Minimally invasive dentistry combining orthodontic therapy and single-unit restoration. 微创牙科结合正畸治疗和单单元修复。

Today's restorative materials enable dental professionals to deliver predictable aesthetic enhancement, particularly for compromised teeth in the anterior region. Treating a single compromised tooth, however, requires a thorough understanding of the restorative material's optical properties, experience in its clinical performance, and awareness of therapeutic modalities that conserve sound tooth structure. In this presentation, the authors demonstrate the use of orthodontic therapy and careful treatment planning to restore a single central incisor using a minimally invasive approach.

Implant dentistry is changing. There are currently two types of computed tomogrophy (CT) scanners--multi-slice and cone beam--available to the dental professional. Computed tomography allows for proactive planning among the entire implant team and with the patient, a concept referred to as "collaborative accountability" In addition, CT surgical guidance that enhances accuracy and precision is available to ensure prosthetic outcomes. A logical and progressive approach is outlined that allows each clinician to assess how to embrace this paradigm shift in his or her clinical practice, and provide better and safer patient care.

Range and mean discordance of individual tooth width of the mandibular anterior dentition. 下颌前牙列单个牙宽的范围和平均不一致。

Proper diagnosis of tooth size for each patient is critical in treatment planning for restorative dentistry. The purpose of this study was to find a clinically relevant and applicable range and mean discordance of individual tooth width of the mandibular anterior dentition. The central incisor (CI), lateral incisor (LI), and canine (CA) teeth varied in range from 4.5 mm to 6 mm, 4.5 mm to 7 mm, and 5.5 mm to 8 mm, respectively (N = 417). Nearly 90% of the patients fell within +/- 0.5 mm of the combined gender normative values. The results suggest that there exists on average discordance between normative values and actual tooth width of the mandibular anterior dentition in a population of male and female patients.

Prosthetic replacement of the missing single maxillary central incisor with an implant-supported crown represents a profound aesthetic challenge for the restorative dentist, laboratory technician, and surgeon. In addition to the visual fidelity of color, translucency, contour, and surface texture, the proper soft tissue outline is sacrosanct to the illusion of a natural tooth. The contrast between the uniformly round shoulder of the implant and the tooth's curvilinear cementoenamel junction is particularly problematic. This clinical report demonstrates a simplified method that precisely controls the facial gingival and proximal soft tissue contours for implant-supported, metal-ceramic crowns in the aesthetic zone, using the cervical anatomy of the maxillary incisor tooth as a guide. A new role for the provisional crown that is intended to maximize the volume of keratinized tissue is also described.

Identification of etiological factors is essential for successful management of tooth wear. In many cases, the diagnosis may be complicated because of the multiple etiologic factors which may confound the clinical appearance of tooth wear. Comprehensively addressing all of these factors is paramount to the long-term success of patient management.

Although direct composite resin procedures yield acceptable results when used correctly, if a large quantity of resin is required, complications may arise. In such cases, an indirect protocol must be followed. Numerous CAD/CAM systems, both laboratory and chairside, are available for practitioner use. In a laboratory system, a traditional impression is made and sent to a dental laboratory, where the restoration is fabricated. A chairside system, however, utilizes a digital impression system, and the restoration can then be milled and cemented in a single visit.
