Hypothyroidism is a highly prevalent disorder and can substantially impact the well-being of an individual. Till date there is no single accurate optimal target zone for thyroid stimulating hormone concentration that exists in the context of replacement of thyroid hormone. Due to the gradually progressive nature and insidious onset of disease, it often remains unrecognized

Distinguishing sarcoidosis from pulmonary tuberculosis can be a great challenge to physicians, especially in Tunisia where there is high prevalence of tuberculosis. Both tuberculosis and sarcoidosis are granulomatous diseases; however, necrosis is commun in tuberculosis and less commun in sarcoidosis. The presence of necrosis may lead to an excessive diagnosis of tuberculosis. We present the case of a 48 years old female in which nasosinusal tuberculosis was retained face to granulomatous inflammation with little fields of necrosis. The patient was treated for many months without healing. The diagnosis was revised when evolutive pulmonary lesions were observed while the patient was under well conducted anti-tubercular treatment. Sarcoidosis was in fine retained with good response to corticosteroid therapy.

Postural analysis is a fundamental tool for evaluating the neuromusculoskeletal structure of a subject. Although it is an important tool, a unique formal analysis process has not yet been determined. The aim of this work is to propose a postural analysis protocol that can be used for both initial investigations and ongoing monitoring of the effects of therapeutic treatment in the field. A second aim of this work, equally important in the opinion of the author, is to promote discussion with other practitioners so as to make continuous improvements in the proposed protocol to provide a tool that can serve the needs of specialists even more efficiently. The proposed system consists of four subsystems: medical history data, photographic references, overall observations, and tests. Even though the initial results are encouraging, additional use of the described analysis system is considered essential to improve the technique and the skills of the operator. In this light, specific requests and observations from other professionals are essential, and obtaining these requests and observations is one of the purposes of the present article, as initially declared. The proposed postural analysis system represents a good balance of various, often opposing needs, being easily completed and reasonably fast and repeatable, making it possible to compare time series data.

Kidney transplantation is the appropriate treatment for patients with end stage kidney disease. Although it offers survival and quality of life, it is still costing and has its complications such as transplantation failure mainly due to immunological reactions. The main objective of the present study is to describe a case study of a patient with renal failure and kidney fibrosis. Kidney transplantation was recommended. The patient and his family asked for possible alternative therapeutic options. The patient was male, 26 years age. The therapeutic option was a combination of the crude extract of Ammi visnaga and the powder of dried leaves of Urtica pillulifera mixed with honey. This regimen was applied for 21 days. At the end of the regimen, both creatinine and urea were restored to their physiological limits. Taken together, it is possible to use herbs of medical importance to replace kidney transplantation.

The overuse of laboratory tests is widely prevalent in hospital settings which increases healthcare costs. Although there is previous research on this subject, little is known about the relationship between symptoms and patient requests. Our study aims to identify if adult patients with multiple symptoms ask for general investigations more than single symptom patients?

Retrospective statistical study a collection of all patients with post intra vitreal injection (IVI) endophthalmitis in last 10 year period between 2010–2019 in eye department of Tobruk medical center -Tobruk and eye department of Alwahda hospital - Derna. We aim in this study to review the microbiology, clinical characteristics, diagnosis, and management strategies of post-intra vitreal injection endophthalmitis, by comparing our results with the different studies designs and treatment protocols. There were 9 patients of the after intra vitreal injections endophthalmitis cases. The age average is 70 years, 6 cases are females and 3 casees are male, all of them were practice post intravitreal Avastin injection endophthalmitis. There were 8 patients had Diabetes mellitus (DM), 2 patients had cardiovascular disease and 1 patient had hepatitis C as a systemic disease. The most common causative organism after intra vitreous injection endophthalmitis is Coagulase-negative staphylococci (CNS) 100% from the 56% positive causative organisms and 44% are no detected microorganisms, the outcome of cases which managed with pars-plana-vitrectomy (PPV) improved significantly about 60 Letters (> 3 Lines) and cases managed without PPV > 15 Letters (> 3 Lines). The cases of post intraocular endophthalmitis which diagnosed and treated early with PPV give a good result and can save the vision. The cases of acute POE in the first 3 day had significant better function (P value = 0.02).

Accurate patient identification is key to ensure the correct administration of therapies. Mix-up of patients’identification and lack of a failsafe system remain common factors in producing erroneous pathology reports or giving the wrong treatment to patients with potential serious ramifications. This paper illustrates the importance of accurate patients’ identification and appropriate action by the clinicians on histology reports. We document 5 cases in which mistakes happened with variable consequences despite robust clinical and laboratory safety measures.
