Background & Aims: Methylcobalamin nasal spray (NasoB12® ) has been developed to overcome the drawbacks of currently available therapeutic options like the poorly absorbed oral route and the painful intramuscular route. The present study was designed to evaluate the safety and efficacy of NasoB12® in patients with Vitamin B12 deficiency, with serum vitamin B12 (svB12) <200 pg/mL (148 pmol/L).

Advocacy for Carbomedtherapy (Carbon Dioxide Therapy) in the Treatment of Diabetic Neuropathy 倡导二氧化碳疗法治疗糖尿病神经病变

The carbon dioxide therapy is called Carbomedtherapy for diseases, and carboxytherapy for aesthetics. The CDT consists of transcutaneous injections of carbon dioxide [5-10]. This technic was first practiced, in 1932, by Dr Barrieu, at Royat-Chamalières, France. Initially the indications were purely vascular especially for Raynaud’s syndrome [11], arteriopathy of the lower limbs, leg ulcers, and of course, in the care of the diabetic foot.

TRAB Measurements: Ready for Prime Time? TRAB测量:准备好迎接黄金时段了吗?

[7] compared an automated 3rd generation TRAb (RAD 120; Radim, Pomezia, Italy) against a second 2nd generation TRAb immunoassay (Lumitest TRAK human; Brahms, Berlin, Germany). The ROC plot of the 3rd generation assay was excellent (AUC 0.994), with a lower optimal threshold of 1.25 U/L compared to second generation value of 1.99 U/l. The 3rd generation assay had a higher sensitivity of 97.6% versus 95.1% for the 2nd generation assay. The 3rd generation assay also had greater analytical precision with a faster turnaround time.

Comparison between Fine Needle Aspiration Cytology of Thyroid Nodules and Histology after Surgical Excision 细针穿刺甲状腺结节细胞学与手术切除后组织学的比较

Background: Thyroid nodules are an extremely common disorder. Fine Needle Aspiration (FNA) is the gold standard procedure for thyroid nodules diagnosis and management; it helps clinicians to make a proper therapeutic decision and minimizes the need for unnecessary surgery. Objective: To assess thyroid FNA sensitivity, specificity, accuracy, Positive Predictive Value (PPV), and Negative Predictive Value (NPV) in Al-Assad University Hospital using the Bethesda system for reporting thyroid cytology. Methods: We prospectively followed thyroid FNAs, then the histological results were followed after performing surgery. FNA was performed by endocrinologists in al-Assad and al-Mouwassat university hospitals, and the cytological study for all samples was performed in pathology department at al-Assad university hospital. All available data were analyzed with SPSS version 23 statistical program. Results: A total of 381 nodules from 324 patients, 281 women (86.7%), aged between 13-80 years (46.1 ± 13.01) had FNA. There were 26(6.8%), 216(56.7%), 52(13.6%), 44(11.5%), 30(7.9%), and 13(3.4%) nodules in categories unsatisfactory, benign, Atypia of undetermined significance/ Follicular lesion of undetermined significance AUS/FLUS, Follicular neoplasm/Suspicious for Follicular Neoplasm (FN)/(SFN) Suspicious for Malignancy, and malignant, respectively. Histology was obtained for 172 nodules, sensitivity, specificity, PPV, NPV, and accuracy were 85%, 79.35%, 47.22%, 96.05% and 80.36%, respectively. Malignancy rate in unsatisfactory, AUS/FLUS, and FN/SFN cytology was 0%, 9.1%, and 6.5%, respectively. Conclusion: Specificity was slightly lower than anticipated based on global data and so did the PPV that may be due to the trend of overestimating cellular irregularity, however fine needle aspiration biopsy remains an accurate procedure in excluding malignancy due to its high sensitivity and NPV.

Effectiveness of the 6 × 6 Dieet® in Obese DMT2 Patients Effectiveness of a Very Low Carbohydrate Ketogenic Diet Compared to a Low Carbohydrate and Energy-Restricted Diet in Overweight/Obese Type 2 Diabetes Patients 极低碳水化合物生酮饮食与低碳水化合物和能量限制饮食在超重/肥胖2型糖尿病患者中的效果

Background: More than a million people in the Netherlands suffer from type 2 diabetes mellitus; 80 percent is overweight or obese. Weight loss through diet is important; low carb diets are increasingly popular in diabetes management. Objective: From preliminary results we concluded that the 6 × 6 dieet® (a very low carbohydrate ketogenic diet) was more effective in patients with type 2 diabetes and overweight or obesity than other diets. To prove its effectivity, we compared the 6 × 6 dieet® (6 × 6) to a moderate Low Carb Diet (LCD) and an Energy-Restricted Diabetes diet (ERD). Design: A retrospective three-arm study in dietitian practices to reduce weight; HBA1C; and use of medication in overweight/obese adults with type 2 diabetes on 6 × 6 (VLCKD); a 50-100 grams/day LCD; an energy restricted diabetes diet at 3, 6 and 12 months. Results: Data of 344 (n=110; 123; 111 in each study arm) patients from 16 practices were analyzed. More weight loss was seen in patients on 6 × 6, both at a 3, 6, and 12 months follow-up than in patients on LCD or ERD (all p-values <0.05). A total of 34.5% of patients on 6 × 6 turned their HbA1c levels to a value <43 mmol/mol after 12-months follow-up which was a higher proportion than in the LCD study arm (p=0.14) and the ERD study arm (p<0.01). The percentage of patients that stopped or reduced their use of Metformin, SU derivatives or Insulin was larger in the 6 × 6 study arm than in the LCD ad ERD study arms, reaching statistical significance when comparing 6 × 6 with ERD. Conclusions: 6 × 6 was more effective than LCD or ERD in overweight or obese patients with type 2 diabetes regarding weight loss, remission of type 2 diabetes, HbA1c-levels, and the reduction of diabetes medication.
