Stress urinary incontinence is a complication after radical prostatectomy. The suburethral sling implantation operation is often performed for mild to moderate male urinary incontinence after radical prostatectomy. There are non-adjustable suburethral slings that are effective up to 73.7% and adjustable slings that are effective up to 92%. We present a case report of a 62-year-old man who underwent radical prostatectomy due to prostate cancer in 2018 and experienced moderate degree urinary incontinence. After non-effective conservative treatment in 2020-02-27, the suburethral transobturator sling implantation was performed in another hospital, but it did not yield effective results. After further examinations in 2023-03-25, an adjustable ATOMS system suburethral male sling implantation was performed and it was effective. 2023-05-03 ATOMS system was adjusted and after this there was no urinary incontinence. ATOMS system is an effective surgical method for the treatment of male recurrent urinary incontinence or for cases where the initial sling implantation has failed.

Introduction. Heterotopic ossification (HO) is a rare diverse pathologic process, with broad spectrum. Case report. This is a case report of a 73 years old patient who had ventral hernia at the medial line on the anterior abdominal wall, and underwent a routine surgery. During the surgery an incidental finding of HO was discovered. Discussion. HO is well documented to occur at increased frequency with certain predisposing conditions, including orthopedic surgery, most commonly hip arthroplasty. Non-genetic HO can occur nearly anywhere in the body, but is often designated by the tissue type it involves, such as myositis ossificans when involving skeletal muscle, or fasciitis ossificans when involving fascia.

Objectives of the study. Our aim was to analyse different antithrombotic drug regimens and duration in intracranial stenting procedures (stent assisted coiling, flow diverter) for unruptured aneurysms and based on the literature review from 2017–2023 to implement dual antiplatelet therapy algorithm for neuro-interventional procedures in Lithuania. Research methods and methodology. A comprehensive literature search of PubMed, BioMed Central, BMJ Journals, EBSCO Publishing, SAGE Journals Online, ScienceDirect, SpringerLink was conducted by two independent readers (MP, GŠ) for studies published from January 2017 to April 2023. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were followed. 23 studies: 6 retrospective cohort, 11 retrospective case-control, 1 prospective cohort, 1 prospective randomized-control, 1 systemic literature review, 3 metanalysis observational studies were identified. Results and conclusions. We found widespread variation in practices even among the same country centres, lending credence to the importance of a future prospective studies of dual antiplatelet drug therapy (DAPT) regimens and duration for the purpose of deriving optimal methods and streamlining tactics. Our suggested algorithm for DAPT in neuro-interventional procedures in Lithuania is provided in Graph 1.

On June, 2023, Vilnius University Hospital Santaros Klinikos started robotic-assisted operations using VERSIUS robotic surgery system (CMR, Cambridge, UK) and performed 29 robotic surgeries within the first 3 weeks. This surgical system was purchased with funding from the European Regional Development Fund. Comprehensive three-month-long learning courses were organized for a total number of 32 persons, forming 8 independent teams, each consisting of 2 surgeons and 2 nurses. Learning courses consisted of online theoretical training modules, a one-week learning course including technical skills training with a simulator and dry runs as well as cadaveric surgical cases. Furthermore, an additional learning module included dry runs in the operating room prior to the first surgeries, following live and online proctored surgical cases. This enabled a rapid and smooth introduction of the robotic surgical system into clinical practice without putting patients at risk. Abdominal surgeons, urologists, and gynecologists have already performed surgeries using the VERSIUS surgical system and no major complications have been reported. Thoracic surgeons are underway to begin thoracic robotic-assisted surgeries, completing the multidisciplinary VERSIUS surgical system implementation process. Our goal is to share initial experience with robotic VERSIUS surgical system and evaluate its implementation in Vilnius University Hospital Santaros Klinikos.

Pulmonary vein stenosis is a potential complication after radiofrequency ablation for atrial fibrillation. We present an unusual case of this complication that progressed to vein occlusion and required lobectomy and review the literature. A 54-year-old man presented with persistent chest pain, dry cough, and hemoptysis. Seven months before he underwent radiofrequency catheter ablation for atrial fibrillation. Chest computed tomography showed a narrowing of the left lower pulmonary vein after the procedure. The patient was treated conservatively. On the presentation, a chest computed tomography scan showed total pulmonary vein occlusion. A quantitative ventilation/perfusion scan revealed no perfusion to the left lower lobe. A balloon angioplasty was performed, however unsuccessfully. The left lower lobectomy was performed. Six years after the lobectomy the patient has neither cardiac nor pulmonary symptoms. Pulmonary vein occlusion after radiofrequency ablation for atrial fibrillation leading to lung resection is still a possible severe complication.

Background. Hepatic angiomyolipomas are uncommon non-cancerous mesenchymal tumors that belong to the perivascular epithelioid cell tumour group. Smooth muscle cells, fat cells and blood vessels can be observed in these types of neoplasm. Case presentation. We present the case of a 50-year-old patient. The patient was referred to the National Cancer Institute after an abdominal ultrasound, which incidentally revealed a mass in the liver. A whole body computed tomography scan showed a large liver mass, in the VII/VIII/V/VI segments. Trisegmenthomy of the right liver lobe was performed. Pathological examination revealed a rare, but benign tumour – angiomyolipoma. The postoperative course was uneventful and the patient was discharged on postoperative day 9. Two months following the surgery, the patient has no recurrence or late complications. Conclusion. Hepatic angiomyolipoma is a rare, usually benign tumour with malignant potential. It is can be diagnosed by radiological tests, biopsy or after surgery. The most common treatment is surgical resection.

Introduction. Sleeve gastrectomy is currently the most popular surgical treatment for obesity worldwide. However, there is still a lack of data on long-term outcomes of this procedure. The objective of this study was to review and analyze the scientific literature and evaluate the long-term (10 years and beyond) results of sleeve gastrectomy for the surgical treatment of obesity. Methods. A literature search was conducted using the PubMed search engine, resulting in 1 443 publications on the long-term results of sleeve gastrectomy for obesity. After applying search criteria and selecting articles by title and abstract, 7 results were included in the systematic review. Results. The included studies involved 2 399 patients after sleeve gastrectomy, with an average age of 41.8 years and 1 577 (65.7%) of all patients being women. The mean preoperative body mass index of the patients in the selected publications was 45.1 kg/m2. Long-term (10 years and beyond) weight loss outcomes were assessed in terms of excess body weight loss. Among the studies, the average excess body weight loss achieved was 54.2%, with a maximum of 70.5% and a minimum of 36%. Arterial hypertension remission was achieved in 23.6%, obstructive sleep apnea remission was achieved in 62.2%, type II diabetes remission was achieved in 54.4%, and dyslipidemia remission was achieved in 42.5% of patients. Conclusion. Sleeve gastrectomy provides a stable reduction in excess body weight and control of comorbidities over 10 years. However, more data on the long-term results of sleeve gastrectomy are needed to compare it with other surgical approaches in the treatment of obesity.

Hydatid cyst is a parasitic infestation by a tapeworm of the genus Echinococcus granulosus which can be localized in all vascularized tissues and causes cystic lesions. We present a case of hydatid liver cyst with portal vein and superior mesenteric artery invasion. Hydatid cysts are still commonly seen in Turkey. We believe that the most efficacious and least morbid treatment alternative for such rare complications of hydatid cysts is surgery.

We present a case of hydatid liver cyst with portal vein and superior mesenteric artery invasion. Hydatid cysts are still commonly seen in Turkey. Although rare, cardiac tumors are a type of surgical pathology that can present itself during clinical practice. Like the majority of neoplasms, these tumors are classified into two groups: primary and secondary or benign and malignant. However, the classification does not determine treatment, but the diagnostic pathway is the same and this pathology does not revolve only around the cardiologist an/or cardiac surgeon for optimal patient outcomes. That is why it is important for doctors of multiple specialties to work on cardiac tumor diagnostics and treatment all together. Aim of this study – to analyze the information about cardiac tumor classification, diagnostics, and treatment. Results of the study. The results clearly show that cardiac tumors are a rare type of pathology and their classification depends on type of tissue the neoplasm originated from. It can either be from one of heart layers (primary tumor) or a metastasis (secondary tumor). It is also important to classify these tumors into benign and malignant because treatment differs drastically. Diagnostic tests that are essential for cardiac tumors are instrumental (heart ultrasound, MRA, CTA) and blood tests (troponins, D-dimers). Treatment varies depending on tumor type however surgery is usually the only option. Treatment of malignant tumors is more complex, involving chemotherapy and/or radiotherapy and outcomes of these neoplasms are generally worse compared to treatment of benign heart tumors.

Inguinal hernia repair is one of the commonest general surgical procedures performed worldwide. Laparoscopic repair is associated with faster recovery, less post operative pain and comparable rates of recurrence. Using the stapling devices increases post TAPP chronic groin pain. We conducted a prospective trial for laparoscopic TAPP inguinal hernia repair on 43 patients in which we did not fix the mesh. The primary purpose of this study was to examine the rate of recurrence and chronic groin pain in patients undergoing TAPP mesh hernia repair for inguinal hernia. We observed that our rates of recurrence and chronic groin pain are comparable to those described previously in the literature. We concluded that laparoscopic inguinal hernia repair by TAPP approach can be done with acceptable rates of recurrences and decreased rates of post operative chronic groin pain.
