The promulgation of the "Internet + Nursing Service" pilot program indicates that the infiltration of the Internet into nursing work becomes imperative from possible. Therefore, the construction of specialized nursing care badly needs reform. Based on the perspective of supply-side reform, this article systematically discusses the construction of the specialized nursing ecosystem from the four levels of "point-line-surface-body", explains how hospitals at all levels can find a correct position in this ecosystem, focuses on the construction of nursing disciplines that suits itself, and pays attention to the new risks brought by "Internet +", in order to make specialized nursing care adapted to the development of the new era. Objective To explore the continuous nursing needs at discharge among geriatric hip fracture patients with hip arthroplasty. Methods From January to March 2019, this study selected 15 geriatric hip fracture patients with hip arthroplasty by purposive sampling. Semi-structured interview was carried out for those patients at discharge. Colaizzi 7-step analysis was used to data analysis to understand the needs. Results By analyzing, marshaling and extracting, there were three themes concerned by postoperative patients. Those themes included the needs for health education, improving unhealthy psychology and health services. Conclusions Hip fracture patients with hip arthroplasty have many burdens and needs for physiology, psychology and society. Nurses should pay more attention to those patients, and provide targeted intervention as well as thorough health education for discharged patients to improve their self-care abilities. Objective To explore the effect of improved skin cleaning method combined with Incontinence-Associated Dermatitis Intervention Tool (IADIT) in the treatment of incontinence-associated dermatitis in critically ill patients. Methods By convenience sampling, we selected the patients in Peking University Third Hospital as subjects. A total of 92 critically ill patients with fecal incontinence from March 2017 to February 2018 were set as the control group while 63 critically ill patients with fecal incontinence from March 2018 to February 2019 were set as the experimental group. Patients in the control group received routine care, and those in the experimental group received IADIT care. The incidence, curative effects and healing time of incontinence-associated dermatitis were compared between the two groups. Results After the intervention, the incidence of incontinence-associated dermatitis in the experimental group was 30.16% (19/63) , which was lower than 44.57% (41/92) in the control group, and the difference between the two groups was statistically significant (P<0.05) . The effective rate of incontinence-associated dermatitis in the experimental group was 94.74% (18/19) , which was higher than 65.86% (27/41) in the control group, and the difference between the two groups was statistically significant (P<0.05) . The healing time of incontinence-associated dermatitis in the experimental group was (6.33±3.14) d, which was lower than (11.55±3.41) d in the control group, and the difference between the two groups was statistically significant (P<0.01) . Conclusions Compared with traditional empirical nursing methods, targeted interventions based on IADIT for critically ill patients with fecal incontinence can effectively reduce the incidence of incontinence-associated dermatitis, improve the cure rate of incontinence-associated dermatitis and shorten the healing time. Methods The convenience sampling method was used. From March to May 2016, the Workplace Violence Questionnaire, Job Performance Scale, and Workplace Mindfulness Scale were used to investigate workplace violence, work performance, and workplace mindfulness encountered by 776 nurses in eight provinces of China. Correlation analysis and mediation effect analysis methods are used to analyze the relationship between the three, and the mediating effects of workplace mindfulness. Results A total of 776 questionnaires were collected and 696 valid questionnaires. The Workplace Violence Questionnaire Score of 696 nurses was (1.21±0.78) , the score of Workplace Mindfulness Scale was (4.74±0.63) , and the score of Job Performance Scale was (3.77±0.96) . Pearson correlation analysis showed that nurses' work performance was positively correlated with workplace mindfulness (r=0.235, P<0.01) ; nursing workplace violence was associated with work performance (r=-0.098, P<0.05) , and workplace mindfulness (r=-0.217, P<0.01) was negatively correlated with nursing workplace violence. The total effect of workplace violence on job performance was (β=-0.130 6, P<0.01) . Workplace mindfulness played a mediating role between the two, and the mediating effect was -0.067 5. Conclusions Violence experienced by patients and their families in the workplace can undermine nurses' professional immersion, alertness, and organizational acceptance levels, thereby restricting the performance of nursing work. Increasing the training of nurses in the prevention of violence and interventions in response to violence are of great significance. Methods From August to September 2019, totally 960 undergraduate nursing students engaging in clinical nursing practice for≤3 months were investigated. The survey content included demographic data, training content of nurse-patient communication ability, and clinical communication ability of nursing students. The status quo and influencing factors of communication ability of undergraduate nursing students in the early stage of clinical practice were analyzed. Results The nurse-patient communication ability score of 960 undergraduate nursing students in the early stage of clinical practice was (2.18±0.21) , with the highest score in the dimension of acute listening (2.42±0.51) and the lowest score in the dimension of verification feeling (1.88±0.51) . Univariate analysis showed that there were statistically significant differences in the nurse-patient communication ability in the early stage of clinical practice between undergraduate nursing students with different gender, with or without training for nurse-patient communication in the early stage of clinical practice in the hospital and with different training forms (P<0.05) . Pearson correlation analysis revealed that the nurse-patient communication ability score of undergraduate nursing students in the early stage of clinical practice was positively correlated with the number of nurse-patient communication training sessions in the hospital (r=0.456, P<0.01) . According to multiple linear regression analysis, the number of training sessions and training forms in the hospital were main influencing factors to the nurse-patient communication ability of undergraduate nursing students in the early stage of clinical practice (P<0.05) . Conclusions The level of nurse-patient communication ability in undergraduate nursing students in the early stage of clinical practice is low and needs to be further improved. Clinical nursing teaching managers should formulate effective training programs based on its influencing factors to strengthen training accordingly. Objective To explore the effects and feasibility of group reminiscence therapy on improving the negative emotions of the elderly inpension sectors. Methods From April to June 2019, using convenient sampling method, a total of 19 elderly people who volunteered and met the selection criteria at two pension sectors in Beijing were selected to participate in a four-week group reminiscence therapy. The scores of the negative emotions, pulse rate changes, behavior changes, and facial photo changes of each subject before and after the intervention were compared, and the relationship between the changes in the negative emotion score and the number of participations was analyzed to evaluate the intervention effect. Results After intervention, the elderly negative emotion scale score increased and the pulse rate increased, and the difference was statistically significant compared with that before the intervention (P 0.05) . Conclusions Group reminiscence therapy can significantly improve the negative emotions of elderly people in nursing homes, and has a positive effect on emotional mobilization, which can be used as a normalizing intervention for elderly people to intervene in mental health in nursing homes. Objective To explore the effects of multifunctional butterfly-shaped chassis ostomy bag for faeces drainage in incontinence-associated dermatitis (IAD) . Methods From January to September 2019, this study selected 84 fecal incontinence patients with IAD of Intensive Care Unit in Zhejiang Provincial People's Hospital by convenient sampling. All of patients were divided into observation group (n=42) and control group (n=42) with the method of random number table. Control group carried out the IAD skin management trilogy combined with the common ostomy bag, while observation group implemented the IAD skin management trilogy combined with the multifunctional butterfly-shaped chassis ostomy bag. We compared the overall effective rate of treatment for IAD, nursing time of nurses for IAD, average retention time, average use time and replacement frequency of ostomy bag, family members satisfaction of patients between two groups. Results The overall effective rate of treatment for IAD in observation group was 95.24% (40/42) higher than that 73.81% (31/42) in control group with a statistical difference (P<0.05) . The nursing time and retention time of observation group was (1.89±0.32) h/d and (5.2±0.4) days per person respectively shorter than those [ (5.73±1.64) h/d and (9.4±1.2) days per person] of control group with statistical differences (P<0.05) . The use time of observation group was (98.27±12.18) hours per bag longer than that (72.18±4.63) hours per bag of control group with a statistical difference (P<0.05) . The replacement frequency of observation group was (1.20±0.10) times per person also statistically less than that [ (3.10±0.70) times per person] of control group (P<0.05) . Family members satisfaction of observation group was significantly higher than that of control group (P<0.05) . Conclusions Compared with traditional ostomy bag, the multifunctional butterfly-shaped chassis ostomy bag has effective treatments for IAD can shorten the nursing time for IAD, lengthen the use time of ostomy bag, reduce the average retention time and replacement frequency and improve family members satisfaction which can be widely used in clinical application. All of patients were divided into observation group (n=42) and control group (n=42) with the method of random number table. Control group carried out the IAD skin management trilogy combined with the common ostomy bag, while observation group implemented the IAD skin management trilogy combined with the multifunctional butterfly-shaped chassis ostomy bag. We compared the overall effective rate of treatment for IAD, nursing time of nurses for IAD, average retention time, average use time and replacement frequency of ostomy bag, family members satisfaction of patients between two groups. \u0000 \u0000 \u0000Results \u0000The overall effective rate of treatment for IAD in observation group was 95.24% (40/42) higher than that 73.81% (31/42) in control group with a statistical difference (P<0.05) . The nursing time and retention time of observation group was (1.89±0.32) h/d and (5.2±0.4) days per person respectively shorter than those [ (5.73±1.64) h/d and (9.4±1.2) days per person] of control group with statistical differences (P<0.05) . The use time of observation group was (98.27±12.18) hours per bag longer than that (72.18±4.63) hours per bag of control group with a statistical difference (P<0.05) . The replacement frequency of observation group was (1.20±0.10) times per person also statistically less than that [ (3.10±0.70) times per person] of control group (P<0.05) . Objective To analyze the causes of malfunction of high speed turbine handpiece abbreviated as turbine handpiece and put forward preventive measure to guarantee the clinical work and increase service life so as to provide a basis for clinical cost estimation. Methods We retrospected the maintenance records of 85 turbine handpieces in the Fourth Outpatient Department of Peking University Hospital of Stomatology for 7 years, from February 2012 to January 2019 to build the malfunction classification system. The causes were divided into 6 categories based on fault source including the bearing problems (abnormal speed, abnormal voice, poor motivation and bur did not turn) , clip shaft problems (non-jammed bur, the bur could not be taken down and installed) , tightness problems (turbine handpiece leaked water) , fibre-optical problems (the light in the head of turbine handpiece did not work) , waterway problems (the head of turbine handpiece discontinuously sent out water, did not send out water, and the water could not reach the bur) and components loosing (the back cover ejected) . We counted the frequency of all categories of malfunction and analyzed the regularity and causes. Results Among 85 turbine handpieces, there were 142 malfunctions within 7 years. The most frequent problems were the bearing problems (48.59%, 69/142) followed by clip shaft problems (18.31%, 26/142) and tightness problems (16.20%, 23/142) . The bearings were changed for 9 times because of bearing problems showing that the bur did not turn, 2 times in the second, third and four year, 3 times in the fifth year; besides, the bearings could work after being maintained for 60 times. Within 7 years, the frequency of malfunction increased unsteadily and the biggest rise (50 times) was in the seven year. The bearing problems and clip shaft problems began to happen from initial application and the incidences of them were high. The waterway problems, components loosing and tightness problems happened or increased in the fourth and fifth year after application. Conclusions Normal or abnormal component abrasions of turbine handpiece are the root causes of all categories of malfunction. Malfunctions of most components occur from 3 to 5 years after application, and them are close to life time in the seventh year. If the turbine handpiece could not be used or maintained improperly, bearing and clip shaft malfunctions will occur in the early stage. The malfunction classifications of new turbine handpiece contribute to clear the fault source, follow-up maintenance and take the corresponding preventive measures. Objective To understand the family function, social support and quality of life in patients with chronic obstructive pulmonary disease (COPD) , and the relationship among them. Methods Using convenient sampling method, 225 patients with COPD admitted to a ClassⅢ hospital from June 2017 to September 2018 were selected as the study subjects. General Information Questionnaire, APGAR Family Caring Index Questionnaire, Social Support Rating Scale and WHO Quality of Life-BREF Scale were used to investigate the patients. Univariate analysis, Pearson correlation and multiple linear regression were used to analyze the relationship between patients' quality of life and social support or family function. Results The scores of family function, social support and quality of life were (7.03±2.15) , (39.43±8.46) and (51.91±11.48) respectively. The score of family function, scores for all dimensions of social support and total score of social support were positively correlated with the score of quality of life (P<0.05) . The family function, social support and disease severity were main influencing factors of COPD patients' quality of life (P<0.01) . Conclusions Good family function and social support are conducive to improving the quality of life of patients with COPD. It is suggested that nursing staff encourage family members to pay more attention to patients, improve the level of social support of patients, and improve the quality of life of patients. Objective To explore the effects of motivational interviewing strategy based on WeChat platform on blood glucose, self-management behavior and pain degree in young and middle-aged patients with type 2 diabetes mellitus (T2DM) . Methods The convenience sampling method was used to select a total of 84 young and middle-aged patients with T2DM who were admitted to Department of Endocrinology of a ClassⅢ Grade A hospital from August to October 2018 as the research objects. According to the random number table, all patients were divided into the observation group and the control group with 42 cases in each group. The observation group was given the motivational interviewing strategy based on WeChat platform for intervention. The control group received routine diabetes education. Before and 6 months after the intervention, the blood glucose, self-management behavior and pain degree of the patients in the two groups were evaluated. Results After 6 months of intervention, the patients' blood glucose, self-management behavior and pain degree were all better than those in the control group, and the differences were statistically significant (P<0.01) . Conclusions The motivational interview strategy based on WeChat platform can improve the blood glucose level and self-management behavior of young and middle-aged T2DM patients, reduce the degree of pain, and provide a reference for the development of home care and continuous care.
