Unilateral Transudative Pleural Effusion due to rare Cause – A Case Report 罕见病因所致单侧胸腔积液1例

Transudative pleural effusions are characterized by low protein and lactate dehydrogenase according to Light’s criteria. Common causes are congestive cardiac failure, nephrotic syndrome, liver cirrhosis, and protein-losing enteropathy. Constrictive pericarditis is a rare cause of transudative pleural effusion. It can cause bilateral or unilateral recurrent effusions and chylothorax rarely. Common causes of constrictive pericarditis include infections such as viruses and tuberculosis, radiation, and cardiac surgery. We present a treated case of pulmonary tuberculosis presenting with massive, left-sided, and transudative pleural effusion due to constrictive pericarditis diagnosed by computed tomography scan and 2D echo with classical findings. Incidental findings were internal jugular vein thrombus and right lower lobar pulmonary embolus. This case highlights the atypical presentation of constrictive pericarditis as unilateral pleural effusion. A proper history with a high index of suspicion is essential in the workup of constrictive pericarditis which can be cured by pericardiectomy.

COVID-19 Pregnancies with Heart Disease: Challenges of Delivery COVID-19心脏病孕妇:分娩的挑战

This case series of four cases of pregnancy with rheumatic heart disease with COVD-19 disease reports on the management of delivery and complications of heart disease with COVID-19, high-lighting the presentation, severity, delivery concerns, and clinical management with the maternal and fetal outcomes. Of the four full-term deliveries, one delivered normally, one instrumental delivery and two by cesarean section. All four required oxygen support post-delivery, 2/4 (50%) were transferred for intensive care unit (ICU) care, 1/4 (25%) required mechanical ventilation, 1/4 (25%) had postpartum hemorrhage, 1/4 (25%) had COVID related sepsis and received convalescent plasma therapy, and 2/4 (50%) received antiviral remdesivir. The most severe disease (COVID sepsis and ICU stay) was seen in patient of heart disease with COVID with preclampsia. All neonates tested SARS-CoV-2 negative, with one early neonatal death. All four mothers were discharged in stable condition of COVID and heart status. COVID-19 in cardiac disease pregnancies has increased rates of complications, oxygen, and ICU requirements than other pregnancies with COVID, requiring multidisciplinary team for intensive monitoring of intrapartum and postpartum period.

High-Sensitivity C-Reactive Protein-to-Albumin Ratio in Predicting the Major Adverse Cardiovascular Event in Acute Coronary Syndrome at Presentation 高敏c反应蛋白与白蛋白比值在急性冠状动脉综合征发病时预测主要不良心血管事件中的作用

Our study aimed to determine the association between high sensitivity-c reactive protein to albumin ratio (CAR) and in-hospital major adverse cardiovascular event (MACE) in patients with acute coronary syndrome and also to assess whether hs-CAR is a better marker than hsCRP or albumin alone in predicting MACE.We enrolled 110 cases who were hospitalized and major adverse cardiovascular event was defined as cardiogenic shock, acute heart failure, reinfarction and death. Blood sample for Serum albumin and hs-crp was taken at the time when patients were admitted.The incidence of MACE was more in patients with high CAR (≥1.8 group) as compared to those with low CAR (<1.8 group).The CAR is an independent predictor of MACE in patients who present with ACS.

Health is defined as a state of complete physical, mental, and social well-being.Humans have settled in both urban and rural areas, which might have influenced people and their physiology in different ways.Cardiovascular fitness as a marker of proper functioning of the whole body is associated with many health-related outcomes, with poor fitness leading to development of cardiovascular diseases (CVD).This study aimed to perform a comparative study of the physiological parameters’ indicative of cardiovascular fitness in urban and rural adolescent girls.

Electrocardiogram as a Tool to Predict Diastolic Dysfunction in the Early Hypertensive Patients and its Comparison in Males and Females 心电图作为预测早期高血压患者舒张功能障碍的工具及其在男性和女性中的比较

Heart failure with preserved ejection fraction continues to be a major healthcare challenge. Assessment of diastolic dysfunction (DD) is an integral part of the routine cardiologic examination, however little is known about ECG changes in diastolic dysfunction. several resting ECG parameters have been investigated to predict left ventricular diastolic dysfunction (LVDD). The study aims to assess diastolic dysfunction in recently diagnosed hypertensive patients by echocardiography and to find the value of ECG-derived parameters in predicting diastolic dysfunction.One hundred and fifty hypertensive patients of less than 1-year duration were divided into two groups based on the presence or absence of diastolic dysfunction. ECG parameters such as P wave dispersion, QT and QTc interval, ventricular activation time, Tend P, Tend Q were analyzed in all patients, and correlation was done with echo parameters to assess diastolic dysfunction in both males and females.In both men and women a significant correlation was found between p wave dispersion, ventricular activation time, Corrected QT interval, TendP, and, Tend Q in ECG with diastolic dysfunction on bivariate regression analysis. Multivariate regression analysis showed that p wave dispersion had a greater correlation with diastolic dysfunction when compared with other ECG parameters (r=0.88, R2=0.78).Analysis of certain ECG parameters especially p wave dispersion is of great value in predicting diastolic dysfunction in both men and women.

Cardiac Allograft Vasculopathy Evaluation by Optical Coherence Tomography 光学相干断层成像评价同种异体心脏移植血管病变

Cardiac allograft vasculopathy (CAV) is a rapidly progressive form of atherosclerosis and it is the common cause of late allograft dysfunction and death in patients following orthotopic heart transplantation. After heart transplantation, patients lack adequate anginal mechanisms and may present with refractory heart failure and sudden cardiac death, so regular screening is required to detect CAV. CAV narrows the coronary arteries in diffuse concentric pattern, so detection by coronary angiogram is difficult. Intravascular imaging such as optical coherence tomography (OCT) and intravascular ultrasound is most sensitive diagnostic test for the detection of CAV. Once CAV is diagnosed, patients should be on statins, adequate immunosuppressive medications such as sirolimus, everolimus in focal lesions angioplasty, and stenting can be done, but in some patients with significant burden of CAV, retransplantation is the only available option. Once CAV is diagnosed, regular surveillance of heart function is mandatory. We are reporting a case of CAV evaluation by OCT.
