Comparative Study between Conventional Thyroidectomy and Subcapsular Saline Injection in Preserving Parathyroid Gland 保留甲状旁腺的传统甲状腺切除术与囊下盐水注射比较研究

Article information Background: Since the parathyroid glands are vulnerable to damage from an accidental movement or a transient, uncontrollable force, the surgeon must exercise massive care when handling them throughout the entire surgery. Aim of the work: The aim of this study is to compare the conventional thyroidectomy versus subcapsular saline injection in the incidence of hypocalcemia due to postoperative hypoparathyroidism. Patients and Methods: This is a prospective randomized study included 40 patients presented by a varies of thyroid pathology who are indicated for total thyroidectomy at the general surgery outpatient clinic and admitted to the department of general surgery, Al-Azhar university hospitals in Assiut and operated from October 2022 to December 2023. These patients were randomly divided into 2 groups: Group “A” who underwent thyroidectomy with subcapsular saline injection [SCASI]. Group “B” who underwent the conventional thyroidectomy. All patients were diagnosed to have goiter on the basis of clinical symptoms and signs, serum level of thyroid hormones and thyroid stimulating hormones, Antibody detection, neck ultrasound, and FNAC of the thyroid. Results: We compared between the two groups regarding the total Ca level all over the follow up periods, and we found that no statistically significant difference between the two groups [P > 0.05 at all follow-up periods]. However, the difference between the two groups regarding the ionized Ca was significant at the 1st week, 1st month and 6th month postoperatively [P = 0.002, 0.02, 0.03 respectively]. Conclusion: The SCASI approach may decrease the occurrence of temporary and permanent hypoparathyroidism

Correlation between The Duration of Cardiopulmonary Bypass Time and The Occurrence of Morbidity and Mortality in Conventional Adult Cardiac Surgery 常规成人心脏手术中心肺旁路时间与发病率和死亡率之间的关系

Article information Background: In the current era of surgical practice, conventional cardiac surgery still occupies the main bulk of operations, but its postoperative complications are numerous and can lead to morbidity and mortality. Cardiopulmonary bypass time is a major risk factor in cardiac surgery. Several surgeons underestimate the importance of reducing bypass time and its effect on patient outcomes, and there is no known cutoff time for bypass that is associated with higher complications. Aim of the work: To assess the correlation between bypass time and patient outcomes and clarify the relevant period or a cutoff time for safe cardiopulmonary bypass. Patients and Methods: A retrospective single-center comparative study of 450 cases of conventional cardiac surgery, 300 valve surgeries and 150 coronary bypass surgeries in the Cardiothoracic Surgery Department, Tanta University between January 2018 and January 2023, categorized into 3 groups: Group A: bypass time less than 60 minutes, Group B: bypass time from 60 to 120 minutes, Group C: bypass time more than 120 minutes. Results: There was a significant difference between the 3 groups in favor of Group A with the least bypass time regarding post-operative renal complications [p-value = 0.002], respiratory complications [p-value = 0.013], neurologic complications [p-value = 0.001], multiple blood transfusions [p-value = 0.04], infections [p-value = 0.04] and mortality [p-value = 0-001]. However, no significant difference regarding demographic data, comorbidities, creatinine level and ejection fraction. Conclusion: Bypass time is a major predictor and is proportionally related to morbidity and mortality in cardiac surgery; a bypass time up to 60 minutes is the safest with the least morbidity and mortality.

Silent Brain Infarction; Risk Factors and Effect on Outcome of Acute Ischemic Stroke 无声脑梗塞;急性缺血性脑卒中的风险因素及其对预后的影响

Article information Background: Stroke is a major contributor to disability and the second a major contributor of death globally. Silent brain infarction is an example of a subclinical risk factor for stroke that, if identified, might lead to earlier and more effective preventative measures. The aim of the work: To detect risk factors of silent brain infarctions [SBI] and its effect on outcome of first ever acute ischemic stroke. Patients and Methods: In a prospective research, 76 cases diagnosed clinically and radiologically as first ever ischemic stroke and admitted to emergency department and stroke unit of Al-Azhar University hospitals were enlisted beginning in December 2022 to May 2023. They were categorized into two groups: patients with SBIs [38 patients] and patients without SBIs [38 patients]. Results: We assessed 76 cases presented with first ever acute ischemic stroke with and without SBI. Age was significantly higher in cases with SBI contrasted with cases without SBI with male to female ratio 1: 1.5 without significant difference. There was a significant variance among both groups regarding HTN, DM, ischemic heart diseases, and the degree of stenosis at right, left carotid and vertebrobasilar arteries, the size of infarction and the assessment of cognitive function using Mental State Examination [MSE] and Montreal Cognitive Assessment Scale [MOCA] after 3 months. The Modified Rankin score after 3 months from onset was significantly different between both groups and all cases with moderate to severe and severe disability were having SBI. And there was a significant variance regarding the baseline & after one-week National Institute of Health stroke scale [NIHSS].

Ischemia Modified Albumin, Serum Uric Acid and Serum Albumin as a Biomarkers of Oxidative Stress in Acute Ischemic Stroke 缺血修饰白蛋白、血清尿酸和血清白蛋白作为急性缺血性脑卒中氧化应激的生物标志物

Article information Background: Stroke is one of the leading causes of mortality and long-term disability. Prompt diagnosis and treatment of stroke are crucial for a better outcome. A blood test, which serves as a biomarker in rural areas, will help immediately transfer patients to a hospital for thrombolytic therapy. The Aim of the work: To evaluate the role of Ischemia modified albumin [IMA] level, uric acid and serum albumin in early diagnosis of acute ischemic stroke [AIS]. Patients and Methods: This study is a case-control comparative study done on thirty ischemic stroke patients presenting with acute stroke within 8 hours of stroke onset from onset of symptoms evaluated at Al-Zhraa University Hospital, and thirty healthy individuals matched for sex and age with patients, serum albumin, serum uric acid, and IMA were measured both on admission and 48 hours after admission. Results: Patients exhibited significantly lower serum uric acid levels [2.98 ± 0.49 mg/dL vs. 5.66 ± 0.87 mg/dL, p < 0.001] and serum albumin levels [3.23 ± 0.90 g/dL vs. 5.12 ± 0.70 g/dL, p < 0.001] compared to controls. Moreover, patients demonstrated significantly higher IMA levels [20.55 [15.9 - 38.6] mg/dL vs. 14.85 [11.2 - 16.8] mg/dL, p < 0.001] compared to controls. Notably, lower serum uric acid levels [r = -0.860, p = 0.008] and lower serum albumin levels [r = -0.516, p = 0.018] were significantly associated with more severe stroke outcomes, as assessed by the modified Rankin scale [MRS]. Conclusion: The IMA and oxidative stress biomarkers may be sensitive and rapid biomarkers for screening of early ischemic stroke.
