A preliminary study on the method of single micro extrusion-based 3D printing of skin tissue 基于单微挤压的皮肤组织3D打印方法的初步研究

Objective To explore the feasibility of single micro extrusion 3D printing method for skin tissue. Methods Fibroblasts were isolated and cultured from human dermis and identified by immunofluorescence staining. Fibroblasts and HaCaT cells were used as seed cells. Gelatin, hyaluronic acid and fibrinogen were used to form scaffolds. HE staining and immunofluorescence staining were used to observe the cell distribution of 3D printed skin. The viability of cells in 3D printed skin tissue was detected by living/dead cell staining kit. Toluidine blue penetration assay was used to detect the barrier function of printed skin. Results More than 98% of the cultured fibroblasts isolated from skin dermis were vimentin positive. When the concentration of fibrinogen in the dermis and epidermis was 5 mg/ml and 1 mg/ml respectively, fibroblasts (4×106 cells/ml) printed 8 layers, and HaCaT cells (5×106 cells/ml) printed 1 layer, the prepared 3D printed skin tissue was cultured in air-liquid interface for 7 days and HE staining and immunofluorescence detection showed that the epidermis and dermis were well stratified with the thickness of epidermis being (63.5±3.5) μm. The viability of cells in 3D printed skin tissue was (90.2±0.9)% and (95.3±0.8)%, respectively, after submerge culture for 2 days and air-liquid interface culture for 7 days. After air-liquid interface culture for 7 days, the 3D printed skin tissue had similar barrier function to normal skin. Conclusion We have established a single micro extrusion 3D printing method for skin tissue, and successfully prepared tissue-engineered skin with structure, epidermis thickness and barrier function similar to normal human skin tissue, which may become an effective method for mass production of tissue-engineered skin in the future. Closed percutaneous reduction by leverage and Kirschner wire fixation for treatment of irreducible phalangeal neck fractures in children 经皮杠杆复位加克氏针固定治疗儿童无法复位指骨颈骨折

Objective To investigate the clinical efficacy of closed percutaneous reduction by leverage and Kirschner wire fixation in the treatment of irreducible phalangeal neck fractures in children. Methods From January 2016 to March 2019, 12 children with irreducible phalangeal neck fractures ware treated. The time from injury to operation was 1 to 46 days, including 5 cases of old fracture. Closed Kirschner wire puncture was used to clear callus and closed percutaneous reduction by leverage and Kirschner wire fixation were performed. The fracture healing time of the children was followed up after operation. Total active motion (TAM) system was used to evaluate the recovery of finger function. Results All the operations were completed successfully, and no infection, skin necrosis and fracture malunion occurred. All the children were follow-up for 6 to 36 months, with an average of 15.3 months. The fracture healing time was (4.2±0.2) weeks. The Kirschner wire was removed and functional exercises were performed 4 weeks after operation. The finger joint function was evaluated as excellent according to TAM system. Conclusion Closed percutaneous reduction by leverage and Kirschner wire fixation in the treatment of irreducible phalangeal neck fractures in children has the advantages of simple operation, less trauma and definite curative effect. 目的探讨经皮闭合复位加克氏针内固定治疗儿童指骨颈骨折的临床疗效。方法自2016年1月至2019年3月,对12例儿童指骨颈不可复位骨折进行治疗。从受伤到手术时间为1-46天,其中陈旧性骨折5例。采用闭合性克氏针穿刺清除骨痂,采用杠杆闭合性经皮复位和克氏针固定。术后随访患儿骨折愈合时间。采用全主动运动(TAM)系统评价手指功能的恢复情况。结果手术全部成功,无感染、皮肤坏死和骨折畸形愈合。所有患儿随访6~36个月,平均15.3个月。骨折愈合时间为(4.2±0.2)周。术后4周取出克氏针并进行功能锻炼。根据TAM系统,手指关节功能被评价为优秀。结论经皮杠杆复位克氏针内固定治疗小儿指骨颈不可复位骨折,具有操作简单、创伤小、疗效确切的优点。要严格掌握陈旧性骨折的手术指征。关键词:指骨;骨折,骨;骨折固定;克氏钢丝

Experimental study on the effect of green tea polyphenols combined with Mecobalamin on the repair of peripheral nerve injury 绿茶多酚联合甲钴胺对周围神经损伤修复作用的实验研究

Objective To investigate the effect of green tea polyphenols combined with Mecobalamin on nerve regeneration after sciatic nerve injury and repair. Methods Sixty adult Wistar rats were randomly divided into two groups, the experimental group and the control group, 30 rats in each group. Another 6 Wistar rats were taken as the blank control group. The experimental group was given green tea polyphenols 50 mg/(kg·d) and Mecobalamin 50 μ g/(kg·d) by gavage, while the control group was given Mecobalamin 50 μ g/(kg·d) by gavage for 2 weeks. Then the general condition, electrophysiological recovery, wet weight recovery rate of triceps surae muscle, the number of myelinated nerve fibers and the difference of histopathology were observed. Results The average nerve conduction velocity and myelinated nerve fiber count in the experimental group were significantly higher than those in the control group at the 2nd, 4th and 6th week after operation (P 0.05), and there was no significant difference between the experimental group, the control group and the blank control group at the 12th week after operation (P>0.05). The recovery rate of wet weight of triceps surae muscle in experimental group was significantly higher than that in control group (P<0.05). In the experimental group, the regenerated myelinated nerve fibers were evenly distributed, arranged relatively orderly, with high density and less hyperplasia of connective tissue. Conclusion The green tea polyphenols may play an important role in promoting peripheral nerve regeneration. Key words: Peripheral nerve; Nerve regeneration; Green tea polyphenols; Mecobalamin
目的探讨绿茶多酚联合甲钴胺对坐骨神经损伤修复后神经再生的影响。方法60只成年Wistar大鼠随机分为实验组和对照组,每组30只。另取6只Wistar大鼠作为空白对照组。实验组灌胃给予绿茶多酚50mg/(kg·d)和甲钴胺50μg/(kg•d),对照组灌胃给甲钴胺50mg/(kg.d),持续2周。观察其一般情况、电生理恢复情况、肌湿重恢复率、有髓神经纤维数量及组织病理学差异。结果实验组在术后第2、4、6周的平均神经传导速度和有髓神经纤维计数均显著高于对照组(P<0.05),对照组和空白对照组在术后第12周(P>0.05)。实验组腓肠三头肌湿重恢复率明显高于对照组(P<0.05),结缔组织密度高,增生少。结论绿茶多酚可能在促进周围神经再生中发挥重要作用。关键词:周围神经;神经再生;绿茶多酚;甲钴胺

Treatment of old mallet finger by folding palmaris longus tendon and pulling out wire technique 掌长肌腱折叠拉丝法治疗陈旧性槌状指

Objective To investigate the clinical efficacy of folding palmaris longus tendon and pulling out wire technique in the treatment of old mallet finger. Methods From June 2010 to June 2017, 19 cases (19 fingers) of old mallet fingers with extensor tendon and insertion defects were treated. The palmaris longus tendon was dissociated and folded, and the extensor tendon was repaired and the extensor tendon insertion was reconstructed by pulling out wire technique. Results All the wounds achieved primary healing after operation. The follow-up time ranged from 3 to 18 months with an average of 6 months. According to the Patel evaluation system for mallet finger, the results were rated as excellent in 7 cases, good in 9 cases, fair in 2 cases and poor in 1 case. Conclusion The method of folding palmaris longus tendon and pulling out wire technique for the treatment of old mallet finger is simple in material, simple in operation, good recovery of finger function and satisfactory in clinical efficacy. 目的探讨掌长肌腱折叠加牵丝法治疗老年性槌状指的临床疗效。方法2010年6月~ 2017年6月对19例(19指)老年性锤状指伸肌腱及止点缺损进行治疗。解离折叠掌长肌腱,采用拔丝法修复伸肌腱,重建伸肌腱止点。结果所有创面术后均获得初步愈合。随访时间3 ~ 18个月,平均6个月。采用Patel法对锤状指进行评价,结果为优7例,良9例,一般2例,差1例。结论掌长肌腱折叠拉丝法治疗老年性槌状指材料简单,操作简单,手指功能恢复良好,临床疗效满意。关键词:肌腱损伤;治疗效果;移植;锤的手指

Clinical application of biological membrane for reconstruction of A2 pulleys in tenolysis 生物膜在肌腱松解术中重建A2滑轮的临床应用

Objective To explore the surgical technique and clinical efficacy of biological membrane for reconstruction of A2 pulleys in flexor tenolysis. Methods From July 2015 to December 2016, 10 cases of digital flexor tendon adhesion were treated. When the pulley system could not be repaired in situ after flexor tenolysis, pulley base (firmly combined with the tissue around the tendon) and body were made from biological membrane to be used for reconstruction of A2 pulleys. After the operation, functional exercises were performed under the protection of braces. Results All the wounds achieved primary healing after the operation. No foreign body reaction, abnormal exudation, redness, swelling, heat, pain, non-union or delayed union occurred. According to the evaluation criteria of Strickland and Glogovac, the results were graded as excellent in 4 fingers and good in 6 fingers within 48 hours after operation. The patients were follow-up form 6 to 18 months with the average of 11 months. According to the evaluation criteria of Strickland and Glogovac, the results were graded as excellent in 4 fingers and good in 6 fingers. No tendon rupture occurred again. The total flexion activity of fingers was improved, ranging from 135° to 260° with an average of 200°. No bowstring deformity occurred in the affected fingers after operation. Conclusion Biological membrane could be used for reconstruction of A2 pulley, which provides a new material for pulley reconstruction in tenolysis. It is a simple and effective method. When the pulley system could not be repaired in situ after flexor tenolysis, pulley base (firmly combined with the tissue around the tendon) and body were made from biological membrane to be used for reconstruction of A2 pulleys. After the operation, functional exercises were performed under the protection of braces. \u0000 \u0000 \u0000Results \u0000All the wounds achieved primary healing after the operation. No foreign body reaction, abnormal exudation, redness, swelling, heat, pain, non-union or delayed union occurred. According to the evaluation criteria of Strickland and Glogovac, the results were graded as excellent in 4 fingers and good in 6 fingers within 48 hours after operation. The patients were follow-up form 6 to 18 months with the average of 11 months. According to the evaluation criteria of Strickland and Glogovac, the results were graded as excellent in 4 fingers and good in 6 fingers. No tendon rupture occurred again. The total flexion activity of fingers was improved, ranging from 135° to 260° with an average of 200°. No bowstring deformity occurred in the affected fingers after operation. \u0000 \u0000 \u0000Conclusion \u0000Biological membrane could be used for reconstruction of A2 pulley, which provides a new material for pulley reconstruction in tenolysis. Clinical efficacy of enhanced recovery after surgery nursing in patients with multiple severed fingers replantation 多指断指再植术中提高术后恢复的护理效果

Objective To study the clinical efficacy of enhanced recovery after surgery nursing in patients with multiple severed fingers replantation. Methods From December 2016 to June 2018, 90 patients (232 fingers) with replantation of severed fingers were divided into experimental group and control group, 45 cases in each group. The experimental group was given enhanced recovery after surgery nursing and the control group was given routine nursing. The complications such as embolism and necrosis of replanted fingers in the two groups were analyzed, and nursing satisfaction was compared. Results The incidence of complications in the experimental group was 4.44%, which was lower than that in the control group of 17.78% with the difference being significant (P<0.05). The nursing satisfaction of the experimental group was 97.78%, which was higher than that of the control group of 84.44% with the difference being significant (P<0.05). Conclusion After the replantation of severed fingers, the nursing plan of enhanced recovery after surgery was used to intervene the patients, which can significantly reduce the risk of complications, promote the recovery of the condition, improve the prognosis effect, and improve the clinical nursing quality. 目的探讨多指再植术后加强康复护理的临床疗效。方法自2016年12月至2018年6月,将90例(232指)断指再植患者分为实验组和对照组,每组45例。实验组给予术后强化康复护理,对照组给予常规护理。对两组再植手指栓塞、坏死等并发症进行分析,并比较护理满意度。结果实验组并发症发生率为4.44%,低于对照组17.78%,差异有统计学意义(P<0.05),实验组护理满意度为97.78%,结论断指再植术后,采用术后强化恢复的护理方案对患者进行干预,可显著降低并发症发生的风险,促进病情的恢复,改善预后效果,提高临床护理质量。关键词:重播;术后并发症;术后恢复增强;护理

The influence of three kinds of internal fixator on blood supply of scaphoid bone: a computer-aided study 三种内固定器对舟状骨血供影响的计算机辅助研究

Objective To study the effects of nickel-titanium memory alloy nail-feet-fixator, Herbert screw and Kirschner wire on blood supply in scaphoid bone using visual digital model of scaphoid with intraosseous vessels. Methods Twelve human forearm specimens were perfused with arterial blood vessels, scanned by micro-CT and reconstructed by Mimics 20.0 software. The reconstructed data were analyzed and compared with the digital models of three internal fixation devices. Results In this study, the visualized digital model of scaphoid with blood supply was combined with three kinds of internal fixation devices to reconstruct the composite model and construct the safe zone of scaphoid nutrient artery. Among them, nickel-titanium memory alloy nail-feet-fixator has the characteristics of less vascular interference. Conclusion The digital model can be used intuitively in the study of internal fixators. All three kinds of internal fixators are located in the safe zone of scaphoid nutrient vessels. Among them, nickel-titanium memory alloy nail-feet-fixator has less interference to internal vessels than Herbert screw and Kirschner wire. Clinical application of TriGen interlocking intramedullary nail combined with suture anchor in the treatment of Neer type III and IV proximal humeral fractures TriGen交锁髓内钉结合缝合锚钉治疗肱骨近端NeerⅢ、Ⅳ型骨折的临床应用

Objective To analyze the clinical efficacy of TriGen interlocking intramedullary nail combined with suture anchor in the treatment of Neer typeⅢ andⅣ proximal humeral varus fractures. Methods From June 2015 to June 2017, 23 patients with proximal humeral varus fractures of Neer type Ⅲ and Ⅳ were treated with TriGen interlocking intramedullary nail combined with suture anchor technique, including 15 cases of Neer type Ⅲ and 8 cases of Neer type Ⅳ. The fracture healing time was followed up. The range of motion of shoulder joint was measured one year after operation. American Shoulder and Elbow Surgeons (ASES) score, Constant-Murley score and visual analogue scale (VAS) were performed. Results All the patients were follow-up for 12 to 16 months, with an average of 14 months. All fractures healed 2 to 4 months after operation, with an average of 3 months. No complications such as wound infection, rotator cuff injury and axillary nerve injury occurred. The ASES score, Constant-Murley score and VAS were (91.00±3.04), (90.00±3.21) and (0.42±0.38) respectively. Conclusion For Neer type Ⅲ and Ⅳ complex proximal humerus varus fractures, TriGen interlocking intramedullary nail combined with suture anchor technique is used to treat it, which increases the strength of fixing the humeral nodules, conforms to the concept of overall fixation, is conducive to early functional rehabilitation, and achieves good results after operation. Application of the first dorsal metacarpal reverse fasciocutaneous flap with the superficial branch of radial nerve for repair of thumb defect 桡神经浅支第一掌背侧逆行筋膜皮瓣在拇指缺损修复中的应用

Objective To investigate the clinical efficacy of the first dorsal metacarpal reverse fasciocutaneous flap with the superficial branch of radial nerve for repair of thumb defect. Methods From January 2013 to June 2017, 14 patients with thumb defect were treated using the first dorsal metacarpal reverse fasciocutaneous flap with the superficial branch of radial nerve. The flap area was 2.0 cm×1.5 cm to 4.5 cm×3.5 cm. The donor area was sutured directly in 11 cases and covered with full thickness skin graft in 3 cases. Results All the 14 flaps were survived uneventfully without vascular crisis, necrosis and infection. The patients were follow-up for 6 to 15 months. The appearance of the fingers was aesthetic, the texture was soft, the skin color was close to the surrounding tissue, and the skin flap had good wear resistance. After the operation, the sensation of the flap recovered to S3+ with the two-point discrimination being 6 to 9 mm. The sensation of pain and temperature were recovered. Patients can use their thumbs for daily work and life. Conclusion The first dorsal metacarpal reverse fasciocutaneous flap with the superficial branch of radial nerve has the advantages of reasonable flap design, reliable blood supply, convenient cutting and simple operation. 目的探讨带桡神经浅支的第一掌背侧逆行筋膜皮瓣修复拇指缺损的临床疗效。方法2013年1月至2017年6月,应用第一掌背侧桡神经浅支逆行筋膜皮瓣治疗拇指缺损14例。皮瓣面积2.0cm×1.5cm~4.5cm×3.5cm,供区直接缝合11例,全厚皮片覆盖3例。结果14例皮瓣均顺利成活,无血管危象、坏死和感染。随访6~15个月。手指外形美观,质地柔软,皮肤颜色接近周围组织,皮瓣具有良好的耐磨性。术后,皮瓣感觉恢复到S3+,两点辨别为6-9mm。疼痛感和温度感恢复。患者可以在日常工作和生活中使用拇指。结论第一掌背侧桡神经浅支逆行筋膜皮瓣具有皮瓣设计合理、血供可靠、切口方便、操作简便等优点。它是修复拇指缺损的理想手术。关键词:外科皮瓣;治疗结果;拇指缺陷;桡神经浅支

目的 探讨游离腕横纹穿支皮瓣修复手部复杂创面的临床疗效。 方法 自2014年2月至2018年8月,我们共收治手部中小面积软组织缺损患者11例,均为累及2指以上的复杂创面,采用游离腕横纹穿支皮瓣修复指端缺损4例、中远节皮肤缺损3例、近中远节皮肤缺损4例,其余创面联合采用其他皮瓣修复。 结果 术后10例皮瓣完全存活;1例因皮瓣过长,远端坏死,经换药后Ⅱ期愈合。所有患者均获得随访,时间为3~12个月,平均6个月,游离腕横纹穿支皮瓣均外观满意,皮瓣颜色、质地、厚度与手部皮肤近似,供区隐蔽。 结论 游离腕横纹穿支皮瓣对于手部复杂创面的修复具有良好的疗效,适合临床推广应用。
