Polishing Resident Advocacy Curriculums in Primary Care 完善初级保健住院医师宣传课程

In 2021, our residency program applied and was awarded the advocacy training grant award through the American Academy of Pediatrics’ Community Practice Training Initiative (AAP-CPTI). Our training program lacked a formal advocacy curriculum. With this grant, we addressed this deficiency and created a unique and original advocacy curriculum to enhance resident education in our training program on how to be advocates for their patients. The goal of the advocacy curriculum was to teach advocacy skills in the areas of media platform utilization, communicating with legislative representatives, and reviewing community resources available relevant to our patient population. To assess the efficacy of the curriculum, surveys were administered to the residents to assess how they prioritized advocacy in their career and if the workshops met the goal of knowledge enhancement. If this goal was met, then the curriculum structure could be implemented for future academic years.

Feasibility and Utility of Point-Of-Care Ultrasound (POCUS) in Primary Care Practice. EKOAP Pilot Study 即时超声(POCUS)在初级保健实践中的可行性和实用性。EKOAP试点研究

Objective: To evaluate the feasibility and utility of point-of-care ultrasound (POCUS) in primary care (PC) practice, considering use, findings and diagnostic coincidence. Methodology: Pilot study of a cross-sectional multicentre nature, registering ultrasounds (July 21-July 22) of six family doctors with experience in POCUS. Variables: age/sex, characteristics of ultrasounds (type, duration, location), diagnostic-therapeutic referrals/interventions, findings and diagnostic coincidence (five months of monitoring). Analysis: Statistical single-variate-inference, bivariate-association of variables, logistical regression (dependent variable: ultrasound alterations); 0.05 significance. Data exported from Excel to SPSS-25 for their analysis. Results: A total of 547 ultrasounds. Mean age 54.3±18.8 years, 55% women. Different duration according to modality: abdominal/ pelvic/urological (37.1% of the total) 13.9±5.2 minutes, osteoarticular (25.8%) 11.8±3.6, soft tissue (20.7%) 8.7±3.5, p<0.001; others: pulmonary, vascular, thyroids/neck, testicle/gynaecological. Scheduled 73.0% vs. 27.0% immediate (mainly pulmonary, 61.9%) and vascular, p<0.001. With ultrasound alterations 65.1% (CI95% 61.9-68.9%) more frequent in osteoarticular 27.3%, soft tissue 21.2%, and urological 14.9%; associate with: modality (soft tissue OR with respect to abdominal/pelvic/urological of 19.9, CI95% 9.2-43.0; musculoskeletal OR 3.6, CI95% 2.2-6.1), p<0.001; being male (OR 1.8, CI95% 1.2-2.8, p=0.008); and age ≥80 years (OR 8.8, CI95% 3.2-24.1, p<0.001). With no ultrasound diagnostic coincidence 5.7% (CI95% 3.8-7.7%), associated with requesting further tests/referrals; two cases of carcinoma referred despite absence of ultrasound findings. Drainage/infiltration in 15 cases. Conclusions: The POCUS is shown to be feasible/compatible with the PC practice (duration and modalities), useful (findings/diagnostic coincidence), safe (diagnostic coincidence/handling). The different care realities could limit the generalization of results, although it also reflects the versatility of the use of the POCUS; with need to explore/ broaden its potential applicability.

Appropriateness of Antimicrobial Therapy for Treatment of Acute Tonsillitis at Primary Care level in Qatar- Clinical Audit Report 卡塔尔初级保健水平治疗急性扁桃体炎抗菌治疗的适宜性-临床审计报告

Background: Acute tonsillitis is considered as a common sore throat and difficulty in swallowing which is indicated by swelling and inflammation of the tonsils and throat, including the adenoids and lingual tonsils. Treatment of tonsillitis is commonly performed at the primary care level, which accounts for approximately 1.3% of total consultations. Appropriate antibiotic prescription is frequently associated with appropriateness in the differentiation of viral and bacterial etiologies. The inappropriate use of antibiotics has resulted to antibiotic resistance. Therefore, it is necessary to find a balance between rational antibiotic use and antibiotic access, both of which complement each other. In this paper, we will discuss the effectiveness of an action plan developed to improve compliance with appropriate antibiotic use in acute tonsillitis, based on baseline audit results, as well as the impact of this action plan on clinical practice,

Validation of a Novel Point-of-Care Testing Device Designed for Assessment of NT-pro BNP 设计用于评估NT-pro-BNP的新型护理点测试装置的验证

Aim: Our main objective was to compare Point-Of-Care Technology (POCT) to central laboratory immunochemistry testing, to assess N-terminal pro B-type Natriuretic Peptide (NT-proBNP) in ambulatory patients. A second objective was to use POCT to analyze NT-proBNP in a cohort of healthy blood donors to define reference values. Methods: Blood samples were obtained from 102 outpatients and 133 blood donors, respectively. Samples analyzed using a point-of-care instrument [NT-proBNPLumiraDx (LumiraDx, Solna, Sweden)] were compared to a commercial electrochemiluminescence immunoassay method [(NT-proBNPRoche) on the Cobas Pro analyzer (Roche Diagnostics, Mannheim, Germany)]. The study was ethically approved (01–367) and complied with the Declaration of Helsinki. Values are given as Median and Interquartile Range (IQR). Results: There was a distinct correlation between the two assays for assessing the circulating levels of NT-proBNP in outpatients (R² = 0.9546). NT-proBNPLumiraDx ranged between 50–3966 ng/L [Median: 276 (IQR: 679)] whereas NT-proBNPRoche ranged between 50–3820 ng/L; (Median: 268 (IQR: 628)]. NT-proBNPLumiraDx was 3% higher than NT-proBNPRoche (p<0.05). NT-proBNPLumiraDx levels were not affected by age in our cohort of blood donors. Conclusion: In cases where short turn-around-times for assessment of NT-proBNP are desirable, the LumiraDx instrument can safely be used as an analytical option.

Prevalence of Hemoglobinopathy in Adult Population at KAMC, Riyadh, Saudi Arabia 沙特阿拉伯利雅得KAMC成人血红蛋白病患病率

Rationale and Objectives: Hemoglobinopathies are among the most commonly inherited genetic disorders in humans and are considered as a major public health problem in Saudi Arabia. The prevalence of hemoglobinopathies is a severe health threat to the society and future generations if the assessment of hemoglobinopathies in pre-marriage cohorts is not monitored. The purpose of this study is to analyze the type of hemoglobinopathies in relation to age, gender and genetic mutations and to assess the prevalence of hemoglobinopathies in pre-marriage cohort. Materials and Methods: This is a retrospective study including a sample of 7054 patients who are older than 14 years (i.e. age ≥ 14 years) and were diagnosed with hemoglobinopathies. This study includes adults of both genders (male and female) from pre-marriage screening cohorts at King Abdul-Aziz Medical City, Ministry of National Guard – health affairs, Riyadh, Saudi Arabia. Patients were analyzed over a period of 4 years i.e. 2013 o 2017. Results: The comparative analysis of patient’s clinical characteristics were recorded and among 7054 patient sample, 3822 (54.2%) were male while 3232 (45.8%) were females. The results of the proposed study showed that Saudi Arabia has a high prevalence rate of Sickle Cell Anemia (30.7%) and β-thalassemia (12.8%). The sickle cell trait was the most repeated hemoglobinopathy in this research. Conclusion: According to the results of this cohort study Β-thalassemia and SCA are the most common hemoglobinopathy disorders in Saudi Arabia with a higher prevalence. In countries with a high prevalence of hemoglobinopathies, premarital screening is helpful for identification and prevention of high-risk marriage challenges. There is need for preventive measures and future comprehensive program with reference to the management, diagnosis and prevention of β-thalassemia and sickle cell diseases among patients in Saudi Arabia.

The technology surrounding renal replacement therapy has not progressed as rapidly as in other areas of medicine, such as cardiology or surgery. The hemodialysis machine has remained largely the same for the past fifty years, as has peritoneal dialysis [1]. Although there are some visionary projects in development, such as the bioartificial implantable kidney [2] and pig kidney xenotransplantation [3], none have advanced to the same level as the technological advancements seen in cardiology [4], diabetes [5], or surgery [6] in recent decades.

Impact of Dementia on Treatment of Peripheral Arterial Disease, Hip and Vertebral Bone Fracture 痴呆对外周动脉疾病、髋部和椎体骨折治疗的影响

Objectives: We hypothesis that dementia impact treatment strategy in cases hospitalized for advanced Peripheral Arterial Disease (PAD), Hip Fracture (HF) and Vertebral Bone Fracture (VF) in Germany. Patients and Methods: Detailed lists of cases hospitalized with the principal diagnosis PAD, HF and VF in the year 2019 were provided by the Federal Statistical Office. In addition, cases were separated by the additional diagnosis dementia and surgical treatment procedures. Results: In 2019, there were 166,244 cases hospitalized for HF, 69,888 with VF and 74,910 with PAD. The rate of cases with the additional diagnosis dementia were 18.2%, 7.7% and 7.1%. In HF cases most frequent surgical treatment was hip arthroplasty in both, cases with (37%) and cases without dementia (34%). In VF cases conservative treatment was the dominant treatment strategy with 70% in those without and 81% in those with dementia. In PAD cases conservative treatment was the dominant treatment strategy with 74% in those without and 67% in those with dementia. Mayor amputation (7% vs 14%) was performed more frequently in those with dementia, but not minor amputation (19% vs 19%). Conclusion: Our analysis supports the hypothesis, that dementia impacts treatment decisions in older patients suffering from HF, VF and PAD in Germany in different ways. In cases with dementia more cases with VB were treated conservatively and in case of HF more cases surgically. In cases with advanced PAD more major amputation were performed.
