Pleomorphic adenomas represent 80% of the salivary gland’s benign tumors. They are most often found in the parotid gland or in the submandibular gland. Pleomorphic adenomas of the nasal cavity are rare, with less than fifty cases reported to date. There are no treatment or follow up guidelines. The purpose of this article is to review the cases already described in the literature and to share our clinical experience. We describe the case of a 38-year-old woman with a history of a slow growing intranasal tumor with recurrent epistaxis, obstruction, and aesthetic deformation. The pre-operative assessment suggested a benign tumor, but the biopsies were inconclusive. The decision was taken to perform an open rhinoplasty to have an en bloc resection with margins control. The diagnosis of pleomorphic adenoma was established on the excised tumor. There were no post-operative complications. The early follow-up showed no signs of recurrence. We decided to closely follow the patient with frequent clinical examinations and yearly enhanced-MRIs for at least five years due to the recurrence and malignant transformation risks.

Aim: The aim of this study is to spot the light on palatal diseases and their prevalence in adults from otolaryngologists' point of view. Materials and Methods: This is an observational study that was conducted on adult patients presented to otolaryngology department of Cairo university hospital in the period from January 2016 to January 2018. Patients were routinely examined by the author however the chief complaint either related to palate or not. Results: Out of 4800 patients have been examined over 2 years, 289 cases have been affected by palatal disease or lesion either primary or secondary. It was clearly noted that the most common pathology affecting the palate is the gastric reflux, 248 patients; the remaining 41 cases have been affected by different diseases of variable etiologies. Conclusion: The palate is a part of routine examination by otolaryngologists. It may carry signs of different diseases and lesions which are not uncommon. The author observed palatal diseases in 6% of the study's sample. Prevalence of those diseases has been entailed in this study.

Objective: oral appliances are one of the treatments of obstructive sleep apnea hypopnea syndrome. They all have dental anchoring points, causing potential changing in the dental occlusion with time. The aim of the study was to evaluate dental displacement induced by Narval® oral appliance in the treatment of the adult mild to severe obstructive sleep apnea hyponea syndrome. Material and methods: The study was based on 15 adults with mild to severe obstructive sleep apnea hypopnea syndrome treated with a Narval® oral appliance for more than one year. Before oral appliance delivery and more than one year after oral appliance delivery, dental impressions were performed and digitized by a 3D scanner. The dental positions were calculated by the software 3D Reshaper® and compared. Results: The mean wearing time of the Narval® oral appliance was 13.5 months (±2.1). The mean mandibular advancement was 6mm (±2). For the maxillary arch, there was a significant distalization of molar and premolar-canine blocks from 0.12 to 0.20mm (all p<0.05) and a significant proclination of incisal blocks of 0.18mm (p<0.05). For the mandibular arch, there was a significant mesialization of the molar and premolar-canine blocks of 0.14 to 0.25 mm (p<0.05) and a non-significant retoclination of the incisors of 0.09 mm. Conclusions: The Narval® oral appliance produced minimal but significant teeth movements on the maxillary and mandibular at the short and medium term.

Background:Correct nasal spray techniqueand patient compliance are the two mostimportant factors in the use of steroid nose spray. Objectives:To find out if patientsuse steroid nasal spray with a correctspray technique and if they are compliant in using the spray. Design: Patient-reported survey Participants: 100 participants (1 excluded) Main outcome measures 1) Nasal spray technique 2) Patient compliance Results: Out of 99 participants included in this study only one patient used steroid nose spray with a completely correct spray technique. 40% of patients received guidance instructions from the prescriber about the spray technique. 70% of patients reported using their spray regularly and 53% of patients were found using the correct dosage of the spray. Conclusions: Wrong spray technique and patient noncompliance is common and can be corrected. This survey emphasizes the importance of giving written and verbal instructions about the use of spray to the patients.

The availability of effective antibiotic therapy has tremendously reduced the incidence of otogenic lateral sinus thrombosis (OLST) and changed its clinical presentation to a subtle one. The nature of predisposing otogenic disease has also changed from acute otitis media to chronic suppurative otitis media (CSOM). The prevalence of CSOM is still high in low-income economies. Objectives: Improvement in anaesthesia has allowed thyroidectomies to be performed mainly under general anaesthesia. There is however a growing interest in performing thyroid surgery under local or regional anaesthesia. The objective of this study was to analyse and share our experience with safety of thyroidectomy under regional cervical plexus block/ local anaesthesia in a tertiary referral hospital in Ghana. Materials and Methods: A retrospective study was conducted on all patients who had thyroidectomy under local anaesthesia from 1st January 2017 to 31st May 2018 in KATH. Data collected were demography, grade of goitre, operating time, and duration of hospital stay, complications and cost effectiveness of the procedure. Data was analysed using Stata version 16.0software. Results: A total of 105 thyroidectomies were done in the study period out of which 16 were done under local anaesthesia. All 16 patients (16 females, 100%) and majority 11 (68.75%) were aged between 30 and 50 years. 11 (68.75) had grade IB goitres. The most frequently performed surgery was thyroid lobectomy 12 (75%) and in 75% of cases the surgery was completed between 60 and 90 minutes. Most patients, 9 (56.25%) were discharged home within 48 hours following surgery. The cost of treatment was averagely 30% less compared to same surgery under general anaesthesia. No complications were recorded in the post-operative period. Conclusions: Comprehensive clinical assessment and careful patient selection for thyroidectomy under local anaesthesia result in good surgical outcomes. The procedure is safe and cost-effective and should be performed by experienced surgeons for the best outcomes.

Objective: the aim is to evaluate the incidence of postoperative throat pain, nausea and vomiting in patients that have been packed with either conventional gauze or pharyngeal tampons. Methods: We included adult patients who were booked for a rhinology surgery that needed throat packs. They were allocated into two groups, pharyngeal tampons, and conventional ribbon gauze. They were then assessed using visual analog scales and Post-operative nausea and vomiting impact scale respectively in the 1st, 4th, and 24th hour. Data was analyzed using SPSS software version 25. Results: There is evidence to suggest that using pharyngeal tampons reduces the mean VAS score at 4 hours compared to using Gauze. [t87=3.294, p=0.001), the is no statistical evidence that show a difference in the other Visual analogue scores or post-operative nausea and vomiting scores. Conclusion: The study shows that the use of pharyngeal tampons is associated with decreased pain scores, however, it also shows that there no difference in postoperative nausea and vomiting.

Methodology: 352 patients with Head & Neck disorders were considered for the study over a period of 1 year who were evaluated clinically and radiologically. After initial evaluation, most of them were referred to higher centre for further management. Results: Nearly 75% of patients presented with malignancy with 5 times predominance in males and around 97% patients belonging to the region from oral cavity up to larynx as they have a common epithelial origin. Conclusion: Head & Neck disorders are never to be neglected as it the “runway” to the crux of the body system. Any sort of limitation or hesitation can cost life and well-being.
