Age and Nursing Research in Australia John McCormack At the Australian Association of Gerontology National Conference, Professor Alan Pearson from the Department of Clinical Nursing at the University of Adelaide delivered a keynote address on the topic of gerontological nursing. He pointed out that while large numbers of older people use the full spectrum of health care services, nurses with specialist knowledge and skills with this group tend to be found in only a narrow range of health care settings. He argued however that if this speciality is to advance, particularly through research, then it may be preferable for nursing researchers to concentrate in those settings where they already have a strong knowledge base, rather than spread themselves too thinly across other settings. This interesting proposition engendered lively debate at the conference, particularly in light of the expected large admission increase of older people across all health settings as the population ages. It also raised related issues about nursing research, such as how nursing can increase its research output ‘ in ’ nursing, rather than nurses having to leave direct practice to take up research positions, often in projects which do not contribute directly to the nursespecific skill base. The selection of recent articles reviewed below provides some examples of current ‘ in ’ nursing research, and draws from Australian gerontological and general nursing journals. This study examined discharge planning and continuity of care experienced by participants aged and over who had been recently discharged from an acute care setting to the domiciliary based Royal District Nursing Service. With the introduction of a capped, prospective type casemix funding to hospitals in Australia, where there is greater pressure for shorter lengths of stay, the study of this key transition is extremely important because appropriate discharge not only reduces readmission but ensures the continuity of care older people require to remain in the community. Through structured interviews, the study found that per cent of the participants felt satisfied with their hospital care and were prepared for discharge. There were however, per cent of participants who felt they were discharged too early because they were feeling weak, or anxious about coping at home. Factors contributing to discharge satisfaction included being involved in the decisions about which discharge services were required, assurance that these services would be Ageing and Society, , , –. The changing nature of family life has become a major issue in contemporary Britain. Concerns that change will bring moral decline and social fragmentation are countered by a more optimistic view which focuses on a future of more equitable and flexible family ties. Research drawing on area-based data in Luton amongst older, middle-aged and younger people with experience of family change suggests that so far as inter-generational relations, caring, and transfers of family wealth are concerned, traditional attitudes towards blood ties, household independence and care and support survive alongside new step relationships. The research also suggests that although several respondents had more than one generation of experience of family change, the language of step relationships is still one which is not yet completely accepted, or one with which people feel completely at ease.

Comparable networks surveys identified the informal relationships which provide social support to older people in urban Japan, provincial Japan, and urban Australia. Spouses, daughters, and sons were major providers of expressive support in all areas. Older Australians had more expressive support from friends while older Japanese had more instrumental support from daughters-in-law. The gender of the older people and their close ties were highly significant in all areas. The many similarities in the social support patterns contrast sharply with East and West differences in cultural prescripts and living arrangements. In these two advanced countries with long life expectancies and high living standards, older people’s interpersonal relationships may be converging on the basis of selective affection and choice, rather than obligation, with individuals in and beyond the household and family.

In this paper it is argued that the problem of formulating a comprehensive theory of successful ageing ‐ and thereby of finding good criteria for success ‐ may be attributed to the lack of a suitable multidisciplinary framework that meets at least three basic theoretical requirements. These requirements are explicated, and some existing theories and approaches of successful ageing are discussed in the light of these requirements. An alternative theoretical framework is presented which can accommodate a number of existing insights, and which is used as the basis of a new theory of successful ageing, including theory-guided criteria for success.
