -independent pathways a meaningful improvement of OA. Mostly preclinical and retrospective cohort studies have shown that metformin exposure could lead to the regulation of cartilage homeostasis, symptomatic relief of pain and postpone surgery for those suffering from OA. Randomized control trials are warranted to justify the preliminary expectations.
