Black Box Warning: Cardiovascular Complications Make Motherhood Unsafe for African American Women. 黑箱警告:心血管并发症使非裔美国妇女成为不安全的母亲。

Background: In the United States, cardiovascular disease and its complications in pregnancy is the leading killer in mothers. The black maternal mortality rate is quadruple the rate among white women.

Main body: The reasons for this staggering discrepancy hinge on two central issues: First, black women are more likely to have pre-existing cardiovascular morbidity that increase the risk of maternal mortality. Second, black women are more likely to experience adverse pregnancy outcomes including small for gestational age, gestational diabetes and preeclampsia. These perinatal complications put them at risk for developing long-term cardiovascular disease.

Conclusions: This article is a call to action to recognize that adverse pregnancy outcomes represent an opportunity to intervene in order to change the long-term cardiovascular health of black women.

Biochemical variation and association of Maternal Lipid Profile with Body Mass Index during Pregnancy 妊娠期母体血脂水平与体重指数的相关性及生化变化

The alteration of lipid profiles of healthy gestating women occurs with multiple physiological changes which may have subsequent risk associated with adverse pregnancy outcomes affecting both maternal and fetal health. Therefore, the objective of this study was focused to examine serum lipid profile and its correlation with body mass between BMI with HDL-C was negatively correlated and found to be significant (r= -0.114; p<0.01). Conclusion: The BMI and serum concentrations of TC, TG, LDL-C and VLDL-C were found to be increased in pregnant women except for HDL-C with advancement of gestational trimesters. But, serum TG and VLDL-C was decreased in 1 st trimester of pregnancy as compared with non-pregnant healthy women. The relationship between BMI and lipid profile was positively correlated and highly significant for all the blood lipid profile except for HDL-C. However, there was negative correlation between BMI and HDL-C and was found to be significant. Gynecologists should recommend blood lipid profile to be integrated as routine investigation during antenatal care visits to avoid future complications in both mother and fetus.

Training of Health Promotion Agents: An Intervention in the Baoulé Community of Kongodékro (North- Central Côte d’Ivoire) 健康促进剂培训:对孔戈德克罗(科特迪瓦中北部)Baoulé社区的干预

Public health problems in Côte d’Ivoire are dominated by infectious and parasitic diseases, the most important of which are malaria, diarrheal diseases and pneumonia. Children and pregnant women are the most vulnerable and affected populations [1]. To this alarming situation is added the progression of chronic and degenerative diseases. From this complex health situation comes a life expectancy backwards. How can the vast majority of the population benefit from a viable health at a lower cost and reduce the burden of disease in Côte d’Ivoire? The relevant assumption for a sustainable health for the entire population is the establishment of a health promotion system anchored in lifestyle. It is an immersion framework in which the different social strata of the population take charge of their own health by acting on the risk factors [2]. This dynamic health framework requires coordinated action [3]. In this context, nurses, midwives and doctors are recruited from the community health training program at Alassane Ouattara University in Bouaké. They are trained in a two-year cycle with the skills of health promotion agents. It is for these auditors to strengthen their ability to support the community in order to define the priorities of health problems, solutions, action plans to change behavior. To be convinced, a simulation aimed at putting auditors in a field intervention situation took place in the village of Kongodékro. In reality, it is about implementing the development and implementation of a community health project. This intervention is understood as an emulation based on the values, knowledge and know-how of stakeholders beneficial to the health of populations. This dynamic training process is based on a particular approach.

A Case of Recurrent Parasitic Fibroids Presenting as an Umbilical Mass 一例复发性寄生性纤维瘤表现为脐肿块

Uterine fibroids are a common condition affecting women worldwide. Treatment modalities are varied, with laparoscopic myomectomy first performed in 1979 [1]. Since the first successful laparoscopic myomectomy performed, it has now become an increasingly popular option due to the multiple advantages of a minimally invasive approach. A rare complication of parasitic fibroids as a result of laparoscopic morcellation during laparoscopic myomectomies was first described in 1997 [2], and multiple case reports have been published since, with the incidence estimated to be between 0.12 and 0.95% [3]. We describe the first case in our centre, of a patient who suffered recurrent parasitic fibroids after a single laparoscopic myomectomy was performed with morcellation and was required to undergo repeated excisions.

It is well-known, that the PAP smear in its various modifications is currently the gold standard of cervical cancer prevention and treatment worldwide [1-3]. It is less well-known, however, that the original paper by Traut and Papanicolaou was entitled, “Cancer of the uterus: The vaginal smear in its diagnosis” [4]. The pioneering research was first presented at the section on obstetrics and gynecology at the seventy-second Annual Session of the California Medical Association (Los Angeles, May 2-3, 1943). Rate and Risk Profile of Deep Venous Thrombosis in Pregnancy and Postpartum Period Among Sudanese Women 苏丹妇女妊娠和产后深静脉血栓形成的发生率和风险概况

Background: Deep venous thrombosis (DVT) during pregnancy is associated with high mortality, morbidity, and costs. DVT can also result in long-term complications that include post thrombotic syndrome (PTS) adding to its morbidity. Objective: To determine rate, timing and risk profile of deep venous thrombosis during pregnancy and puerperium among Sudanese women. Methodology : It was prospective case control and hospital-based study carried out at Shendi Teaching and Elmec-Nimir University Hospitals -Sudan from March 2017 to March 2018. Seventy-eight pregnant women or in puerperium with Doppler confirmed deep venous thrombosis were enrolled in the study, representing the main study group, while another 156 pregnant women without DVT were selected as the control group. Results: The current study showed the frequency of DVT was (0.622%), 78 out of 12,542 deliveries during the whole study period, with (0.176%) and (0.446%) occurring antenatal and postnatal respectively. The rate was 622 DVT per 100,000 births. This study revealed that women who are primigravida, had positive family history of VTE, had past history of DVT, had Anti phospholipids, anemia and delivered by C/S showed statistically significant association with DVT. Conclusions: The prevalence of DVT in our study was 622 per 100 000 births per year in pregnant and postpartum women. There is an urgent need for prophylaxis measures against DVT for pregnant women who found at higher risk for DVT.

DVT can also result in long-term complications that include post thrombotic syndrome (PTS) adding to its morbidity. Objective: To determine rate, timing and risk profile of deep venous thrombosis during pregnancy and puerperium among Sudanese women. Methodology : It was prospective case control and hospital-based study carried out at Shendi Teaching and Elmec-Nimir University Hospitals -Sudan from March 2017 to March 2018. Seventy-eight pregnant women or in puerperium with Doppler confirmed deep venous thrombosis were enrolled in the study, representing the main study group, while another 156 pregnant women without DVT were selected as the control group. Results: The current study showed the frequency of DVT was (0.622%), 78 out of 12,542 deliveries during the whole study period, with (0.176%) and (0.446%) occurring antenatal and postnatal respectively. The rate was 622 DVT per 100,000 births. This study revealed that women who are primigravida, had positive family history of VTE, had past history of DVT, had Anti phospholipids, anemia and delivered by C/S showed statistically significant association with DVT. Conclusions: The prevalence of DVT in our study was 622 per 100 000 births per year in pregnant and postpartum women. Haemoglobin D Iran With Beta Thalassemia in A Primigravida With Anaemia 原发孕妇贫血伴β地中海贫血的伊朗血红蛋白D

Anaemia is quite common in female population of developing countries including Pakistan. Beside haemoglobinopathies are often seen in India, Pakistan and Iran due to traditional practices of consanguineous marriages. Haemoglobin D-Punjab is one of the most common subvariants (55%) of Haemoglobin D, which can be inherited as a homozygous or a heterozygous trait with other haemoglobinopathies. Although, Haemoglobin D-Punjab is commonly seen but another variant of HB D like Hb D Iran with heterozygous trait of β thalassemia is rarely reported. We present a rare case of co-inheritance of Haemoglobin D- Iran and β thalassemia in a young primigravida of Kashmiri origin in her twenties. She was six months pregnant and was diagnosed to have anaemia. She had history of diarrhea. On examination, she was pale looking and mildly icteric. Systemic examination was insignificant except for mild splenomegaly. The fundal height was normal for age to date. On investigation, she had mild anaemia with microcytic hypochromic blood picture. Hemoglobin electrophoresis showed a band of Hb A2+Hb F+S/D. Molecular studies of combined Haemoglobin D –Iran and beta thalassemia. She was counseled about his disease and advised follow-up up at 6 months after delivery after delivery.
