Purpose: To evaluate the frequency of diabetic retinopathy in Lubumbashi (Democratic Republic of Congo).
Methods: Descriptive survey including 369 diabetic patients examined in our department of Ophthalmology of Lubumbashi from May 2006 to March 2008. Rates of glycaemia and blood pressure were noted. All the patients had a complete ophthalmological examination including visual acuity measurement, anterior biomicroscopy, intraocular pressure and fundus examination after pupil dilation.
Results: At the time of ocular examination, the mean age of diabetic patients was 57 years (range 16 to 85) with a sex ratio of 1.2 males for 1 female. The frequency of type 1 diabetic patients was 28% and of type 2, 72%. High blood pressure was found in 42% of the patients. The frequency of diabetic patients (all forms included) was 13% and diabetes-induced blindness was 0.27%. In our study, the frequency of diabetic retinopathy increased with the patient's age and the duration of the diabetes. Diabetic retinopathy was more frequently found in adults. We did not find correlation between the level of glycaemia and the incidence of diabetic retinopathy.
Conclusion: From our epidemiological study, diabetic retinopathy is a concern for adult public health. It requires organized screening and early treatment to prevent blindness of those diabetic patients.

Introduction: The purpose of our study is to determine the incidence of orbital complications that occurred after insertion of orbital porous polyethylene implant in children under the age of 15 years.
Material and methods: We report a series of 21 eyes of 21 patients younger than 15 years and in which a porous polyethylene implant is used for reconstruction of the orbital cavity after enucleation between January 2003 and December 2008.All patients were operated on by the same surgeon using the same technique.
Results: These 11 boys and 10 girls, whose average age is 5.7 years. Histopathologic diagnoses after enucleation are dominated by the retinoblastoma (10 eyes) and phthisis bulbi (6 eyes). After a mean follow up of 23 months it was observed two cases of implant extrusion in children enucleated for retinoblastoma. No cases of orbital cellulitis or enucleated syndrome have been reported.
Discussion: The most common complication of porous polyethylene implants in children is exposure. Risk factors may be related to surgical technique, infection, the implant, use of wrapping material and the association with adjuvant chemotherapy. Using a porous polyethylene implant uncovered remains an appropriate technique in children under 15 years for the reconstruction of the anophthalmic cavity, provided a rigorous surgical technique.

Butterfly hair is known to cause eye injury. In Africa, incriminated butterflies are Hylesia (spp). We report a case of a sub-epithelial keratitis associated with anterior uveitis following a trauma by a butterfly that was complicated by late lens opacity due to butterfly hair. Ocular lesions caused by butterfly hair are rare, but require an urgent management to prevent late and severe complications due to intraocular migration of the hairs.

Purpose: This review article addresses the different aspects that influence the choice and sequence of surgical treatment in patients with coexisting open-angle glaucoma and cataract. The effect of phacoemulsification on the intraocular pressure (lOP) and on a preexisting bleb is discussed and phacotrabeculectomy and trabeculectomy are compared. Moreover, the most recent surgical pressure lowering techniques in combination with phacoemulsification are reviewed: iStent, Trabectome, Hydrus, Cypass and Canaloplasty.
Methods: Medline database was used to search for relevant, recent articles.
Conclusion: A sustained IOP decrease of 1.5 mmHg can be expected after a phacoemulsification in patients with open-angle glaucoma. The higher the preoperative pressure, the greater the IOP lowering will be. A phacoemulsification on a trabeculectomized eye will often lead to reduced bleb function and an IOP rise of on average 2 mmHg after 12 months. Compared to a trabeculectomy, phaco-trabeculectomy will have a less IOP lowering effect and a higher complication rate. iStent and Trabectome combined with phacoemulsification can decrease the IOP with 3 to 5mmHg, with a low complication rate. The combination of Cypass and Hydrus with phaco-surgery may have a more significant IOP lowering effect but long term results are not yet published. Combining Canaloplasty with phacoemulsification is a more challenging surgery but if a tension suture can be placed, an IOP decrease around 10 mmHg might be expected.

Purpose: To study the clinical and the optical coherence tomography (OCT) features of crystalline_macular dystrophy in a 14-year-old man with Sjögren-Larsson syndrome (SLS).
Case report: A 14-year-old man was hospitalized because of a severe hyperkeratosis, epilepsy, spastic paraplegia and mental retardation. The diagnosis of SLS was considered. A bilateral crystalline maculopathy was detected by fundus ophthalmoscopy. OCT showed bilateral focal perifoveal hyperreflectivities and microcystoid spaces.
Discussion-conclusion: OCT features of the crystalline maculopathy could point at SLS in undiagnosed patients. It is a low cost and non invasive procedure that could be of value in elucidating the pathophysiological mechanisms of SLS.

Purpose: To evaluate the clinical characteristics and therapeutic challenges of retinal detachment in highly myopic eyes.
Patients and methods: We retrospectively reviewed the medical records of 83 eyes in 79 patients with high myopia (> 6.00 diopters or axial length > or = 26.00 mm) who underwent surgery for retinal detachment between 2001 and 2008. The surgical approaches depended on the type and location of the retinal break, the degree of myopia, and the grade of PVR.
Results: The mean age of patients (48 men and 31 women) was 53.9 years. Refractive error ranged from - 10 D to - 25 D (mean was - 14.0 D). The mean follow-up was 19.4 months. Peripheral or equatorial retinal tears were present in 52 cases (62.6%), a macular hole in 14 cases (16.8%), a giant retinal tear in 6 cases (7.2%), and posterior paravascular retinal tears in 11 cases (13.2%). Single-surgery anatomic success was achieved in 65 cases (78.3%), with 17 cases after scleral buckle surgery and 46 cases after pars plana vitrectomy. Final anatomic success was achieved in 76 cases (91.5%). Per and postoperative hemorrhagic complications occurred in 16 cases (19.2%).
Conclusion: Retinal detachment is a serious complication of high myopia. It often occurs in young patients. Treatment is difficult due to anatomical and clinical conditions.

Only five cases associated with XP have been published since 1960. We report a sixth case occurring in a girl aged 14. Surgical treatment consisted of an orbital exenteration extended to the eyelids and the evolution is marked by a good healing of the orbital cavity without local recurrence after four months.

Purpose: To report a case of anterior segment ischemia (ASI) with rubeosis iridis after circular buckling surgery in a highly-myopic patient which was successfully treated with a second intravitreal bevacizumab injection.
Methods: Case report and review of the literature.
Discussion: ASI is a rare but potentially serious complication of posterior segment surgery. Finally it leads to neovascular glaucoma as a result of rubeosis iridis. An encircling band can compromise anterior segment circulation in different ways: by manipulation or disinsertion of the recti muscles, by occlusion of the vortex veins through compression or by changes in the blood supply of iris and ciliary body. This patient developed rubeosis iridis secondary to ASI. There was a remarkable regression of rubeosis iridis one month after a second intravitreal bevacizumab injection. Other case reports of bevacizumab use in neovascular glaucoma have shown clinical improvements of these patients, with intraocular pressure control and reduction of the neovascularization process.

This review will highlight a new technology and recent insights into measuring retinal oxygen saturation in several ophthalmic diseases. A growing body of evidence suggests that disturbances in retinal blood flow and oxygenation are related to several retinopathies and glaucoma, which can severely impair vision. The retinal oximeter may allow researchers and physicians to gain deeper insights into retinal physiology and clarify the impact of ischemia on retinal health and function. There are two commercially available systems to measure retinal oxygen saturation: the Oxymap retinal oximeter (Reykjavik, Iceland) and the Imedos Systems UG (Jena, Germany). In this review we will focus on the results obtained with Oxymap. Direct and non-invasive measurement of retinal oxygen saturation have potentially useful diagnostic and therapeutic indications in various eye diseases such as diabetic retinopathy, age-related macular degeneration, central retinal vein and artery occlusion, anterior ischemic optic neuropathy and retinopathy of prematurity. Despite several limitations, oxygen saturation assessment in the retinal vessels is a significant advancement in the understanding of ocular diseases. Nevertheless, further studies are needed to validate the use of oximetry in retinal vasopathies and glaucoma.
