Vicious Relational Circles and Chronic Couple Conflictuality: An Empirical Study 关系恶性循环与夫妻慢性冲突的实证研究

The study aims to empirically assess the control-mastery theory hypothesis that considers chronic couple conflictuality as the repetition of relational vicious circles—that is, interactions where both partners test their pathogenic beliefs and fail their reciprocal tests, confirming their reciprocal pathogenic beliefs. In addition, the study aims to verify if interpersonal guilt is more activated during couple conflicts. Our study involved 11 couples treated by four experienced therapists and nine trained, independent judges who, after reading verbatim transcripts of the couples’ psychotherapy sessions, used the Patient Scale of Couple Testing (PSCT), and the Patient Interpersonal Guilt Rating Scale (PIGRS) to rate segments of couple sessions. The results were obtained by applying generalized estimating equations and confirm our hypotheses: we could observe a greater presence of testing activity and confirmation of pathogenic beliefs in segments classified as conflictual for both partners and a stronger presence of interpersonal guilt in conflictual versus nonconflictual interactions. These findings support the idea that conflict interactions can be seen as failed attempts by both partners to disconfirm their pathogenic beliefs.

A Toolbox for Use During the Post-Pandemic Era: Preparing Youth for Re-entry 大流行后时代使用的工具箱:为青年重返社会做准备

Abstract While most individuals hope that the COVID-19 viral global pandemic is over, waves of infection and multiple mutating variants persist prompting considerable uncertainty. Re-entering previously familiar routines is especially difficult for youth challenged by excessive intolerance of uncertainty and accompanying anxiety. Therefore, constructing a conceptual map to explain these stressors and a viable skill set to cope with their anxiety are potentially helpful tasks. This is the exact focus of this article. A review of the past and current impact of COVID-19 on children and adolescents’ lives sets the proper context. Intolerance of uncertainty and health anxiety are two core constructs used to understand the psychological impact of the viral outbreak. Consequently, these variables are explained and summarized. A cognitive-behaviorally based toolkit focused on equipping youth experiencing high intolerance of uncertainty and anxious symptoms with skills for re-entry into an uncertain environment is outlined. Various processes and practices are illustrated via a representative confabulated case example.

Pitfalls and Opportunities of the Therapist’s Metacommunication: A Self-determination Perspective 治疗师元沟通的陷阱与机会:一个自我决定的视角

Abstract Psychotherapy research identifies alliance ruptures and their resolutions as significant events in psychotherapy, influencing outcome. However, we know little about the process how such events influence outcomes, only assuming if clients stay in therapy that the rupture was resolved, and the outcome will be positive. The purpose of this paper is to problematize this assumption against the backdrop of self-determination theory, introducing motivation and relational positioning as relevant theoretical concepts for understanding rupture resolution and the effect on outcome. A therapeutic transcript demonstrating best practice for alliance rupture resolution in a brief integrative therapy is critically examined, calling the attention of both clinicians and researchers to the risk of prescribing and blindly following techniques during therapeutic impasses. Our analysis of metacommunication demonstrates how the therapist’s use of a certain technique for resolving threats to the therapeutic alliance can lead to the client’s external motivation and compliance, negatively influencing therapeutic outcome. Focusing on the therapist’s relational positioning we present two alternative courses of therapeutic action, ‘mindfulness in action’ and ‘embracing the patient’s ambivalence’, for supporting the client’s autonomous motivation for the therapy process.

Effectiveness of a Multicomponent Group-Based Treatment in Patients with Medically Unexplained Physical Symptoms: A Multisite Naturalistic Study 多组份分组治疗在医学上无法解释的躯体症状患者中的有效性:一项多地点自然研究

Abstract Psychotherapy is expected to be effective in the treatment of patients with medically unexplained physical symptoms (MUPS). However, evidence is scarce. The aim of this study was to examine the effectiveness of a multicomponent treatment based on group therapy in patients with MUPS in a naturalistic setting and to explore potential predictors of the outcomes. A multisite naturalistic uncontrolled effectiveness study. A total of 290 patients with MUPS participated in group psychotherapy across seven clinical sites. Somatic symptoms, depression, anxiety, general psychotherapy outcomes operationalized as the Outcome Rating Scale (ORS) score, well-being, role functioning interference, as well as a number of pretreatment predictors were measured using a battery of self-report measures. Multilevel modeling and lasso regression with bootstrapping were used for the analysis. Medium to large pre-post effects were found for somatic symptoms, ORS, depression, anxiety, well-being, role functioning interference found in completers after controlling for site and group effects, pretreatment outcome values, and treatment length. Changes reported at 6- and 12-month follow-up were higher for most variables. No substantial pretreatment predictors of the patients’ posttreatment status were found in addition to the pretreatment level of outcome variables. Somatic symptoms seem to be less malleable in psychotherapy than psychological outcome variables. However, there was a trend of further improvement after treatment completion.
