A classification of predicted vertical height of the interdental papillae has been developed based on whether a papilla has a tooth, a pontic, or an implant on either side of it. Based on this classification, it may be prudent to avoid the placement of adjacent implants in multiple-unit restorations. To further enhance the aesthetics of multiple-unit restorations, endodontically retained roots may be submerged between implant sites. Some patients, however, require adjacent implants due to limited bone support. Others request them for personal reasons. In these cases, additional efforts must be made to create naturally appearing soft tissue architecture with full and symmetrical papillae.

In recent years, advancements have been made in CAD/CAM technology that have allowed for the development of different treatments regarding the rehabilitation of patients with natural dentition, as well as patients with dental implants. Contemporary systems can also allow prosthetic rehabilitation for partially and completely edentulous patients. This article describes a restorative alternative to fixed implant-supported reconstruction of completely edentulous patients, utilizing a CAD/CAM-generated framework and CAD/CAM-generated all-ceramic cement-retained crowns. In addition to delivering an optimally aesthetic restoration, this design permits a precise and passive prosthetic fit.

In this article, the author proposes objective definitions in one area of tooth appearance that has historically been subjective and relative, without standardized descriptions. This concept has evolved from the need for a classification system that would enable defined, reliable, and repeatable communication between the dentist and ceramist. Following detailed observations of natural teeth, categories of appearance attributes have been developed with classifications assigned in each category. These definitions are comprehensive and provide a utilitarian description of all essential features that comprise tooth appearance other than shade. Subjective vagaries in the communication process between dentist and ceramist can be eliminated while the time, effort, and cost associated with delivering high-quality aesthetic restorations can be reduced.
