Synovial sarcoma arises primarily in the lower extremities with a predilection for sites in proximity to large joints, such as the knee. It rarely occurs in the head and neck region, and the tonsil is an unusual site for the tumor, with only eight previously published cases in this anatomical site. 6–10 % of all soft tissue tumors, only 3–5 % arise in the head and neck region, and SS arising in the tonsil is a rare finding, with only eight well-documented cases reported in the English language literature. We describe the histopathological findings of a primary synovial sarcoma of the palatine tonsil arising in a 20-year-old male, with presentation of dysphagia and sleep disturbance.

When thinking of smell, we usually consider it only as one of the five senses. Compared to the rest of senses, smell has been underestimated. By conducting a research, we will be able to discover how mistaken we are. The human sense of smell is more powerful than it's usually given credit for and plays a major role in human health and behavior than many experts in the field are aware of the sense of smell is present in our daily activities and, depending on the gender, perception may differ, as well as from the anatomical structure of the olfactory organ between genders to the stimulus of the odor. The fragrances determine our everyday food choices, places where we choose to spend with our life partner. Scents can determine our sexual behavior, in building infant-parent connection and create our habits as well. It can also warn us against the selection of food, persons, also death. In this case, the loss of smell can be related with an olfactory organ dysfunction, but as well can have an endocrine, genetic or psychical basis. The olfactory dysfunction can change our habits and basically our whole life. It is irrefutable that while talking about smell which we consider as one of the senses, we can talk about sensuality which broadly covers the relationship between smell and perception of reality. Sensuality in a general sense can be defined as the perception of the surrounding things through the senses, as sensory pleasure, and unlike sexuality, which can be translated as the reception of biological, psychological and physical stimuli. Aim of the study: Currently all of our senses are mostly well known. We are trying to consider the topic from many different sides to make ourselves more and more advanced. We are trying to unite a couple of subjects to prove that we can connect one with another to explore how advanced our organism is. In this research we are trying to look closely at the two different topics and glue them together. Trying to improve and update the connection between the sense of smell and sensuality. Melkersson-Rosenthal syndrome (MRS) is a rare disease, with unknown etiology characterized by oro-facial oedema, fissuring tongue and peripheral facial palsy. The mainstay treatment is corticosteroids. We report the case of a 38-year old female, who presented six recurrent episodes of left peripheral facial palsy associated with simultaneous oedema of the median frontal area, treated successfully by short course oral corticosteroids with neuromuscular facial re-education. However, the recovery of the last episode was incomplete and the patient kept a left House-Brackmann grade II facial palsy at six months. The median frontal area was slightly inflammatory, being the location of simultaneous repetitive oedema and the tongue was fissured. These cardinal symptoms realise the complete triad of Melkersson-Rosenthal syndrome (MRS). The histopathological examination of the lip biopsy showed lymphocytic inflammation around the blood vessels. Since the residual facial palsy was graded as mild dysfunction, the frontal oedema fully recovered and the fissured tongue was not painful, the treatment consisted on appropriate neuromuscular re-education. The 6 months follow-up showed no recurrence with a stable grade II left facial palsy. We present this case to supplement the rare literature data concerning the management of this rare entity. Patients should be prepared to the risk of recurrent episodes with longer duration of symptoms and more incomplete recoveries, which may indicate other therapeutic options.

Aim/Background: Individuals deprived of full hearing ability suffer from many problems in their social, professional, and personal lives. Prolonged hearing deprivation, particularly in children, results in many consequences including loss of control of one’s voice. Impaired hearing control of the voice leads to dysphonia of different degrees. The aim of this study was to analyse acoustic voice parameters in hearing-impaired, school-aged children (7–12 years) and the influence of treatment type on voice quality. Material and method: The subjects in this study were a group of 83 Polish school children, aged 7–12 years. ENT and phoniatric examination were performed on each subject prior to inclusion. Acoustic voice analysis was performed for each individual. The linguistic material was a prolonged [a] vowel. The acoustic analysis was performed with a digital spectrograph KAY 4300B for multidimensional voice analysis MDVP (Multi-Dimensional Voice Profile). The criterion to include patients in the study group was a profound prelingual, sensorineural hypoacusis. Within the study group, four subgroups were categorised according to the use of hearing devices: 20 children without any type of hearing device (HL), 20 children with hearing aids (HA), and 20 children with cochlear implants (CI). The children using hearing aids and cochlear implants were included in the study 6 months after the hearing rehabilitation was implemented. The control group (NH) consisted of 23 normal-hearing children, in the same age group, without any co-existing voice disorders. The statistical level of significance p < 0.05 was used. Results: The selected, clinically significant voice parameters were analysed for the groups of children in the study.

Objectives: to assess voice changes of patients after thyroidectomy without inferior and superior laryngeal nerves injury according to the intraoperative neuromonitoring. Material and methods: This monocentric study included all patients who underwent thyroidectomy with laryngeal nerves neuromonitoring between September, 2018 and December, 2019. The voice assessment was performed 1 month before and 3 months after surgery. It was both subjective (with Voice Handicap Index 10 and GRBAS scale) and objective (Dysphonia Severity Index). Voice was considered impaired if there was an increase ≥ 4 points of the Voice Handicap Index 10 score or ≥ 1 point of the GRBAS scale score or a decrease ≥ 2 points of the Dysphonia Severity Index score between the two vocal assessments. A composite score was created, combining both subjective and objective scores. Predictive factors of altered voice were seek. Results: Fifty-nine patients were analyzed. Mean Voice Handicap Index 10 varied from 3.39 ± 5.40 to 2.90 ± 5.29 (p = 0.62) before and after the surgery respectively, mean GRBAS from 0.64 ± 0.76 to 0.73 ± 0.85 (p = 0.57) and mean Dysphonia Severity Index from 8.47 ± 1.15 to 8.31 ± 1.03 (p = 0.42). Voice impairment composite score identified 14 patients with an impaired voice (23.7%). Age ≥65 years, preoperative Voice Handicap Index 10 score ≥ 3 and thyroidectomy past history were independent risk factors of voice impairment. Conclusions: the voice quality 3 months after thyroidectomy was not significantly subjectively and objectively changed compared to before thyroidectomy. When grouping voice impairment scores, age ≥65 years, preoperative Voice Handicap Index 10 score ≥ 3 and thyroidectomy past history were independent risk factors of voice impairment.

Objectives:Nasal septal deviation (NSD) is a common dilemma which cause nasal obstruction. Septoplasty is usually accomplished to solve nasal obstruction in patients with NSD. NSD is related strongly with hypoxia. Hypoxic conditions increase oxidative DNA damage in cells. This study aimed to investigate the effect of septoplasty on the level of antioxidant agents in patients with marked NSD. Methods: Sixty-one patients who submitted to septoplasty were involved in our prospective investigation. Results: The difference between the pre- and postoperative levels of MDA and 8-OHdG/106 dG was statistically significant. Conclusions: Septoplasty is an elective surgery performed to prevent the decrease in quality of life due to nasal obstruction and frequent respiratory tract infections in patients with NSD. As seen in this study, septoplasty is an urgent surgery that should be performed urgently to prevent DNA damage in patients with NSD

Infectious mononucleosis is caused by Epstein-Barr virus, which infects more than 98% of the world's adult population. Approximately 90% of adults become antibody-positive before the age of 30. The ages of 15-24 years are the most likely time for the onset of symptoms. We report the case of a 13-year-old girl who came to our emergency department for bilateral erythematous-poltaceous tonsillitis complicated by dysphagia and dyspnea. She had a diagnosis of infectious mononucleosis. For some hours the young woman presented an inconsistency between a marked respiratory distress and the patency of her upper airways. Finally the clinical picture fell suddenly, requiring an emergency tracheotomy to ensure a patent airway. In the following days a diagnosis of a left parapharyngeal abscess was reached. A tonsillectomy with drainage of the parapharyngeal abscess was required. It is well known that infectious mononucleosis is typically a silent infection. We describe the case of a girl who risked losing her life due to aggressive infectious mononucleosis, and how we treated her. Such a complicated case has never been described in Literature.

Sino-maxillary mucormycosis is an opportunistic, aggressive, invasive and fulminant fungal infection, which has the ability to rapidly cross anatomical barriers, causing extensive hard and soft tissue destruction, leading to significant cosmetic deformity, functional debility and morbidity. It usually affects susceptible, debilitated or immunocompromised individuals. A comprehensive Review Paper is presented, with a descriptive report on an unusual Case presentation, involving the dentoalveolar complex which thereafter invaded the maxillary antrum, in an otherwise healthy, adult patient with nil comorbidities. A probable intraoral portal of entry, and a likely iatrogenic etiopathogenesis resulting from inoculation of the fungus or its spores during dental extractions carried out in an unsterile setting, has been considered and elucidated. An emphasis has been placed on according a high index of suspicion in diagnosis of this fungal infection even in immunocompetent and healthy adults, presenting with a seemingly simple, deceptively indolent odontogenic infection. The importance of careful, albeit rapid differential diagnosis, and a prompt and aggressive medical-surgical treatment protocol instituted immediately following a confirmatory diagnosis, helps in limiting the spread and degree of destruction which can be caused by this potentially lethal fungal infection.

Introduction: Evaluating vocal cord mobility is important both before and after thyroid surgery but can result in excess hospital appointments which is unfavorable in the current COVID-19 era. Aim: To describe and demonstrate a successful adaptation during the COVID-19 pandemic using a portable and disposable flexible laryngoscope for vocal cord assessment before and after thyroidectomy. Methods: A description of practice over a 6-month period where the described adapted method was used for 26 thyroidectomy cases. Results: Assessment of vocal cord mobility was performed in all cases with the portable disposable flexible laryngoscope without any issue and no adverse events were encountered. Conclusion: The authors advocate the use of a portable disposable flexible laryngoscope used twice for each patient pre- and postoperatively in thyroid surgery for perioperative vocal cord checks due to the significant advantages in practicality, efficiency and safety; particularly during the covid-19 pandemic.
