Breaking Barriers to Diabetes Management in Rural Communities: Student Nurses Make a Difference Using Point-of-Care Testing 打破农村社区糖尿病管理的障碍:学生护士使用护理点测试发挥作用

Just as in many parts of the world, rural communities in Cameroon are experiencing a disturbing increase in the prevalence of diabetes. Inadequate or lack of trained personnel, lack of equipment and knowledge deficit about diabetes are among the many factors hindering implementation of control measures. To address this problem, a 14 hour curriculum on the use of point-of-care testing and diabetes management was designed and taught to 14 enrolled nursing students. During their subsequent 12 weeks placement in two rural communities, the students with the involvement of the health center nurses screened for and managed diabetics and high risk cases. A total of 334 clients were screened; overall diabetes prevalence was 4.89%; 11.31% were at high risk; and 35.78% were at risk. The progress reports of the diabetic and high risk clients who were followed-up revealed significant improvements in their health. The health centre nurses gained knowledge and skills in the course of working with the students while the students among many other benefits, improved their leadership skills and accountability awareness. Challenges such as lack of cooperation from lab technicians were overcome.

Using Remote Activity Monitoring and Guideline System for Home Care Clients to Support Geriatric Nursing Care in the Community 利用远程活动监测和居家护理客户指南系统支持社区老年护理

There is increasing concern to support older adults who want to live independently in their own homes. The purpose of this study is to investigate the integration of two non-intrusive approaches to monitoring home care clients activity level, along with access to best practice guidelines for clinicians at the point of care. A prototype Remote Activity Monitoring and Guidelines System has been developed that uses a GPS-equipped BlackBerry to monitor a person’s mobility outside the home. The System includes a pressure-sensitive mat that is placed under a regular bed mattress and can monitor sleep disturbances, and how long it takes to enter and exit the bed. This research study provided valuable information about the feasibility and effectiveness of integrating bio-mobility information obtained through remote monitoring with decision support technologies to support safer client care for seniors.

Analysis of the Impact of Rural Households Membership in the Micro Health Insurance on the Utilization of Health Services in Tanzania 坦桑尼亚农村家庭加入微健康保险对卫生服务利用的影响分析

This paper analyses the utilization of health services among the members of micro health insurance schemes in rural areas in Tanzania. Most literature on health insurance in rural areas in developing countries, including Tanzania, had analyzed socio-economic determinants of the households’ demand for health insurance plans. Little attention has been given to the outcomes of purchasing the health insurance plans as the determinants of membership in the schemes. This is the research knowledge gap that this study seeks to fill. In order to examine the effect of households’ membership in the micro health insurance schemes, on the utilization of health services, the matching estimator method has been adopted. In addition, the data from Tanzania Demographic and Health Survey of 2011 were used to analyse the outcome of households’ membership in the health insurance schemes on the utilization of health services. The findings revealed that the membership in the micro health insurance plans increased the utilization of the health services among the poor households, but not for the non-poor. Moreover, the insurance schemes did not provide protection against catastrophic health spending among the rural households in the episodes of illness. The reasons are that the households had to incur additional expenditures when visiting the health facilities in episodes of illness and that the plans provide limited range of benefits to households. Thus, it is recommended that poor households should be encouraged to enrol in the micro health insurance schemes. Moreover, the health facilities should provide adequate supplies so as to minimize out of pocket spending for households in the episode of illness.

Operating Manifold Services in Hospitals: A costly affair? 在医院操作多种服务:昂贵的事情?

Manifold services form an important cost centre in hospitals. Most public sector hospitals do not have standard cost accounting system and therefore budget allocation is often difficult for support services such as the manifold. The study was conducted in a 1045 bedded tertiary care hospital in New Delhi, India with the objective of identifying the cost incurred in delivery of manifold services. This traditional costing exercise was conducted from 01 July to 30 Sept 2010 and included classification of costs, identification of the cost centres in the hospital, cost allocation and apportioning. Direct and indirect costs as well as capital and operational costs were calculated. The process of manifold services from delivery to the operational aspects was studied. Tender documents and supply orders were perused to arrive at expenditure on manifold gas pipeline system, equipment & accessories. Total annual cost of manifold services at the hospital was calculated to be $ 333,676 and per day cost was calculated to be $ 941 .Annual cost per manifold terminal unit was $ 207.3. The daily cost of manifold system was calculated to be $ 0.56. The annual and daily cost per bed was deduced to be $ 320 and $ 0.87 respectively. While manifold is a vital support service, the fact remains that it is cost intensive and underutilized and often poorly planned in terms of cost effectiveness. The focus on technology in manifold services needs to be sustained.

Transition in Stroke Patients: Conceptual Analysis 脑卒中患者的转变:概念分析

Stroke patients experienced the event of non-disable person to disable one. After having a stroke, they got many changes. Physical, social, emotional and spiritual adjustment needs to be addressed for the stroke patients. They underwent transition during recovery and rehabilitation process. This conceptual analysis is important to reveal the basic element of the transition concept in stroke patients make definitions and terminology associated with it. There were several themes that associated with the concept of transition in stroke patients, such as changes in the temporal order of life (daily routines and activities), change in sense of self and the experience of time, change in connectedness, and change in community integration. This concept analysis will help nurses or health provider during taking care of stroke patient to make the best service for them. The model presented here may useful for nurses and other health professional to develop intervention programs to promote quality of life of stroke patients.

An Innovative Integration of Genetics and Genomics Content into a Baccalaureate Nursing Curriculum: The Web-Based Mini-Course in Genetics and Genomics 将遗传学和基因组学内容创新地整合到护理学士学位课程中:基于网络的遗传学和基因组学迷你课程

The purpose of this project was to develop a self contained web-based mini-course which meets the identified genetics and genomics competencies for undergraduate nursing education so that it can be utilized by a baccalaureate nursing program. The Plan-Do-Study-Act model for quality improvement was used to guide this project that included assessing knowledge in genetics and genomics of 2 cohorts of baccalaureate nursing students. Clear outcome measures were established and both groups took a 16 question quiz based on established genetic competencies for baccalaureate nursing education. Additional outcomes were measured regarding a family pedigree construction and a genetics resource/referral activity. The mean genetics quiz score for the pre-intervention student cohort was 49.3% versus the mean genetics quiz score of 88.7% for the student cohort who completed the web-based mini-course in genetics and genomics. Nursing scholars and educators have clearly documented the need to integrate genetics and genomics into the baccalaureate nursing curriculum, but limited documentation exists on how to do so. This quality improvement project offers an innovative and effective method in which to accomplish this challenging task and suggests additional application to master’s and doctoral level nursing educational programs as well.

Between 2000-2010, the Asian population grew the most in America, increasing by 43%. It is important for health care workers to be able to provide culturally sensitive care to individuals of different ethnic backgrounds, such as Asians, who may be either Hindu or Buddhist. Many times, these individuals are more comfortable utilizing Complementary and Alternative Medicine (CAM) for treatment before exploring Western treatment. CAM will be described in detail within the body of this paper. The differences in not only health care systems between Indonesia, Nepal, and the United States, but also CAM usage among the three countries will be discussed. Advantages and disadvantages in relation to CAM usage will be addressed as well as implications for nurses who are looking to widen their scope of practice.

Transgressive first clinical experiences (August 2014) 越轨首次临床经历(2014年8月)

This Study seeks to comprehend learning experiences of nursing students during their first clinical in-service placement. This Paper is part of a longitudinal development project interviewing the Student Nurse after each one of the five clinical in-service placements and then one year after graduation as a Nurse. The Study has a qualitative methodology, inspired by Michael Eraut’s thoughts on learning in the workplace. When the workplace perspective is applied, learning seems to be concentrated on actual situations which the Learner is in, in contrast to employing constructed concepts. The nursing students’ learning seems to be oriented towards socialization in the clinic as a workplace. This means that the nursing students seek to deal with overwhelming experiences concerning the naked bodies of patients and death, useful application of theoretical knowledge, the path from novice to advanced beginner, and adjusting to the workplace community. The conclusion is that the learning of nursing students during their first clinical in-service placement appears informal and not founded on evident best practice.

Retaining consumer participation in the Fremantle Primary Prevention Study: a general practice team based cardiovascular risk reduction study 保持消费者参与弗里曼特尔初级预防研究:一项基于全科医生团队的心血管风险降低研究

Recruitment and retention of participants to lifestyle modification studies can be challenging within a primary care setting. The management of factors such as travel, busy lifestyles and lack of regular follow up of participants can result in considerable attrition rates over the lifecycle of the project. The aim of the study was to understand from a research participant perspective the perceived barriers and facilitators in retaining community participants in primary health care research. This is a qualitative study with data collected from four focus groups facilitated at the end of each of three community forums focusing on sharing the findings of the Fremantle Primary Prevention Study. Overall twelve focus groups were facilitated with participants that had usual care or who were in the intervention arm of the Fremantle Primary Prevention study. Group size ranged from 6 to 10 participants with the interviewer following a semi structured focus group schedule. Thematic analysis extracted common themes from each focus group around contributing factors to participation. The key message was that relationships built over a longer time period with a GP, practice nurses and other practice staff was an important factor in retention of participants. In addition, ease of travel, convenience and regular check ups with the practice were all significant enticement to remain in a study over the course of the research. Being part of their regular GP practice’s involvement in a study was clearly worthwhile as an incentive for participants and offers hope for practice-based research networks undertaking other community-based studies in the future.
