Clinical Effectiveness of Composite Resin Bonded with Universal Adhesive for the Restoration of Non-Carious Cervical Lesions Using Air Abrasion: A Randomised Controlled Trial 复合树脂粘接万能粘接剂用于空气磨损修复宫颈非龋齿病变的临床疗效:一项随机对照试验

Restorative treatment is very challenging for non-carious cervical lesions (NCCLs) due to functional and structural complexities. The purpose of this randomised controlled trial (RCT) was to evaluate the clinical performance of nanocomposite restorations bonded using universal adhesive in self-etch mode with and without air abrasive surface treatment for NCCLs. A total of 70 NCCLs, from a group of consenting patients who fulfilled the inclusion criteria, were recruited for the study. The study was carried out following CONSORT guidelines. Block randomisation was done for equal allocation of lesions into; Group 1 (surface treatment with alumina air abrasion) and Group 2 (control group without mechanical surface treatment). The lesions were restored with nanocomposite using a universal bonding agent in self-etch mode. The clinical performance of the restorations was evaluated by two examiners using modified United States Public Health Service (USPHS) criteria at baseline, 3 months, 6 months and 12 months. A Chi-square test was performed for inter-group comparison. Cochran’s Q test and Dunn’s post hoc analysis were used for intra-group comparison. The inter-group comparison revealed no statistically significant difference between the experimental and control group for all the parameters assessed. With the intra-group analysis, it was found that there was a significant decrease in the performance of the restorations concerning marginal staining, marginal adaptation and surface texture during the evaluation period (p < 0.05). However, all of them demonstrated clinically acceptable performance. According to the results of this RCT, it was concluded that airborne particle abrasion of NCCLs did not improve the clinical performance of nanocomposite resin bonded using universal adhesive.

A Systematic Review of the Role of Mitochondria in Cleft Pathology: A Forgotten General? 线粒体在腭裂病理学中作用的系统评价:一个被遗忘的将军?

Orofacial clefts (OFC) are one of the most common birth defects that affects the lip, palate, or lip and palate of an infant. The deterioration of clefts is multifactorial involving multiple genes, various interactions from environmental factor and most forgotten, mitochondrial abnormality. The aim of this review is to highlight the importance of mitochondrial activity related to non-syndromic OFC deformity. Despite its important role in cells, the study on mitochondrial activity in cleft pathology was scarce and almost forgotten compared to other genetic investigations. This systematic review was completed based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) checklist. The literature search was done via the following databases: Google Scholar, Pubmed and Scopus with a total of nine studies of mitochondrial abnormalities were included. We hypothesise that mitochondria play an important role in early craniofacial development. A decreased in its function or activity may result in cleft lip formation. Hence, we would like to shed light on the remarkable role of mitochondria activity in the pathogenesis of non-syndromic OFC.

Facial Palsy with Tongue Ulcer: A Rare Initial Presentation of Granulomatosis with Polyangiitis 面瘫伴舌溃疡:肉芽肿病伴多血管炎的罕见首发表现

Granulomatosis with polyangiitis (GPA) is a rare multisystem disease. Although GPA is rare, it commonly presents in a localised stage where its manifestation involves the upper or lower respiratory tract before progressing to a generalised stage. Therefore, most patients with GPA will visit an oral surgeon or an otolaryngologist to seek treatment. However, the diagnosis of GPA is often delayed as GPA is not frequently considered as a differential diagnosis in common oral and facial diseases. The lack of gold standard investigation for the diagnosis of GPA makes management of this case, a diagnostic conundrum. We herein report a patient who was diagnosed with bilateral acute otitis media and left mastoiditis complicated with facial nerve palsy, and later developed tongue ulceration one month after his initial presentation. The ear, facial and oral symptoms represent a diagnostic red herring to a fullblown generalised stage of GPA.

Reliability of OneCeph Cephalometric Analysis Application on the Devices with Different Screen Size OneCeph头颅测量分析在不同屏幕尺寸设备上应用的可靠性

This study aimed to assess the reliability of the OneCeph application according to personal computer (PC), tablet and smartphone screen size in comparison with Dolphin software on PC as a gold standard. Cephalometric landmarks were identified on 100 digital radiographs. Twenty-four cephalometric measurements were made with Dolphin software as a gold standard comparing with OneCeph application on smartphone (OS), OneCeph on PC (OP) and OneCeph on tablet (OT). All measurements were repeated after four weeks for intra-examiner reliability with intraclass correlation coefficients (ICC). One-way ANOVA and Kruskal Wallis test were done for measurement comparison between methods (Dolphin, OS, OP and OT). Results for OneCeph on smartphone and tablet, 21 measurements were comparable with Dolphin while other three (NLA, H-angle and UL to S-plane) were not. In OneCeph on PC, 20 measurements were comparable with Dolphin while other four (NLA, H-angle, U1 to A-point and UL to S-plane) were not. All different measurements were clinically insignificant except H-angle. Intra-examiner reliability represented ICC above 0.9. In conclusion, OneCeph application on three different screen size is reliable to use for cephalometric measurement. Most of the measurements are comparable with gold standard and adequate to be utilised in clinical routine. OneCeph on smartphone and tablet are advantageous from the portable feature over PC.

Oral Care Management of Head and Neck Cancer Patients at Hospital Universiti Sains Malaysia: A 7-Year Retrospective Study 马来西亚圣山大学医院头颈癌患者口腔护理管理:一项7年回顾性研究

Head and neck cancer patients require oral assessment and management prior, during and after cancer treatment to minimise oral complications. Hence, this study aimed to evaluate sociodemographic data, oral health management, oral complications derived from cancer treatment, time frame in completing dental treatment and association between oral management and complications. Purposive record sampling of head and neck cancer patients who completed radiotherapy and/or chemotherapy at Hospital Universiti Sains Malaysia (HUSM) from 2011 to 2017 were reviewed. A checklist was used to record the data and analysed using descriptive statistics and Pearson Chi-square test. A number of 194 patient records were reviewed with majority were Malays (88.1%), males (61.3%) with the mean age of 57.18 years old. Only 21% of the patients had oral assessment prior to cancer treatment. About 84% of the referred patients were dentally fit within 1 month. Mucositis and fungal infections were the most prevalent complications. The relationship between oral assessment prior to cancer treatment and oral complications was not significant (p = 0.134). Despite the insignificant association due to several limitations, oral assessment and management prior, during and after cancer therapy are recognised as crucial for improving quality of life. A standard institutional operating procedure is recommended to improve patient care.

Social Media Use and Marketing within the Orthodontic Practice in Malaysia 社交媒体的使用和营销在正畸实践在马来西亚

This study investigated personal and professional social media use among orthodontists in Malaysia, to determine marketing strategies and to identify potential determinants associated with their behaviours. A cross sectional study using an online questionnaire distributed to members of the Malaysian Association of Orthodontists (MAO). Data were analysed using SPSS software to derive descriptive statistics and analysis of variance was applied to compare responses between age groups and working sector. Responses to open ended questions were analysed using thematic analysis. A response rate of 41% was obtained (n = 72). Almost all respondents were social media users with Facebook being the most common online platform. Smile aesthetic, known as the static and dynamic relationship of the dentition and supporting structures to the facial soft tissues, is one of the most important elements of facial attractiveness. The objective of the study was to assess the perception of smile aesthetics and attractiveness through digital image manipulation of aesthetic variables and to compare those perceptions according to diverse sociodemographic data among female Saudi laypeople attending the dental clinic. A crosssectional study of 193 female Saudi participants were randomly selected and consented to answer the study questionnaire. Nine smile photograph images were created to compare different smile aesthetic perceptions. Two groups were recruited: 120 participants in the first group (under 30 years old) and 73 participants in the second group (30 years old or above). All participants in both groups were asked to choose the attractiveness of each smile image using multiple-choice options. A statistically significant finding showed that normal buccal corridors were chosen as the most attractive smile by 42.5% of the participants in the younger group and by a significantly higher ratio of the participants with a bachelor’s degree or higher level of education at 49% (p < 0.05). Laypeople’s preferences regarding smile attractiveness vary, but a normal appearance was the ideal choice for the majority. Orthodontic treatment should consider the general sociocultural understanding of smile perception.

Oral health professionals frequently provide oral hygiene instruction (OHI) during dental visits, yet the evidence for its effectiveness is inconclusive. The main objective of this study was to examine the evidence on the effectiveness of various oral hygiene teaching approaches on periodontal health in adults. The study searched the PubMed, Google Scholar and Scopus databases for reports published between January 2000 and June 2020 in the English language. The quality of the selected papers was evaluated using Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines by two authors. Overall, five reports met the inclusion and exclusion criteria. The heterogeneity of outcome measurements across studies did not allow for direct comparison. Verbal and brochures were among the methods used in the studies. OHI was found to have short-term (up to 13 weeks) effects on dental plaque levels and/or gingiva bleeding. No significant differences were noted among the different oral hygiene methods affecting periodontal health. There is evidence that OHI is effective in reducing plaque and gingival bleeding scores in adult individuals, but it is still unclear if the effect continues beyond 13 weeks. No single method can be highlighted as the best medium of instruction.

Health-Related Quality of Life among Patients with Oral Potentially Malignant Disorder and Oral Cancer in Malaysia 马来西亚口腔潜在恶性疾病和口腔癌症患者与健康相关的生活质量

Presently there is a lack of health-related quality of life (HRQOL) measure and its corresponding utility values for oral cancer and oral potentially malignant disorders (OPMD). This limits patient-centered outcomes for cost-effectiveness evaluations. The study aimed to determine post-treatment HRQOL of patients and ascertained differences between OPMD, early and late-stage oral cancer. A crosssectional survey was conducted among patients in oral maxillofacial specialist clinics in two public tertiary hospitals. Consented participants were required to complete the EQ-5D-5L questionnaire with the EQ Visual Analogue System (VAS). Kruskal-Wallis test was used to explore differences in values between stages. Multiple linear regression was used to explore factors that influenced the HRQOL. A total of 50 OPMD and 52 oral cancer patients were surveyed. The mean EQ-5D 5L health utility values was 0.842 (n = 50, SD = 0.139), 0.822 (n = 10, SD = 0.150) and 0.626 (n = 42, SD = 0.310) for OPMD, early- and late-stage cancer, respectively. The mean values of the EQ-5D-5L index and EQ-VAS scale showed significant differences between groups and between early- and late-stage cancer with good discriminative properties. Results of the multiple linear regression indicated that ethnicity, income, residency, diagnosis, and treatment modality were able to significantly account for 25% of EQ-5D-5L utility values, F(10,91) = 3.83, p < 0.001, R2 = 0.360. Indian ethnicity, rural location, income less than RM4,360, late-stage cancer, and multi-modal therapies were all predictors of poorer HRQOL. This study evidenced disease severity and treatment modality to greatly impact the HRQOL of patients, in addition to socio-demographic factors such as ethnicity and income.

A Clinical Audit and Impact of Interventions on Antibiotic Prescribing Practices at a Public Dental Primary Care Clinic 临床审计和干预措施对抗生素处方实践在公共牙科初级保健诊所的影响

Inappropriate antibiotic prescribing in dentistry has been widely reported but local studies are scarce. We aimed to evaluate antibiotic prescribing practices among dental officers in a public dental primary care clinic against current guidelines: specifically assessing the number, appropriateness, accuracy of prescriptions, type of antibiotics prescribed and repeated prescribing of the same type of antibiotics within a specific duration. A retrospective audit consisting of two cycles (1st cycle: July to September 2018, 2nd cycle: July to September 2019) was carried out by manually collecting relevant data of patients (aged 18 and above) who were prescribed antibiotics from carbon copies of prescription books. Between each cycle, various interventions such as education through a continuous professional development (CPD) session, presentation of preliminary findings and making guidelines more accessible to dental officers were implemented. When the 1st and 2nd cycles were compared, the number of antibiotic prescriptions issued reduced from 194 to 136 (–30.0%) whereas the percentage of appropriate prescriptions increased slightly by 4.1%. Inaccurate prescriptions in terms of dosage and duration decreased (–0.5% and –13.7%, respectively) whilst drug form and frequency of intake increased (+15.7% and +0.7%, respectively). Repeated prescribing of the same antibiotics by the same officer within a period of ≤6 weeks no longer occurred. Amoxicillin and metronidazole were most commonly prescribed in both cycles. Overall, the antibiotic prescribing practices did not closely adhere to current guidelines. However, clinical audit in conjunction with targeted interventions resulted in improvement in the antibiotic prescribing patterns. Thus, further intervention and re-audit is necessary.
