Instrument Separation Outside the Root Canal: Handling Methods and Difficulties 根管外器械分离的处理方法与难点

In endodontics, fracturing of the instrument in the root canal can arise as an inevitable problem for thedentist, especially during the application of the metal-nickel-titanium devices to the root canal. What ismore, the broken parts of the instruments may be beyond the point of the apex, which can produce a rangeof symptoms and cause additional physical and mental pain to the patients that must be relieved as fast aspossible. A few cases are available, in which the separated endodontic instruments beyond the apex areaddressed with surgical approaches, indicating that some cases can be resolved by surgical management.However, it is difficult to achieve the ideal results often due to bleeding, a lack of clinic experience, and aninability to accurately comprehend soft tissue activities. The best way, then, is to prevent the occurrenceentirely if possible.

The Efficacy of a Novel Temporary Crown System Marginal Seal and Structural Durability in Terms of Cyclic Loading - A Pilot Study 一种新型临时冠系统边缘密封和结构耐久性在循环载荷方面的有效性-一项试点研究

Background and objectives: There has been a lack of studies regarding the marginal seal and structuraldurability of a novel temporary crown system (Hi-tempo) manufactured from a polylactic (PLA) based resinthermoplastic material especially in terms of cyclic loading. Therefore, the purpose of this pilot study wasto investigate the efficiency of this novel temporary crown system’s (Hi-tempo) marginal seal and structuraldurability in terms of cyclic loading.Methods: A crown preparation was duplicated in bovine teeth (n =15) using a copy mill machine. Thespecimens were divided into three groups and temporary crowns were fabricated that consisted of: controlgroup, Luxatemp (bis-acrylic), Hi-tempo crown, PLA based and Hi-tempo window type crown, PLA basedwith resin cusps. Marginal dimensional change was measured before and after cyclic loading with a 93.36N force, 0.63 HZ, for 45360 cycles (stimulating 21 days in vivo) in water pigmented with a red dye. Themarginal change was statistically analysed and the surface roughness was analysed using scanning electronmicroscopy (SEM).Results: Statistical significance (p<0.05) was found in the marginal changes of the control versus the Hitempo window and the Hi-tempo groups. SEM analysis revealed that the control crowns, had a muchrougher surface with micro-cracks compared to PLA based crowns which had homogenous surfaces. Allthe specimens showed leakage of the red dye into the internal fitting surfaces of the crowns.Conclusions: The novel system showed promising and comparable results in terms of structural durabilityand marginal seal compared to the conventional system.
