The purpose of this study was to determine the frequency of HSV infections and recurrences among HIV-infected patients and to examine different regimens for suppression of HSV recurrence. A randomized retrospective chart review of HIV-infected patients at a public hospital in Los Angeles County was conducted. We reviewed 224 patients' charts; 26 percent had AIDS based on the 1987 CDC definition. HSV infection was documented as a clinical event in 51 records (23 percent). Patients with an AIDS diagnosis had a greater incidence (53 percent) of HSV infections than did those with a diagnosis of symptomatic or asymptomatic HIV infection (p < 0.001, Fisher's exact test). Recurrences of HSV occurred in 26 (51 percent) of the 51 HSV-infected persons during a period of 1042 patient months. Eighteen patients who had received acyclovir suppression at 600 mg/day had three HSV recurrences in 382 patient months, whereas 14 who received 400 mg/day had eight recurrences in 282 patient months (p = 0.02). HSV infections occur in 23 percent of HIV-infected patients, increasing to 53 percent in AIDS patients. Acyclovir suppression prevents recurrent HSV, and a dosage of 600 mg/day is more effective than 400 mg/day.

The use of alternative treatment modalities by persons with HIV/AIDS poses many ethical problems. Because the medical community and the AIDS community have differing opinions about alternative therapies, patients and physicians can have difficulty deciding exactly how they ought to deal with alternative treatments. On one hand, many doctors feel that all alternative treatments are either examples of the placebo effect or that they are fraudulent. On the other hand, there are patients who feel that no alternative treatments are beyond reason, and that most of these therapies are better than traditional medical therapies. Because of such widely differing opinions, the therapeutic alliance between physician and patient can be greatly strained. This strain has its roots in the power struggle between physicians and patients.

Many different models have been developed for health care delivery to HIV-infected persons. Nevertheless, limited experience has been accumulated in the development of evaluation activities of the system of care. We report on a quality assessment program implemented at our institution that is coordinating the delivery of care to the whole community of HIV-infected persons living in the Bergamo province. A total of 1599 patients, mostly IV drug users, were included in the study, during 1985-1992. Following three years of testing, we selected a number of demographic and clinical parameters in order to define the studied population and to allow computing of two outcome criteria and three process criteria: evaluating survival, incidence of major opportunistic infections (OIs), length of the follow-up preceding the diagnosis of AIDS, timeliness of initiation of antiretroviral therapy, and rate of hospitalization.
