The article presents the results of the study of pons and cerebellar tissue perfusion in patients with large and giant cerebellopontine angle tumors. Perfusion computed tomography was performed in 10 patients with vestibular schwannomas and 8 patients with temporal bone meningiomas. The parameters of cerebral blood flow were calculated in 6 regions of interest in the area of the pons and the cerebellar hemispheres. There are manifestations of brain tissue hypoperfusion in the pons and hyperemia in the nearby parts of the cerebellar hemisphere. The revealed changes indicate the possibility of disorders of local cerebral blood flow after surgery.

The role of biomarkers for detecting tumors has aroused interest in their use for diagnostics as well. Biomarkers can be isolated from biological fluids, such as blood, cerebrospinal fluid, and urine. Apparently, the most promising is the study of blood plasma and serum, which are isolated from whole blood and are the most studied biological resources for the potential detection of molecular markers. This direction includes studies to identify immunological or biochemical biomarkers of the presence of tumors in the cerebrospinal fluid or in the blood of patients. Infrared spectroscopy of plasma and blood serum of patients with tumors is more promising in terms of identifying not only individual mRNAs as markers of gliomas, but also fats, proteins, and carbohydrates. The study of the profile of metabolites in blood plasma by methods of the recently emerged science of "metabolomics" has good prospects. This gives information about the biochemical processes occurring in tumors and provides knowledge for determining the treatment tactics for recurrent growth.

Modern wars and military conflicts are characterized by a high prevalence of mental traumatization due to military operations, which leads to an increase in the frequency of stress-related mental disorders formation. In this regard, the development of algorithm for therapeutic and diagnostic support of patients with this pathology at different stages of medical care, using the results of modern research methods, including neuroimaging data, is extremely relevant. The paper presents the results of neuroimaging diagnostics of post-traumatic stress disorders, describes its methodology, and highlights the changes in the functional connectivity of the brain in the examined patients compared with the control group.

The paper considers a case of surgical treatment of a patient with a completed stroke in the brain stem in anamnesis and with a clinical picture of chronic vertebrobasilar insufficiency caused by combined occlusion of the right vertebral artery (VA), hypoplasia of the left VA, and pathological tortuosity of the right internal carotid artery (ICA). The patient underwent extra-anatomical reconstructive surgery: simultaneous lowering of the loop of the right ICA and its resection in the middle third with redressement and reimplantation in the proximal third of the ICA with the formation of the ICA-ICA end-to-side anastomosis and the subsequent creation of an end-to-side anastomosis between the free end of the resected ICA and the V3 segment of the right VA. In addition, a case of surgical treatment of isolated bifurcation of the common carotid artery (CCA) that occurs when the proximal CCA is occluded and the blood flow is maintained in the external carotid artery (ECA) and ICA is considered. These reconstructive surgical interventions for combined lesions of the main arteries of the head are safe and reliable methods for the treatment of chronic cerebral ischemia and effective prevention of recurrent ischemic strokes.

Over the past few years, a significant amount of clinical data have been obtained in international and Russian medicine,indicating that patients of the older age group with established cardiovascular diseases (CVD) and overweight, sometimes even with mild obesity, apparently have a more favorable clinical prognosis of short- and long-term survival in comparison with patients who have normal body mass index (BMI). This phenomenon has been described as the «obesity paradox», but the existence of this phenomenon remains a subject of controversy. Based on the National Medical Research Center for High Medical Technologies — A. A. Vishnevsky Central Military Clinical Hospital of the Ministry of Defense of the Russian Federation (Krasnogorsk), a singlecenter prospective study was conducted. The study involved 117 patients who underwent carotid endarterectomy (CEA) after a comprehensive examination. In our article, we presented the research data on the effect of the BMI factor on the risk of postoperative cerebral dysfunction (PCD) in CEA and summarized the results of recent studies concerning this phenomenon in patients with CVD. Taking into account the data of the conducted study and world research, it is possible to make several important conclusions. Firstly, normal BMI in older patients with established CVD and planned surgery, mainly on the cardiovascular system, may be a potential risk factor for postoperative complications. Secondly, in this cohort of patients, it may be necessary to revise the reference BMI range in order to prevent the development of PCD and form a clear algorithm for preoperative preparation, including the correction of risk factors.

The article is devoted to evaluating the effectiveness of transcranial magnetic stimulation in the complex therapy of treatment-resistant depressions complicated by residual organic insufficiency of the central nervous system. For this purpose, a sample of 109 patients with this pathology was randomly divided into a main group (55 people) and a control group (54 people). The patients of the main group received a combination of antidepressants and 12 sessions of transcranial magnetic stimulation according to the iTBS protocol. The patients of the control group received only psychopharmacotherapy with a combination of antidepressants. The patients of both groups were examined at 2 and 4 weeks from the start of treatment, as well as in the catamnesis after 1 month and 6 months after the end of treatment, using BDI, BAI, CGI-S, and CGI–I questionnaires. It has been proved that the complex therapy of treatment-resistant depressions complicated by residual organic insufficiency of the central nervous system with the use of TMS sessions and psychopharmacotherapy is more effective than with isolated psychopharmacotherapy, which was confirmed by a statistically significant more pronounced reduction in the severity of depression and anxiety according to psychometric questionnaires and an improvement of the clinical condition on the CGI–I scale.

Background. Brain arteriovenous malformations (AVMs) are the most complex pathology in vascular neurosurgery. Collecting and summarizing data on the results of brain AVM treatment is extremely important.Objective. Analysis of complications and functional outcomes of the AVM microsurgery with preoperative endovascular embolization. Material and methods. The study included 56 patients with brain AVMs who underwent preoperative embolization at the N.N. Burdenko National Medical Research Center of Neurosurgery. The control group included 113 patients who underwent only microsurgical removal of AVM. Within the framework of this article, the features of preoperative embolization, such as radicality, timing and number of procedures, the choice of adhesive compositions, and the causes of functional deterioration and complications of the combined treatment are described. A comparative analysis of functional outcomes is performed by means of the pseudo-randomization method. Results. The radicality of AVM preoperative embolization was >75% in 21 patients (37.5%), 75-50% in 10 patients (17.9%), and <50% in 25 patients (44.6%), respectively. The average interval between embolization and surgery was 1.84 days (0-19 days). The mean 1.74 afferents (1-5 afferents) were embolized during one endovascular intervention. The radicality of the combined treatment of brain AVM was 98.2%; the lethality rate was 3.6% (2 cases). The functional outcomes of surgery in pseudo-randomization analysis did not differ statistically. In the discussion, the obtained results are compared with the published data. Conclusion. Preoperative endovascular embolization brings acceptable functional outcomes for complex brain AVMs; however, it has a risk of complications. Successful AVM treatment requires careful selection of patients and a multidisciplinary approach.

An overview of current trends in the study of clinical and physiological aspects of the organization of functional pain systems is presented. The pathophysiological features of the physiological and pathological functional pain system are revealed.
