This article reported the application of narrative nursing in one case of patient with corona virus disease 2019 (COVID-19) and acute stress disorder. There were three times of narrative persuasion for the patient during his hospitalization according to narrative theory model of narrative nursing with significant intervention effects. The patient could correctly face stress event, copy with negative emotions and receive psychosomatic rehabilitation. Influence of COVID-19 knowledge, attitude on protective behavior among clinical nurses 新冠肺炎知识、态度对临床护士防护行为的影响

Objective To explore the corona virus disease 2019 (COVID-19) knowledge and attitude, and its influences on protective behavior among clinical nurses so as to provide a reference for developing effective intervention strategy. Methods Cross-sectional investigation was used to hospital-wide nurses selected by convenience sampling in February 2020 through sending a link of Wenjuanxing to work WeChat group of Nursing Department of Beijing Hospital and to circle of friends. Clinical nurses met the inclusion and exclusion criteria were invited to fill in the questionnaire. The self-designed COVID-19 related knowledge, attitude and behavior questionnaire was used to the investigation. Results Among 750 clinical nurses, the self-assessment of COVID-19 related knowledge was≥7, with 70.04% for the average percentage. There were 91.6% of nurses with the confidence to fight with epidemic situation. After COVID-19 broke out, 94.1% of nurses strictly carried out the hand hygiene. Behavior changes in "wearing gloves when potentially contacting bloods, body fluids, secretions and excreta" , "wearing goggles and protective clothing when bloods, body fluids, secretions and excreta potentially splashing" accounted for 89.2%, 81.5% and 71.6% respectively. Pearson correlation analysis showed that COVID-19 related knowledge of 750 nurses had no correlations with three dimensions "worrying about infecting COVID-19" , "effectiveness of standard precaution" and "influencing factors of taking standard precaution" of attitude with no statistical differences (r=-0.053, 0.036,0.014;P>0.05).Variance analysis results showed that COVID-19 related knowledge had an influence on "views on COVID-19" , and had no influence on hand hygiene with no statistical difference (P>0.05), and had influences on "wearing gloves when potentially contacting bloods, body fluids, secretions and excreta" , "wearing goggles and protective clothing when bloods, body fluids, secretions and excreta potentially splashing" with statistical differences (P 0.05). Conclusions COVID-19 related knowledge of clinical nurses need to be improved and protective behavior should be changed further. Key words: Nurses; COVID-19; Knowledge; Attitude; Behavior

The role of grid management in community epidemic prevention and control and its implication to community health service systems 网格化管理在社区疫情防控中的作用及其对社区卫生服务体系的启示

In the battle against Coronavirus Disease 2019 (COVID-19) outbreak in China, the community has become a frontline to combat the epidemic. By leveraging the advantages of grid management, including household connection, clear responsibility, leaving no dead ends and all-round implementation, "early detection, early reporting, early quarantine, and early treatment" for the epidemic has been guaranteed. Grid management has played an important role in the battle against COVID-19, and meanwhile enabled the timely and accurate service of epidemic prevention and control in the community health service system. It indicates that the scientificity, integrity, accuracy, feasibility and timeliness of community health services may be enhanced by embedding the community health service system into the community grid. Bibliometric analysis of clinical practice ability of Chinese graduate nursing students 我国护理研究生临床实践能力的文献计量分析

Objective To explore the status quo and development trends of clinical practice ability research of Chinese nursing graduate students. Methods Articles about the clinical practice ability of nursing graduate students were searched from China National Knowledge Infrastructure (CNKI) , Chinese BioMedical Database (CBM) , Wanfang and VIP databases. Bibliometric analysis was performed over the year, journal, author, region and institution, fund, methods and contents of these articles. Results Totally 67 valid articles were included, including 46 studies. 41 journal articles were distributed in 18 journals, and Chinese Nursing Research had the largest number of papers. There were 10 core authors, accounting for a co-authorship rate of 64.18% and a cooperation degree of 2.72. The first authors came from 16 provinces, municipalities and autonomous regions, and most of the research institutions were medical schools. The fund support rate was 52.17%. The top 7 keywords were professional degree, clinical practice model, Delphi method, training model, education, evaluation indicator and analytical hierarchy process. Research topics could be divided into 5 categories. Conclusions The study of the clinical practice ability of Chinese graduate nursing students generally shows an upward trend, and the distribution of research regions and institutions is imbalanced. The core author group has not yet been formed, and the degree of cooperation research is not high. The research content is rich, but the quality and depth of research still needs to be improved. 目的探讨我国护理研究生临床实践能力研究的现状及发展趋势。方法检索中国知网(CNKI)、中国生物医学数据库(CBM)、万方数据库和维普数据库中有关护理研究生临床实践能力的文章。对这些文章的年份、期刊、作者、地区和机构、基金、方法和内容进行文献计量分析。结果共纳入有效文献67篇,其中研究46篇。在18种期刊上发表41篇期刊文章,以《中国护理研究》发表论文最多。核心作者10人,合著率为64.18%,合作度为2.72。第一作者来自16个省、市、自治区,研究机构以医学院居多。基金支持率为52.17%。排名前7位的关键词分别是专业学位、临床实践模式、德尔菲法、培养模式、教育、评价指标和层次分析法。研究课题可分为5类。结论我国护理研究生临床实践能力研究总体呈上升趋势,研究地区和机构分布不均衡。核心作者群体尚未形成,合作研究程度不高。研究内容丰富,但研究质量和深度还有待提高。关键词:护理;教育;研究生;临床实践能力;文献计量学

A systematic review of incidence and influencing factors of nurses' job burnout in emergency department in China 我国急诊科护士工作倦怠发生率及影响因素的系统评价

Objective To understand the status of nurses' job burnout in emergency department in China and to provide a scientific basis for making intervention measures by systematically review incidence and influencing factors of nurses' job burnout in emergency department in China. Methods Web of Science, PubMed, EMBASE, Cochrane, China Biology Medicine disc, WanFang Data, VIP and CNKI databases were searched to collect literatures related to nurses' job burnout in emergency department in China, and the search time was from the construction of the database to July 2019. Two researchers performed quality evaluation and data extraction on the literature to conduct a systematic review and studies that were consistent with quantitative analysis were given meta-analysis by Stata 12.0. Results A total of 23 articles met the inclusion criteria and reported the occurrence of nurses' job burnout in emergency department in China. A total of 91.3% (21/23) of studies used the Maslach Burnout Inventory. Meta-analysis based on 13 studies showed that 42% of nurses experienced high emotional exhaustion, 43% were highly depersonalized and 57% had low personal accomplishment. Factors related to job burnout might include age, gender, marital status, night shifts, lifestyle, workplace violence, empathy, social support and so on. Conclusions The incidences of high emotional exhaustion, high depersonalization and low personal accomplishment of nurses in emergency department in China are relatively higher, and managers need to implement targeted intervention strategies based on their influencing factors. 目的通过对中国急诊科护士工作倦怠的发生率及影响因素的系统回顾,了解中国急诊科护士工作倦怠的现状,为制定干预措施提供科学依据。方法检索Web of Science、PubMed、EMBASE、Cochrane、中国生物医学光盘、万方数据、VIP、CNKI等数据库,收集中国急诊科护士工作倦怠相关文献,检索时间为建库至2019年7月。2名研究人员对文献进行质量评价和数据提取,进行系统评价,对与定量分析一致的研究采用Stata 12.0进行meta分析。结果共有23篇文章符合纳入标准,报道了中国急诊科护士工作倦怠的发生情况。91.3%(21/23)的研究使用了Maslach职业倦怠量表。基于13项研究的荟萃分析显示,42%的护士情绪衰竭程度高,43%的护士高度去个性化,57%的护士个人成就感低。与工作倦怠相关的因素可能包括年龄、性别、婚姻状况、夜班、生活方式、工作场所暴力、同理心、社会支持等。结论中国急诊科护士情绪耗竭程度高、人格解体程度高、个人成就感低的发生率较高,管理者需要针对其影响因素实施有针对性的干预策略。关键词:护士;工作倦怠;急诊科;影响因素;系统评价;荟萃分析

Application of diagnosis related groups indexes in nursing human resource allocation of internal medicine system 诊断相关组指标在内科系统护理人力资源配置中的应用

Objective To explore the effects of diagnosis related groups (DRGs) related indexes in nursing human resource allocation of internal medicine system so as to provide a basis for reasonably allocating nursing human resource. Methods This study retrospectively consulted the medical record information of inpatients in internal medicine system of Linyi People's Hospital from 1st January 2017 to 31st December 2017, analyzed the production capacity of internal medicine system in 2017. Based on the capacity, this study allocated 2018 nursing human resource allocation of internal medicine, and compared the production capacity scores of internal medicine capacity, the medical service efficiency, medical service safety, nursing quality, satisfaction of nurses with nursing human resource allocation and patients' satisfaction with nursing care before and after implementation. SPSS18.0 software was used in data processing and analysis. Results After allocating nursing human resource based on DRGs capacity score, the production capacity scores were higher than those before implentation; the time efficiency of medical service efficiency reduced compared with that before implementation; ward management quality, nursing safety management quality, clinical nursing service quality, nursing documentation writing quality, health education quality, critical patients management quality, disinfection isolation quality, patients' satisfaction with nursing care and satisfaction of nurses with nursing human resource allocation in 2018 internal medicine system improved compared with those before implementation with statistical differences (t=-3.636、3.367、-13.090、-14.665、-11.630、-10.005、-11.572、-13.941、-8.906; P<0.05) . Conclusions Nursing human resource allocation based on DRGs capacity score could cover the demand of clinical nursing work, improve the work efficiency and nursing quality, and increase satisfaction of patients and nurses under the condition of limited nursing human resource. 目的探讨诊断相关组(DRGs)相关指标在内科系统护理人力资源配置中的作用,为合理配置护理人力资源提供依据。方法回顾性查阅2017年1月1日至2017年12月31日临沂市人民医院内科住院病人病历资料,分析2017年内科系统生产能力。基于能力,本研究分配了2018年内科护理人力资源配置,并比较了实施前后内科能力生产能力得分、医疗服务效率、医疗服务安全、护理质量、护士对护理人力资源分配的满意度和患者对护理的满意度。数据处理和分析采用SPSS18.0软件。结果根据DRGs能力得分进行护理人力资源配置后,生产能力得分高于实施前;医疗服务效率的时间效率较实施前有所下降;病房管理质量、护理安全管理质量、临床护理服务质量、护理文书撰写质量、健康教育质量、危重病人管理质量、消毒隔离质量,2018年内科系统患者对护理的满意度和护士对护理人力资源配置的满意度与实施前相比有所提高,差异有统计学意义(t=-3.636、3.367、-13.090、-14.665、-11.630、-10.005、-11.572、-13.941、-8.906;P<0.05)在护理人力资源有限的情况下,能够满足临床护理工作的需要,提高工作效率和护理质量,提高患者和护士的满意度。关键词:护理人员;护理人力资源配置;诊断相关组;容量;内科学

Effects of nursing based on PRECEDE-PROCEED in community patients with type 2 diabetes mellitus 基于pre - proceed的社区2型糖尿病患者护理效果

Objective To implement PRECEDE-PROCEED based intervention in community patients with type 2 diabetes mellitus (T2DM) and explore its effects on patients' diabetes knowledge and self-efficacy. Methods Totally 86 T2DM patients treated in a community health service center in Hangzhou between May and October 2014 were selected using convenient sampling and divided into intervention group (n=43) and control group (n=43) . Patients in the intervention group received 6-month intervention based on PRECEDE-PROCEED, while patients in the control group received a regular community health education course once during the same period. The diabetes knowledge and self-efficacy scores were evaluated between the two groups before and 3 and 6 months after intervention. At last, a total of 40 patients in the intervention group and 42 patients in the control group completed the study. Results Repeated measures analysis of variance showed that there were time-dependent, between-group and combined effects in diabetes knowledge and self-efficacy scores in the two groups (P 0.05) , but there were statistically significant differences in diabetes knowledge and self-efficacy scores between the two groups 3 and 6 months after intervention (P<0.05) . Conclusions The PRECEDE-PROCEED based intervention can effectively improve the diabetes knowledge and self-efficacy of T2DM patients, which is worth promoting in clinical practice. Objective To explore the effects of power feedback method on knowledge, attitude, practice and quality of life in patients with permanent colostomy for rectal cancer. Methods Totally 110 patients receiving permanent colostomy for rectal cancer in the Second People's Hospital of Lianyungang from April 2018 to March 2019 were selected by convenient sampling. The 55 patients admitted between April and September 2018 were divided into control group, while the 55 patients admitted from October 2018 to March 2019 were divided into observation group. Patients in the control group learned the knowledge about colostomy care by presentation to group, while patients in the observation group received interventions based on the power feedback method. Colostomy Patient Colostomy Knowledge Attitude and Practice Scale (CPCKAPS) and City of Hope-Quality of life-Ostomy Questionnaire (COH-QOL-OQ) were used to compare the effects of intervention. Results The CPCKAPS scores in the observation group were higher than those in the control group after intervention, and the difference was statistically significant (P 0.05) . Conclusions The power feedback method can improve the level of knowledge, attitude and practice of patients with permanent colostomy for rectal cancer, and further large-scale and long-term research is needed to explore its effects on the quality of life of patients. Objective To explore the effects of Cox interaction model of client health behavior (Cox-IMCHB) -based nursing intervention on health outcomes and quality of life (QOL) among patients receiving liver transplantation. Methods From October 2014 to August 2016, this study selected 65 patients met the inclusion and exclusion criteria in the First Bethune Hospital of Jilin University as subjects. Patients were divided into control group (n=32) and observation group (n=33) according to surgery order. Control group carried out the routine nursing for patients receiving liver transplantation, while observation group implemented the comprehensive nursing based on the Cox-IMCHB theory. Treatment compliance, follow-up rate and nursing service satisfaction of patients between two groups were observed one month and six months after surgery. Results One month after surgery, there was no statistical difference in the compliance of patients between two groups (P>0.05) . Six months after surgery, the total score of treatment compliance, scores of self-monitoring compliance, living habits compliance and follow-up compliance in observation group were 42.00 (40.00, 43.00) , 2.00 (2.00, 3.00) , 17.00 (16.00, 18.00) and 7.00 (7.00, 8.00) higher than those in control group with statistical differences (P 0.05) . Six months after surgery, the patient satisfaction and rate of follow-up in observation group were higher than those in control group with statistical differences (P<0.05) . Conclusions Cox-IMCHB-based nursing intervention for patients receiving liver transplantation can improve patients' long-term treatment compliance and patient satisfaction with utilization of medical service resource as well as nursing care so as to promote favorable outcomes. Current situation and influencing factors of compassion fatigue in ICU nurses ICU护士同情疲劳的现状及影响因素

Objective To investigate current situation of compassion fatigue of ICU nurses and to analyze its influencing factors. Methods By convenient sampling, a self-designed general information questionnaire, Chinese version of the Compassion Fatigue Short Scale (C-CFSS) , Ego-Resilience Scale (ERS) and Perceived Social Support Scale (PSSS) were used to investigate 194 ICU nurses in a ClassⅢ Grade A hospital in Yantai from May 2019 to June 2019. The t-test, one-way ANOVA, Pearson correlation analysis and multiple linear regression were used for statistical analysis. Results The total C-CFSS score of ICU nurses is (58.85±36.2) . ICU nurses had a moderate compassion fatigue level. The ICU nurse's ERS score was (39.96±7.65) , the PSSS score was (61.15±14.65) . The univariate analysis showed that there were statistically significant differences in C-CFSS scores among nurses of different ages, department beds, final education, self-perceived health status, monthly income, parenting (P<0.05) ; Correlation analysis showed that ICU nurse ERS score was negatively correlated with C-CFSS score (r=-0.200, P<0.01) ; PSSS score was negatively correlated with C-CFSS score (r=-0.278, P<0.01) . The results of multivariate analysis showed that age, the number of beds in the department, final education, self-perceived health status, and understanding of social support were the main influencing factors of compassion fatigue among ICU nurses, which together explained 30.1% of the total variation of compassion fatigue (F=9.673, P<0.01) . Conclusions The overall degree of compassion fatigue of ICU nurses is moderate. As nursing managers, they should pay attention to ICU nurses with different characteristics such as age, educational background and health status, carry out lectures and training on compassion fatigue, increase the social support of ICU nurses and formulate corresponding preventive intervention measures, so as to reduce the occurrence of compassion fatigue. Ding, Hua Qu","doi":"10.3760/CMA.J.CN115682-20200106-00101","DOIUrl":"https://doi.org/10.3760/CMA.J.CN115682-20200106-00101","url":null,"abstract":"Objective \u0000To investigate current situation of compassion fatigue of ICU nurses and to analyze its influencing factors. \u0000 \u0000 \u0000Methods \u0000By convenient sampling, a self-designed general information questionnaire, Chinese version of the Compassion Fatigue Short Scale (C-CFSS) , Ego-Resilience Scale (ERS) and Perceived Social Support Scale (PSSS) were used to investigate 194 ICU nurses in a ClassⅢ Grade A hospital in Yantai from May 2019 to June 2019. The t-test, one-way ANOVA, Pearson correlation analysis and multiple linear regression were used for statistical analysis. \u0000 \u0000 \u0000Results \u0000The total C-CFSS score of ICU nurses is (58.85±36.2) . ICU nurses had a moderate compassion fatigue level. The ICU nurse's ERS score was (39.96±7.65) , the PSSS score was (61.15±14.65) . The univariate analysis showed that there were statistically significant differences in C-CFSS scores among nurses of different ages, department beds, final education, self-perceived health status, monthly income, parenting (P<0.05) ; Correlation analysis showed that ICU nurse ERS score was negatively correlated with C-CFSS score (r=-0.200, P<0.01) ; PSSS score was negatively correlated with C-CFSS score (r=-0.278, P<0.01) . The results of multivariate analysis showed that age, the number of beds in the department, final education, self-perceived health status, and understanding of social support were the main influencing factors of compassion fatigue among ICU nurses, which together explained 30.1% of the total variation of compassion fatigue (F=9.673, P<0.01) . \u0000 \u0000 \u0000Conclusions \u0000The overall degree of compassion fatigue of ICU nurses is moderate.
