Objective: To study features of nonlinear dynamic and oscillatory processes in the skin microcirculatory flow in patients with obliterating atherosclerosis in lower extremities arteries (OALEA).Material and methods. 56 male patients with obliterating atherosclerosis in the arteries of lower extremities and 15 practically healthy individuals were taken into the study. Microcirculation in the skin on feet with the assessment of nonlinear dynamic processes and spectral wavelet analysis of blood fl ow fl uctuations was studied by laser Doppler flowmetry, device “LACK-M” (firm Lazma, Russia). Normalized amplitude parameters of blood flow fluctuations in frequency ranges of hemocirculation tonus-forming factors were analyzed: endothelial, neurogenic, myogenic. Myogenic microvascular tone and capillary blood flow indices were calculated. To study nonlinear dynamic processes, the following parameters were assessed: fractal dimension, entropy and phase portrait analysis.Results. Spastic-atonic microcirculation disorders were revealed in patients with OALEA. They were characterized with metarteriole constriction and with restriction of capillary blood flow, with an increased myogenic tone and dilated arterioles. Statistically significant shifts in parameters of nonlinear dynamics in this case- in particular, decrease in entropy (–10.6 %) and in dimensions of phase portrait (–9.3%) under energy deficit in oscillatory microcirculation processes (–20.8 %) – can be interpreted as a decrease in complexity of laser Doppler flowmetric signal and better ordering in regulatory mechanisms of the peripheral blood flow.Conclusion. In patients with obliterating atherosclerosis in the arteries of lower extremities, the obtained results indicate significant functional microcirculation disorders of the spastic-atonic nature leading to the restriction of nutritive blood flow and accompanied by the lack of energy in oscillatory processes. These results have also shown the decrease in the system chaos severity which indicates limitation of connections of microcirculation control factors, simplification of the mechanisms of functioning of the microvascular bed and, as a consequence, decrease in the compensatory adaptive potential.

Objective. To study the effectiveness of endovascular laser coagulation in patients with recurrent varicose veins in their lower extremities.Material and methods. Outcomes after the treatment of 54 patients with varicose vein relapses in the lower extremities were analyzed. Under the tumescent anesthesia, endovascular laser coagulation was made: in 5 patients – stump of the great saphenous vein; in 22 patients – recanalized, residual and preserved subcutaneous venous trunks; in 27 patients – insufficient communicating veins of the thigh and lower leg.Results and discussion. In average, surgery lasted for 31 ± 0.6 minutes; intensity of the postoperative pain syndrome was 3.4 ± 0.4 points. Early postoperative complications were registered in 1 (1.8 %) patient, side effects in the form of hyperpigmentation and paraesthesia – in 4 (7.4 %). In 12 months, all operated patients demonstrated an improvement in all parameters of their quality of life by 29.2– 40 %; cosmetic outcome after surgery was 7.8 points out of a ten-point scale. The long-term observation revealed the relapse in varicose veins only in 1 (1.8 %) patient.Conclusion. The justified use of endovasal laser coagulation in patients with relapses of varicose veins in the lower extremities allows to avoid manipulations in the area of scar-altered tissues, to reduce tissue damage and duration of surgery, to improve patients’ quality of life, to avoid additional incisions as well as to improve cosmetic outcomes after surgery.

Purpose. To assess microcirculation and mechanisms of its regulation in the liver after its extended resection.Materials and methods. 10 Wistar rats of both sexes weighing 180–270 g which had median laparotomy under general anesthesia were taken in the trial. Microcirculation in the liver of rats was assessed with LAKK-02 Laser-Doppler flowmeter (Russia). A light guide for determining microcirculation was placed on the visceral surface of the right liver lobe; parameters of Laser-Doppler flowmetry (LDF-gram) were registered for 7 min. Then, ligatures were sequentially applied to the left lateral and median lobes of the liver followed by their cutting off; then, about 70 % of the organ were removed. After removal of both liver lobes, LDF-gram was recorded again for 7 minutes with a sensor placed on the visceral surface of the right liver lobe. Basic microcirculation parameters were registered before and after extended liver resection.Results. After the extended liver resection, average arithmetic parameters of microcirculation in the liver parenchyma decreased by 11.71 % (p < 0.05), which indicates the decrease in the microvasculature perfusion. At the same time, indicators of standard deviation of the microcirculation index and the coefficient of variation did not change statistically significantly. The median maximum amplitude of neurogenic oscillations after the resection increased by 2.3 points (p < 0.05). The amplitude of myogenic oscillations also increased after resection (p < 0.05); however, the degree of increase was less than that of the neurogenic rhythms. It was found that in 70 % of animals the maximum amplitude of endothelial oscillations increased in the liver microcirculation after the extended resection; however, in general, differences between the average endothelial oscillations before and after the resection were not statistically significant.Conclusion. In the experimental liver resection, the most informative and reliable LDF criteria on ischemia development in the liver stump were average arithmetic microcirculation indices as well as the amplitude of neurogenic and myogenic oscillations. The authors emphasize that the degree of changes in these indices may be a sign of ischemia severity in the liver stump after its resection and may have a prognostic value, although the results obtained require further research.

Purpose: to assess the effectiveness of carbon dioxide laser light in the treatment of benign skin neoplasms.Material and methods. The researchers have analyzed outcomes in 1394 patients treated for benign skin neoplasms. In the main group (n = 1184), laser photocoagulation (vaporization) and laser photohydraulic preparation were applied. Laser surgical devices “Lancet-1” and “Lancet-2” in continuous and pulsed modes, power up to 20 W, focused beam up to 1 mm were used. In the control group (n = 210), traditional techniques (excision and electrocoagulation) were applied.Results. High effi ciency of CO2 laser light for the removal of benign skin neoplasms has been confi rmed. Such a technique has a minimal rate of complications and relapses and satisfactory aesthetic outcomes, compared to the conventional treatment.Conclusion. Carbon dioxide laser light is highly effective for treating benign skin neoplasms.

The researchers applied light-oxygen therapy (LOT) in a patient with plaque psoriasis. Diode laser “Super Seb” with wavelength 1265 nm (manufactured by LLC “New Surgical Technologies”, Moscow) was used as a source of laser light: radiation power – 1.2 Wt, power density – 1.06 W/cm2, exposure dose – 191 J/cm2. Sessions were performed 2 times a week with an interval of 3–4 days. After the treatment (3 sessions) a complete clinical remission was achieved.

Purpose. To study outcomes after the minimally invasive treatment of combined hemorrhoids.Material and methods. Two hundred and three patients with combined hemorrhoids were included in the study. Patients of the main group (n = 101) had laser coagulation of internal hemorrhoids; patients of the control group (n = 102) – ligation of internal hemorrhoids. Then, excision of external hemorrhoids was made in both groups.Results. The intensity of postoperative pain syndrome decreased more rapidly in patients from the main group comparing to the controls. The number of early postoperative complications in patients of the main group was 2 times less than in the controls. Relapse of the disease one year after the treatment was seen in 5.94 % of patients of the main group and in 22.55 % of the controls.Conclusion. Laser coagulation of internal hemorrhoids followed by the excision of external ones in patients with combined hemorrhoids can improve outcomes and reduce the number of relapses.

Purpose: to study the effectiveness of surgical treatment of patients with snoring and obstructive sleep apnea syndrome using TruBlue laser light.Material and methods. Forty patients suffering of snoring and obstructive sleep apnea syndrome were taken into the study. Outcomes of their treatment are described in the present article. All patients had surgical interventions in the volume of uvulopalatoplasty performed with blue TruBlue laser light, wavelength 445 ± 5 nm (main group – 18 patients) and with Surgitron radio wave apparatus (control group – 22 patients). An objective assessment of respiratory disorders during sleep was done with Apnea Link Air respiratory monitoring system; the obtained fi ndings showed that most of the patients in both groups had snoring without apnea (44.4 and 45.4 %); the obstructive sleep apnea syndrome in them was of mild (38.9 and 36.4 %, correspondingly) and of moderate form (16.7 and 18.2 %, correspondingly).Results. Intraoperative blood loss was less in the main group 1 (2.3 ± 1.1 ml) than in the controls (7.1 ± 1.4 ml). In the postoperative period, patients of the main group had less points (7.1 ± 1.4 by the visual-analog scale) and less prolonged pain syndrome (up to 12 days) than in the control group (7.9 ± 1.2 points, up to 14 days). The subjective positive assessment of symptoms (decreased snoring, daytime sleepiness) was registered in both groups. Respiratory monitoring revealed no deterioration in sleep breathing parameters; decreased apnea/hypopnea index was noted in all observations; however, mild obstructive sleep apnea syndrome was detected in patients in both groups (27.8 and 22.7 %) one month after the surgical treatment.Conclusion. TruBlue laser light applied in the snoring and obstructive sleep apnea syndrome has been shown to be effective, causing less pronounced blood loss and pain syndrome, if to compare to the radio wave technique.

Purpose: to study the effectiveness of photodynamic therapy in combination with diathermoelectroconization in the treatment of cervical intraepithelial neoplasia and cervical cancer.Materials and methods. The study included 108 patients aged 33.13 ± 1.18 years. Human papillomavirus was detected in all patients. The study group consisted of 62 patients with cervical diseases. The fi rst subgroup included 28 patients with moderate to severe cervical intraepithelial neoplasia and pT1AN0M0 cervical cancer who underwent diathermoelectroconization and photodynamic therapy. The second subgroup included 34 patients with severe cervical intraepithelial neoplasia who underwent only cervical diathermoelectroconization. The control group included 46 patients with unchanged cervix. To carry out fl uorescent diagnostics and photodynamic therapy, the patients of the fi rst subgroup were intravenously injected with a solution of photosensitizer “Fotoditazin” (“Vetagrand”, Russia) at a dose of 0.8–1.2 mg/kg. Three hours later, using the AFS device (“Polironik”, Russia), fl uorescent diagnostics was performed followed by photodynamic therapy with an ALHT-ELOMED laser (“Elomed”, Russia) in continuous mode. The wavelength was 662 nm, the output power – 3 W, the radiation dose – 200–300 J/cm2 for the cervix and 100–125 J/cm2 – for the cervical canal. All patients underwent complex antiviral therapy.Results. All patients after photodynamic therapy showed complete eradication of human papillomavirus and regression of cervical intraepithelial neoplasia. Morphological studies have shown that the effectiveness of combined treatment of moderate and severe cervical intraepithelial neoplasia, reaching 100 %, was higher than with the use of diathermoelectroconization alone (95 %). There were no recurrences of cervical intraepithelial neoplasia and cervical cancer after combined treatment.Conclusion. Photodynamic therapy of cervical intraepithelial neoplasia and cervical cancer is a highly effective method.

The authors discuss perspectives for applying photodynamic therapy (PDT) in patients with vulvar intraepithelial neoplasia (VIN); they also analyze domestic and foreign literature sources. Up to now, some experience on the treatment of background, precancerous conditions and vulvar cancer with PDT has been obtained. The authors describe application of various photosensitizers (5-aminolevulinic acid, hematoporphyrin, E6 chloride and others) as well as several techniques for photosensitizer administration and various parameters of photodynamic effect. PDT is a non-invasive modern curative option. This type of therapy has proven to be effective in many directions of medicine; it demonstrates a wide range of its potentials in antitumor, antimicrobial, antimycotic, antiviral, immunostimulating and many other effects. With PDT technique, one can preserve not only the function, but also the integrity of the organ; it gives an excellent cosmetic result as well as rapid recovery after the treatment. However, despite the low traumatic complication rate and low level of adverse reactions, data on PDT effectiveness are contradictory. In literature, there is a signififi cant range of set parameters for treatment and exposure modes from 50 to 350 J/cm2. Literature data demonstrate that PDT effectiveness in VIN treatment ranges from 30 to 95 %, while in some cases it reaches 100 %. So as to reliably assert PDT effectiveness in the discussed pathology, further research work is needed. This review analyzes other curative options, their advantages and disadvantages. The authors underline that effectiveness of other curative modalities is still low with a high rate of relapses.
