Cancer Treatments of the Irreversible Electroporation 不可逆电穿孔治疗癌症

Irreversible electroporation (IRE) is a kind of non-thermal tumor treatment strategy. Based on this, Nanoknife emerged, which is approved by FDA in 2011. From then on, clinical application of nanosecond electrical pulsed have showed an explosive growth. At the cell level, the biophysical mechanism of nonthermal irreversible electroporation of electrical pulsed was studied. At the animal level, it has studied the safety of irreversible electroporation and preclinical animal experiments, also the clinical application of irreversible electroporation. We reviewed Animal studies of IRE and Clinical application of IRE. Also traditional medical imaging methods such as CT, magnetic resonance imaging (MRI) and ultra-sound imaging can be used in the IRE therapy. The evidences can prove the beneficial effects and safety of IRE in tumor treatment and verify the potential of IRE in clinical treatment to some extent. However, larger clinical trials are still needed to confirm these conclusions. Hopefully, as we go deeper into IRE, we get a more thorough understanding of it and the application will appear in various fields.

Awareness is the key ingredient in psychotherapy. For instance, psychoanalysis explores unconscious conflicts and brings them to awareness. CBT explores hidden dysfunctional key convictions and brings them to awareness to challenge them and transform them into functional beliefs. In the daily practice of psychotherapy, awareness is of ultimate importance. And still, we don’t know so much about how to implicate awareness.

Effect of Auditory Neurofeedback Training on Upper Extremity Function and Motor Imagery Ability in a Stroke Patient: A Single Case Study 听觉神经反馈训练对脑卒中患者上肢功能和运动想象能力的影响:一项个案研究

Methods: A stroke patient with a left putaminal hemorrhage participated in this study. This study consisted of baseline and intervention periods (4 weeks each). The participant performed standard rehabilitation during the baseline period, and auditory neurofeedback training with motor imagery in addition to standard rehabilitation during the intervention period. The Fugl-Meyer Assessment (FMA), Motor Activity Log (MAL), Hand Laterality Judgement Task (HLJT), Kinesthetic and Visual Imagery Questionnaire-10 (KVIQ-10) scores were measured before (Pre1) and after (Pre 2) the baseline period, and then after the intervention period (Post). Results: The shoulder/elbow/forearm subscale of the FMA, the amount of use and quality of movement subscales of the MAL, the accuracy and reaction time of the HLJT, and the visual and kinesthetic subscales of the KVIQ-10 improved in the Post compared with the Pre1 and Pre 2. Conclusion: This study suggests that auditory neurofeedback training contributes to improving upper extremity function and motor imagery ability in stroke patients.

Effect of Hand Massage on Occupational Leg Swelling and Resting-state Electroencephalographic Activity: A Randomized Cross-over Study 手部按摩对职业性腿部肿胀和静息状态脑电图活动的影响:一项随机交叉研究

Background: Hand massage is recommended for leg swelling with circulatory disturbance because sitting or standing for hours leads to occupational leg swelling and increases the risk for development of chronic venous disease. Furthermore, studies have revealed that hand massage improves emotion and mood. Hence, this study aimed to investigate the effects of hand massage on occupational leg swelling and resting-state electroencephalographic (EEG) activity. Methods: This study was designed as a randomized cross-over study. Eighteen healthy women who work in a sitting or standing position were included in this study and were randomly divided into 2 groups (A and B). First, group A received hand massage to their right leg, and group B received machine massage to their right leg, for 15 minutes each. After 2 weeks, group A received machine massage to their right leg, and group B received hand massage to their right leg, for 15 minutes each. Lower leg volume, restingstate EEG activity were measured before and after each massage. Moreover, the emotion and mood states were measured after each massage. Results: Lower leg volume was significantly reduced after hand and machine massages (p < 0.05). The degree of pleasant, relaxation and refreshing was significantly higher after hand massage than after machine massage (p < 0.05). Resting-state alpha activity after hand massage significantly increased in the left anterior cingulate cortex compared with resting-state alpha activity after machine massage (p < 0.05). Conclusion: This study suggests that hand massage attenuate occupational leg swelling and increases the resting-state alpha activity in the left anterior cingulate cortex.

Lambl's Excrescences and Stroke: A Scoping Study. 兰博氏赘肉与中风:一项范围研究。

Stroke / Cerebrovascular accident (CVA) is a leading cause of morbidity and mortality in the world. Ischemic stroke accounts for 87% of the cases, 14-30% of which is attributed to cardio-embolic stroke. Lambl's excrescences (LE) were first described in 1856 by a Bohemian physician- VilemDusanLambl and is considered a rare cause of cardioembolic stroke subtype. LE are branched filiform structures with undulating movements, 1×4-10 mm in size that are usually found on aortic and mitral valves. An atheroma from LE or LE fragments per se may embolize to cerebrovascular arterial territory causing stroke. Multiple isolated cases of stroke associated with LE have been reported in the literature. We hereby report a scoping study of the findings associated with such cases. A total of 27 cases were identified after various scientific databases including PubMed and Google scholar were searched with keywords "lambi's excrescences, stroke, cerebrovascular accidents". Data from these cases were tabulated and analyzed. The mean age at presentation was 51 ± 14.2 (± SD) years with 55% of patient younger than 55 years of age. 56% of cases were males. Transesophageal echocardiogram was more effective in detecting LE when compared to transthoracic echocardiogram. LE were most often found on aortic valve and LE related stroke was most often noted in middle cerebral artery territory. Recurrent stroke was reported in 30%. Management of these cases was highly variable and likely derived from individual experience as LE management guidelines are largely lacking. Single and dual antiplatelet therapy, anticoagulation and valvularsurgery were among the various management strategies employed. We recommend dual antiplatelet after the first episode of CVA related to LE and an antiplatelet in combination with anticoagulation after the second CVA attributed to LE. Also it is reasonable to offer valve replacement after second CVA related to LE as the reccurence rate of CVA is high. Due to rarity in LE reporting and its management a shared decision making has to be made depending on the clinical status of the patient. The formation of a worldwide registry for LE using standardized reporting criteria for the diagnosis with or without incident stroke, would help establish guidelines for the diagnosis and management of this rare, yet serious disease with increased risk of morbidity and mortality.
