Epigenetic and Transcriptional Modulator Potential of Epigallocatechin-3-gallate and Genistein on Fetal Hemoglobin Reactivators Genes 表没食子儿茶素-3-没食子酸酯和染料木素对胎儿血红蛋白再激活基因的表观遗传学和转录调控潜力

Background

β-hemoglobinopathies are one of the most common recessive genetic diseases worldwide, with limited treatments available, particularly in developed countries where the prevalence is higher. Pharmacological reactivation of Fetal Hemoglobin (HbF) is a promising therapeutic strategy. However, approximately 25% of the patients do not respond to Hydroxyurea (HU), the first and most commonly used HbF inducing agent approved by the FDA.

Objective

Here, we performed an in vitro assessment of transcriptional effects induced by natural bioactive compounds, namely Epigallocatechin-3-gallate (EGCG) and genistein (GN) in globin genes (HBA1, HBB, HBG1 and HBG2) in HbF regulators/silencer genes (KLF1, BCL11A, MYB and BGLT3) and in epigenetic regulator genes (DNMT1, DNMT3A, DNMT3B, HDAC1, HDAC2, HDAC3 and HDAC8). Moreover, we evaluated EGCG’s in vivo effects in hematological parameters of healthy volunteers.

Methods

K562 cells were exposed for 72 and 96 h to GN and EGCG at 100, 250 and 500 ng/mL. Cell proliferation and viability were measured, and transcriptional levels were evaluated by qRT-PCR. For in vivo assay, complete blood count was determined by flow cytometry and HbF level was determined through HPLC in 30 healthy individuals before and after 225 mg EGCG ingestion per day during a 90-day period.

Results

Both compounds impact cellular metabolism and proliferation with no cytotoxic effects. Divergent GN and EGCG effects in globin and BGLT3 expression levels suggest the involvement of divergent signaling pathways. As for the epigenetic potential, EGCG particularly affects HDAC2 and HDAC8 transcription, whereas GN significantly affects expression patterns of methylation and acetylation modulators. HU appears to have time divergent effects, with greater impact in methylation at 72 h (overregulates DNTM3A) while affecting acetylation mostly at 96 h (downregulates HDAC1 and HDAC8). Additionally, in vivo, EGCG demonstrated a modulator effect in hematopoiesis and HbF induction.

Conclusion

Our results advocate EGCG and GN with HbF pharmacological reactivation potential and sustain further research as new alternative approaches for β-hemoglobinopathies therapies.

Traditional Chinese Medicine Constitution and Clinical Data Association with Machine Learning for Prediction of Spontaneous Abortion 中医体质和临床数据与机器学习预测自然流产的关联

Background

Early prevention of Spontaneous Abortion (SA) is essential for the treatment of recurrent spontaneous abortion.

Objective

In this retrospective study, a variety of machine learning methods were used to develop predictive models and diagnose the potential risk of SA.

Methods

A total of 663 pregnant women participated in the case-control study, 586 of which were SA patients and 77 were normal parturition women. The research data included 25 features of Traditional Chinese Medicine (TCM) constitution and clinical data related to SA. This work utilized 8 machine learning techniques including logistic regression, gradient boosting decision tree, k-nearest neighbor, classification and r-egression tree, multilayer perceptron, support vector machine, random forest and XG-Boost to predict SA. The performances of the applied models were evaluated by using the method of 10-fold cross-validation and by computing the diagnostic test characteristics, including accuracy, precision, recall, F1 score, and the AUC of ROC curve.

Results

The F1 scores of these eight machine learning techniques were all above 97.5%. Among them, gradient boosting decision tree had the best prediction result on SA. The accuracy, precision, recall, F1 score, and the AUC of ROC curve of gradient boosting decision tree were 97.9%, 99%, 98.6%, 98.8%, and 97.3%, respectively.

Conclusion

The paper has accurately predicted the risk of SA combined with TCM constitution and clinical data.

Background

Pain is considered as a multidimensional conscious experience that includes a sensory component and a negative affective-motivational component. Electroacupuncture (EA) is widely used to treat pain and pain-induced negative emotions, however, little is known about the mechanisms underlying the effect of EA.

Objective

This study investigated the effect of EA on alleviating the anxiety-like behaviors in pain aversion rats and its anterior cingulate cortex (ACC) regulation mechanism.

Methods

After a Freund's complete adjuvant (CFA)-conditioned place aversion (C-CPA) model was established in rats, EA treatment (2/100 Hz, 30 min, once/day, 4 days totally) was applied at bilateral Zusanli (ST36) and Kunlun (BL60) acupoints. Von Frey filaments were used to measure changes of pain withdrawal threshold (PWT) at indicated time points. Elevated zero maze (EZM) was used to investigate the changes of pain-related anxiety and CPA was used to investigate the changes of pain aversion. The protein expression levels of GAD67, PV, and NPY in ACC were detected by Western blotting.

Results

Compared with the control group, the staying time in the "CFA-paired compartment" was significantly reduced, and the PWT was decreased in model group. In the EZM assessment, the distance and the time in open arm, as well as the number of open arm entries of model group were significantly lower than those in the control group. In the CPA assessment, the time spent in the "CFA-paired compartment" was significantly decreased in model group compared with control group, and EA reversed the changes in pain sensation and in pain-related emotions. Western blotting showed that the NPY level, but not the levels of GAD67 and PV, was significantly increased in the ACC of the model group compared to that of the control group. The increased expression of NPY in the ACC was significantly downregulated by EA, while sham EA produced no such effect.

Conclusion

EA can effectively relieve the pain and pain-related emotions, and its mechanism may be achieved by down-regulating the expression of NPY in the ACC.

METASTATIC LESION OF THE BRAIN. DIAGNOSIS AND TREATMENT 大脑的转移性病变。诊断与治疗

The article provides an overview of the current literature on the diagnosis and treatment of second-ary (metastatic) damage to the brain substance. Over the past years, Russia has seen a high incidence of malignant neoplasms, with a clearly observed upward trend. In 2019, this figure was 376.3 per 100,000 population compared to 369.6 per 100,000 population in 2018 and 293.7 per 100,000 population in 2000. However, thanks to modern methods of diagnosis and treatment, the life span of patients suffering from malignant neo-plasms is significantly increased, and, as a result, the chance of “surviving” before the onset of a metastatic lesion of the brain substance increases. Today, this formidable complication of the tumor process occurs in approximately 8-10% of cancer patients, while in most of them they are symptomatic. In some nosological forms of malig-nant neoplasms (small cell lung cancer, melanoma), the frequency of brain metastasis can reach 80% or more.

The influence of physical culture on the health of the cardiovascular system (CCC) and the possibility of using physical therapy (exercise therapy) for the prevention of cardiovascular diseases (CVD) are considered. For the prevention of CCC diseases, physical education is recommended for everyone, but especially for people with risk factors. Various forms of classes can be used. Exercise therapy for CVD has a powerful positive effect on the patient's body, improves physical and mental condition, reduces the risk of many diseases and helps to recover from them, regulates the body, increases activity and efficiency.

Dermatomyositis is an autoimmune disease characterized by damage to proximal symmetrical muscle groups, skin in the form of erythema, as well as other systems. With timely diagnosis, dermatomyositis responds well to therapy with high doses of glucocorticosteroids. However, serious complications may develop during the course of the disease, for example, aspiration pneumonia, which significantly affect the patient's management tactics and the course of the disease. This article presents a clinical case of aspiration pneumonia on the background of dermatomyositis, which significantly limited the arsenal of therapy used.

Goodpasture syndrome refers to systemic vasculitis with small-caliber vascular lesions, immunocomplex vasculitis. Immunocomplex vasculitis - vasculitis with moderate or pronounced deposits of immunoglobulins and/or complement components in the vascular wall, mainly small vessels (capillaries, venules, arterioles and small arteries). Glomerulonephritis is often observed. Diagnosis is difficult due to the rare frequency of the disease. The article presents a clinical case of Goodpascher syndrome.

The publication considers a case of ovarian malignancy in a patient suffering from 11 years of overlap syndrome: a combination of progressive systemic sclerosis with dermatomyositis (antisynthetic syndrome). Timely recognition and treatment of cancer prevented an unfavorable outcome for this patient. At the same time, the termination of the basic anti-inflammatory therapy of systemic diseases (in this case, the cancellation of mycophenolate mofetil due to the lack of the drug in the pharmacy network of the city) led to a deterioration of the course of the disease, an aggravation of the clinical picture, which can create a so-called "vicious circle" of these pathologies.
