Article information Background: The chorda tympani is frequently harmed during middle ear surgery. Surgery-related chorda tympani injury is rarely associated with impaired taste perception in patients. The use of microscopes marked the beginning of modern tympanoplasty techniques, and the endoscopic tympanoplasty approach is now described in terms of minimally invasive surgery. Aim of the Study: The aim of the current study is to compare chorda tympani nerve [CTN] injuries occurring because of tympanoplasty performed using microscopic and endoscopic methods. Patients and Methods: This prospective study was carried out on 50 patients from Al-Azhar University hospitals who suffered from chronic suppurative otitis media [safe type]. Patients were randomized equally using opaque sealed envelopes into two groups. Group A: 25 patients underwent transcanal endoscopic tympanoplasty with elevation of the tympamomeatal flap. Group B: 25 patients underwent microscopic tympanoplasty by postauricular incision. Results: Chorda tympani nerve was with no injury in 24 [96%] cases in group A and 14 [56%] cases in group B, while 1 [4%] case in group A and 8 [32%] cases in group B had stretched chorda tympani nerve and 3 [12%] cases in group B had transected chorda tympani nerve. The Chorda tympani nerve was significantly less injury, less stretched in group A than group B, and more transected in group B than group A [P value=0.012]. Conclusion: In patients undergoing tympanoplasty, Endoscopic ear surgery provides superior visualization, reduces the necessity for extensive manipulation of the chorda tympani, and subsequently lowers the occurrence of CTN injury compared to microscopic ear surgery.

Article information Background: There are many options to replace missing teeth, but a lot of people choose dental implants. It is important to know if patients know about dental implants in order to plan healthcare and marketing services. Aim of the work: To assess how much Libyan patients know about using implants as a way to replace a missing tooth, we also wanted to know what information they had about replacing missing teeth in general and where they learned it from. Patients and Methods: A questionnaire survey was conducted to collect information from 400 people who went to different dental clinics and hospitals in Libya. We gathered information about the patients' traits, their knowledge on using implants for missing teeth, and where they found out about other ways to replace teeth. Results: Out of 500 patients, 400 patients said "yes" to joining this study. A high percentage [80.2%] were aware of dental implants, with dentists being the primary source of information. The main barriers to considering implants were cost [78.8%] and perceived treatment time [19.8%]. Participants had varying estimations of the functional lifespan of implants, with the majority [37.5%] estimating it to be less than 10 years. A majority [80.2%] indicated their willingness to proceed with restoring missing teeth using dental implants. Almost half of the participants had heard about successful implant experiences from their friends. Participants had diverse views on the importance of functional outcomes and the qualifications of dentists providing implant treatment. Conclusion: A lot of patients feel that dental implants are expensive, so insurance companies and health authorities should find ways to make them more affordable. Dentists are very important as they help with replacing missing teeth by educating patients about dental implants and explaining why they are good options.

Article information Background: Digital reporting of adverse events remains the most important tool to improve pharmacovigilance information related to drugs introduced in the market with good efficacy and limited safety knowledge perceived from clinical trials. Aim of the Study: The study aimed to identify the knowledge and awareness of digital reporting of adverse drug events among healthcare professionals working at a tertiary care hospital in India. Materials and Methods: A cross-sectional descriptive questionnaire-based study was conducted with Physicians, Pharmacists, Technicians and Nurses. The questionnaire comprised items regarding awareness of pharmacovigilance and digital reporting of ADRs and perception and attitudes of healthcare professionals in digital reporting of adverse drug events. Descriptive statistics were used to analyse the data. Results: Healthcare professionals received 200 questionnaires in total, and 200 participants responded, yielding a 100% response rate among which 108 were male and 92 were female. 98 doctors, 32 pharmacists, 11 technicians, and 59 nurses comprised the group of healthcare professionals. 72% of healthcare professionals were familiar with the phrase "pharmacovigilance." Nearly 73% of healthcare professionals did not know the method of digital reporting of ADR and their nearby pharmacovigilance centers. In addition, 88% agreed that ADRs need to be reported digitally because it is easy and convenient and 92% agreed that it is their professional responsibility. Conclusion: Our study shows that awareness of pharmaco-vigilance and digital ADR reporting among healthcare professionals is relatively low.

Article information Background: There are multiple techniques used for tonsillectomy including monopolar cautery. Monopolar electrocautery creates an electrical arch that occurs between the tissue and the appliance, which cause thermal damage with subsequent post-operative discomfort. It is hypothesized that normal saline injection may reduce thermal damage during monopolar cautery. Aim of the work: To evaluate the role of intraoperative use of hypertonic saline in tonsillar fossa for reduction of patients’ pain and discomfort. Patients and Methods: This study included 50 pediatric patients indicated for tonsillectomy. Patients were divided into 2 groups; group [A]: 25 patients have electrical mono polar tonsillectomy with hypertonic saline injection into peritonsillar fossa, and group [B]: 25 patients have electrical mono polar tonsillectomy without hypertonic saline injection. The main outcomes were intraoperative blood loss, surgery duration and postoperative pain. Results: In our study, patients in group [A] had significant reduction of blood loss [P=<0.001]. Regarding pain scores, patients in group [A] showed significant decrease of VAS started 2 hours after surgery, and continued for the first 24 hours [p-value < 0.001]. Conclusion: Hypertonic saline injection significantly associated with less blood loss, decreased post-operative pain and need for analgesia. Further large-scale studies are recommended.

Article information Background: Vitiligo is a common skin condition that causes patches of lightening that affect 0.2 to 0.5 percent of the world's population. It is marked by chalky white macules because melanocytes are being selectively lost. Several ideas have been put forward about what causes the disease, such as an autoimmune, genetic, neural, biochemical, melanocyte cell self-destruction, or oxidative stress theory, which means that too much hydrogen peroxide [H2O2] builds up in the top layer of skin. Aim of the Study: We want to find out how well a special gel called "pseudo catalase" with reducing ingredients fights the over-oxygenation of skin melanocytes and epidermal cells that is made worse by excimer light exposure in a person with stable non-segmental vitiligo compared to a gel that doesn't do anything different on the same person. Patients and Methods: The study included 31 stable non-segmental vitiligo patients who attended the dermatological outpatients' clinic at Al-Hussein University Hospital in Cairo, Egypt, during the period between September 2021 and August 2022. The study was divided into 2 groups of lesions in the same patient: group A [test gel + excimer light, Narrow Band UVB exposure] and group B [placebo gel + excimer light, Narrow Band UVB exposure]. Results: The following results were recorded for our patients: The mean age was 25.7 ± 9.3 years. There were 18 males [58.1%] and 13 females [41.9%]. 22 patients [71%] were of skin type III and 9 cases [29%] were of skin type IV. The VASI score values detected showed a great difference before and after treatment, as group A was [0.42 ± 0.3] when compared with the VASI score in group B [1.12 ± 0.5]. Conclusion: Excimer-light-enhanced topical antioxidant pseudo-catalase gel is a highly efficient therapy in the treatment of stable non-segmental vitiligo.

Article information Background: Endocan is a potential marker of many immune-inflammatory conditions. It may be linked to liver cirrhosis. Increased endocan levels is linked to liver diseases [e.g., chronic hepatitis] which could be developed to liver decompensation and may be elevated in ascetic fluid. Aim of the work: The current work aimed to evaluate the role of ascetic endocan in the diagnosis of Spontaneous Bacterial Peritonitis [SBP] among patients with decompensated cirrhosis. Patients and Methods: In this cross-sectional study, 100 patients with cirrhotic ascites were selected from patients who were admitted to Nasser institute and Al-Azhar University Hospitals during the period from April 2022 to April 2023. All patients were assessed on a clinical basis, and laboratory investigations were performed. The ascetic levels of endocan were measured and compared between groups. Results: Both groups were comparable regarding patient demographics. All patients in non-SBP had negative culture, while 12 patients in SBP group had positive results [10 of E-Coli, one Klebsiella and one Streptococcus viridans]. The major cause of cirrhosis in both groups was the HCV [76% of SBP compared to 80% of the non-SBP group]. The endocan levels showed a significant increase in the SBP than the non-SBP groups [5.20±1.80 vs 2.96±1.80 ng/ml]. Endocan significantly and positively correlated with each of TLC, AST, ALT, creatinine, albumin, bilirubin, INR, CRP, HBsAg and HCV antibodies. The hazardous ratio [HR] for endocan was 1.644 [95% CI: 1.011 – 2.707]. The endocan is also predictive of SBP development with an HR 1.673 [95% CI:1.031 – 2.806]. Conclusion: Ascetic Endocan is increased in ascitic fluid and represents an independent risk factor for SBP, with a significant diagnostic value in patients with decompensated cirrhosis.

Article information Background: Hysterectomy is a gynecological surgical procedure that has been linked to raised women morbidity and mortality. The two most common problems related to hysterectomy are bleeding and pain. Hemostatic measures are constantly evolving, and LigaSure could be one of the significant milestones in this field. Aim of the work: To compare impacts of electrical bipolar vessel sealing clamp and conventional suturing method in woman undergoing vaginal hysterectomy. Patients and Methods: This was a prospective comparative randomized controlled trial, included 40 women diagnosed with benign lesions in uterus and scheduled for vaginal hysterectomy. Patients were separated into two groups; group [A]: Electrical bipolar clamp sealing method [20 patients], and group [B]: Traditional suturing ligation method [20 patient]. Groups were compared regarding operative duration, intra operative blood loss, post-operative pain and short-term complications from operation until postoperative day fourteen. Results: The LigaSure group had significant short operative duration [53.95 ± 6.94 min. vs. 62.90 ± 7.28 min.; P= < 0.001] and less blood loss [P= 0.002] compared to conventional method. Postoperative analgesia requirement at evening after surgery, day-1 and day-2 was significantly lower among vessel sealing than among conventional. Postoperative analgesia requirement at week-6 was not required in both groups. Conclusion: The use of LigaSure device can decrease operative time. When compared to traditional suture ligatures, it allows for faster, safer and more useful hemostasis
