Prevalence and Correlates of Complementary and Alternative Medicine (Cam) Use Among Hypertensive Patients in Tamale, Ghana 加纳Tamale地区高血压患者补充和替代医学(Cam)使用的患病率及其相关因素

When it comes to chronic conditions like hypertension, patient health outcomes are tightly linked to adherence to orthodox care. And Complementary Alternative Medicine (CAM) use may harm patient healthcare outcomes concerning poor antihypertensive medications adherence. The purpose of the study was to assess the use of CAM among hypertensive patients of TTH for management of hypertension. An analytic cross-sectional design was adopted for this study, using the method of survey for quantitative data. Data analysis was done using SPSS version 20. The bivariate analysis was done using Chi-square to determine factors associated with CAM use. And predictors of CAM use were identified using the multiple logistics regression model. A survey was done for 131 participants, the average age of the hypertensive study participants was 56.9 ±15.5 years, and more than half (58.9%) of the respondents were females. The prevalence of CAM use was 24.6%. Factors with significant association with CAM use among the study participants were; Respondents' level of education, age, duration of hypertension illness, and comorbidity condition predicted CAM use. In conclusion, CAM use was high and, this may lead to anti-hypertensive medication non-compliance.

Baseline Assessments of Tb Detection Centres in Selected Public and Private Laboratories in India and Need for Introducing Quality Management Systems 印度选定的公立和私立实验室结核病检测中心的基线评估和引入质量管理体系的必要性

Quality is sine qua non of healthcare services. Laboratory driven Quality Management Systems (QMS) are key to provide quality diagnosis. Missed diagnosis due to laboratory lacunae stymies Tuberculosis (TB) control measures, amplifies morbidity, mortality and, disease transmission. Hence, a pilot was conceptualized with Central TB Division, National TB Elimination Program (NTEP), Ministry of Health and Family Welfare, Government of India and Indian Council of Medical Research (ICMR) to perform baseline assessments for QMS at four laboratories from public and private centers at Delhi and Odisha. Data was collected using a standardized checklist. Scores were compiled for compliance to quality standards for attributes assessed across seven thematic domains. The assessment helped identify current status in terms of performance, processes followed and, areas requiring improvement. TB laboratories were found lacking in human resources and, backup staff that disrupted service delivery. Other gaps included quality aspects in documents, records, process controls in smear microscopy, optimum resource utilization and laboratory safety. Adage for missed TB diagnosis included laboratory errors, failure to document, collect and, test repeat samples for Nucleic Acid Amplification Tests (NAAT), lack of patient centric laboratory signages, equipment handling and, maintenance. Baseline assessments of quality at TB laboratories mainly focused attention on areas of greatest need to ameliorate the health system challenges and, develop roadmaps to accelerate improvement. By adapting to local context, QMS can be enacted through a shared vision in quality care to positively impact TB case finding and, optimize utilization of NAAT for diagnosis thereby setting stage for nationwide adoption and, scaleup.

Long-Term Analysis of Efficacy and Toxicidad of Zoledroinic Acid in Patients with Multiple Myeloma 唑来膦酸治疗多发性骨髓瘤的长期疗效及毒性分析

Myeloma bone disease MBD) is the most common and dangerous effects in patients with multiple myeloma. Bisphosphonates (BP), are the treatment of choice. But efficacy and adverse events has not been evaluated at longer follow-up. Thus, we review our experience in a tertiary cancer center, whose received zoledronic acid (ZO): in a large number of cases (1041) and longer follow-up: 4.2 to 21.6 years (median 13.8).Skeletal event (SE) were statistically better in patients who received ZA : 52 events :8.96%,compared to 207 (42.6) p < 0.001 in patients whose did not received ZA, independently if the patients received autologous stem cell transplant or not. But progression free survival and overall survival did not show any benefit. Adverse events were minimal, and jaw necrosis were not observed. Thus, we considered that ZA, remain to be the best treatment of MBD, but we confirm that ZA did not show at longer follow-up any antitumor effect.

Electroacupuncture in The Treatment of Trigeminal Neuralgia Pain 电针治疗三叉神经痛的疗效观察

Objective: This case report aimed to examine the effectiveness of electroacupuncture (AE) in treating trigeminal neuralgia pain and improving overall health in a 43-year-old woman. Methods: The study protocol involved administering electroacupuncture to the patient twice a week for a period of four consecutive weeks. During each treatment session, needles of size 0.30x60mm were inserted into specific acupoints on the patient's face, including ST8 (Touwei), GB14 (Yangbai), SJ23 (Sizhukong), EX-HN5 (Taiyang), GB3 (Shangguan), SI18 (Quanliao), ST7 (Xiaguan), and ST6 (Jiache), using the acupoint-to-acupoint penetrative needling technique. Results: The results of this case report indicate that following eight treatments with electroacupuncture, the patient's pain was completely resolved. Additionally, the patient reported improvements in overall health, including increased energy levels, positive mood, deeper sleep, and a return to normal appetite. Furthermore, the function of the trigeminal nerve, which was previously impaired, was restored. Conclusions: This case report suggests that electroacupuncture (AE) may be an effective treatment for relieving the clinical symptoms of trigeminal neuralgia and improving trigeminal nerve function. The results of this study support the high clinical application value of AE in the treatment of this condition.

The widely spread pandemic of COVID-19 throughout the world is due to the Corona virus known as Sars-CoV-2 which was first detected in the Wuhan district of China at the end of 2019. With the increase of pandemic in 2020 all countries of the world are facing serious threats to public health and economic crisis. So far seven types of coronavirus have been detected such as SARS-CoV, MERS- CoV, HKU1, NL 63, OC 43, 229 E and recent one SARS-CoV-2. Of these coronaviruses, SARS-COV, MERS-CoV and SARS-CoV-2 cause severe diseases to humans and other viruses show mild symptoms.

Intracerebral hemorrhage is a common emergency in neurology. After the onset, primary and secondary injuries are formed, resulting in severe neurological deficits and brain damage. In response to this injury, the brain activates self-repair mechanisms such as microglia polarization and autophagy activity, aiming to maintain the homeostasis of the central nervous system by inhibiting injury and promoting repair. However, this endogenous repair has certain limitations. Therefore, exogenous intervention to promote the body to inhibit damage and promote repair is particularly important. In recent years, the treatment of cerebral hemorrhage with traditional Chinese medicine has received extensive attention due to its multi-target, multi-path, and multi-link action characteristics. To explore the dual mechanism of TCM intervention in promoting the body to inhibit damage and promote repair after cerebral hemorrhage will help to further clarify and improve the clinical efficacy of TCM in the treatment of cerebral hemorrhage.

After the onset, primary and secondary injuries are formed, resulting in severe neurological deficits and brain damage. In response to this injury, the brain activates self-repair mechanisms such as microglia polarization and autophagy activity, aiming to maintain the homeostasis of the central nervous system by inhibiting injury and promoting repair. However, this endogenous repair has certain limitations. Therefore, exogenous intervention to promote the body to inhibit damage and promote repair is particularly important. In recent years, the treatment of cerebral hemorrhage with traditional Chinese medicine has received extensive attention due to its multi-target, multi-path, and multi-link action characteristics. Oxidative stress in hemodialysis patients infected with HIV 感染HIV的血液透析患者的氧化应激

Background: HIV-infected population presents the impaired renal function as a risk factor for death, and comorbid conditions are related in some traits to oxidative stress (OS). Objetive: To analyze the influence of OS in Human Immunodeficiency Virus Infection (HIV) and chronic kidney disease (CKD) by comparing the redox status between HIV patients with and without CKD and HIV hemodialysis patients. Methods: A comparative longitudinal study of the hemodialysis process was developed. The study included 96 individuals divided into four groups namely, supposedly healthy volunteers, patients with HIV infection without renal illnesses, patients with HIV infection and chronic renal disease, and HIV(-) with chronic renal disease. Indexes evaluating redox, hematological, hemochemical, immunologic, and virological aspects were determined. These indexes were also assessed before and after hemodialysis. Results: Viral load, uric acid, creatinine, and urea concentration were significantly (p<0.05) lower after hemodialysis. The two HIV infected groups were significantly different (p<0.05) regarding redox indexes of damage and antioxidant status compared to the group of supposedly healthy volunteers. Significantly increased values (p<0.05) of malondialdehyde, advanced oxidation protein product (AOPP), and peroxidation potential (PP) and significantly lower values glutathione (GSH) (p<0.05) were found after hemodialysis in both groups. In the case of HIV patients, increased values of superoxide dismutase were also found. HIV infected patients under HD tretament exhibited significantly (p<0.05) higher values of AOPP and PP and lower values of GSH than HIV(-) hemodialysis patients after hemodialysis. Conclusion: Oxidative stress occurs in both HIV and CKD conditions, and it is also increased after hemodialysis intervention. Otherwise non-viral control could influence on oxidative status in HIV/CKD patients, that´s why ART affectivities should be monitoring using HIV progression markers. Redox indexes should be diagnosed in HIV hemodialysis patients for treatment and management adjustment.

More and more diseases do not find a basis at the level of causes. One of the causes is disturbances in the effectiveness of intracellular enzymes. Without their perfect function there is no life. Until now, no industrial production of these enzymes for therapy existed. This changed, because the company Citozeatec in Milano produces several enzyme preparations, which are active in the cells, mitochondria and cell nuclei. With these, success can be achieved in many chronic diseases.

The Effectiveness of Problem-Solving Therapy on Anxiety in Patients with Diabetes 问题解决疗法对糖尿病患者焦虑的治疗效果

Background and purpose: This study aimed at investigating the effectiveness of problem-solving therapy on anxiety. Diabetes is a chronic disease with an increasing prevalence. Psychiatric problems are more common in diabetic patients than in the general population, affecting the prognosis and success of treatment. Problem-solving therapy is a short-term psychological intervention that can be used alone or with other therapeutic approaches. Methods: This case study was performed on four patients with diabetes who were selected based on available sampling. The DASS-21 questionnaire, which was answered by the subjects during three stages of pre-test, post-test, and two months after training as a follow-up, was used to collect data. First, these patients responded to the questionnaire in the baseline. Then, they were treated for problem-solving therapy for six sessions of 90 minutes and then answered again. Results: The studied data through descriptive statistics and analysis of variance with repeated measures showed that the score of individuals in the post-test stage had a significant decrease on this scale. A review of the data collected in the follow-up phase shows that problem-oriented treatment somewhat reduces anxiety and the resulting changes are relatively stable.
