The novel SARS-CoV-2 virus was thought to affect the respiratory system primarily. During the Coronavirus Disease 2019 (COVID-19) pandemic, it became apparent that it affected many organ systems, resulting in numerous extra-respiratory manifestations. Though much has been learned about the disease during the pandemic, various clinical features still have unclear pathophysiology. Here we outline the case of a 49-year-old with mildly symptomatic COVID-19 infection and bilateral acute limb ischemia (ALI).

Solitary Pulmonary Cryptococcoma without diffuse infiltration of the lung is an extremely rare manifestation of cryptococcosis, even in immunosuppressed individuals. This report presents an immunocompetent elderly man with a large solitary right lung mass, preoperatively diagnosed as inflammatory lesion with granuloma on CT guided biopsy, later proved to be Cryptococcoma on histopathological examination after Right Postero-lateral Exploratory Thoracotomy and Excision of the Mass.

Deep Vein Thrombosis (DVT) occurs when there are clots in the deep veins of the body, accounting for the majority of venous thromboembolism (VTE) cases. Half of the cases may report symptoms of pain, swelling, warmth and discoloration or redness which may be similar to cellulitis. Here we report a 47-year-old Filipino female who presented with bullae on her left leg, associated with pain, swelling and erythema which improved with anticoagulants and antibiotics.

Ischemic stroke following a snakebite is a rare complication. We report a case of a 49-years-old male who developed weakness of right upper and lower limb slurring of speech with facial asymmetry following Russell’s viper bite. Background: Meningiomas are the most common non-glial primary tumors of the central nervous system and extra-axial neoplasms. The annual incidence of meningioma is around 1,28 and 7,8 of 100.000 based on the worldwide data registries. Meningiomas vary based on the clinical findings, imaging, and histopathology from benign to malignant types. Case presentation: A 63-year-old female presented with progressive headache and visual disturbances in the last two months before admission. She also complained of a limp on the left side in the previous two weeks. General physical examination revealed bilateral visual acuity 1/300, cranial nerve deficits, motor deficits shown as left flaccid hemiparesis, and positive Babinski reflex on the left foot. CT Scan was done suggesting primary brain tumor suspect high-grade astrocytoma while the histopathological study showed atypical meningioma (WHO grade II). Discussion: The most frequent clinical findings in grade II and III meningiomas, respectively, are motor deficit, cranial nerve deficits, seizures, protrusion of the skull, intracranial hypertension, superior cortical function deficits, headache, and incidental. The imaging of meningiomas WHO grade II and WHO grade III is extra-axial mass with irregular demarcation, intratumorally necrotic areas, extensive brain edema, and changes of the bone. Conclusions: The imaging of atypical meningiomas WHO grade II and anaplastic meningiomas WHO grade III are various. Histopathology analysis is the gold standard to define it.

Celiac Disease is a malabsorptive disorder of the small intestine that often presents with diarrhea as the most common symptom. Confirmatory diagnostic testing includes endoscopy, serologic testing, and biopsy of the small intestine. These two cases of Celiac Disease are unique because two women, mid-thirties and medically obese, presented with non-classic symptoms of CD. Patient A complained of recurrent upper abdominal discomfort and pain, while Patient B complained of persistent daily symptoms of nausea. Focal intraepithelial lymphocytes were present on small bowel biopsy for both patients. Villous atrophy consistent with Marsh Type 1 and Type 3 classification was also present on small bowel biopsy, for Patient A and Patient B, respectively. Both patients tested negative for Helicobacter pylori based on biopsy not through serology, yet the pattern of inflammation on gastric biopsy showed the possibility of H. pylori gastritis for Patient A.

Introduction: Volume overload, whether overt or subclinical, is associated with increased cardiovascular mortality in chronic haemodialysis patients. The assessment of dry weight remains a challenge and relies on clinical and paraclinical methods. Pulmonary ultrasound could thus prove useful as an additional tool in improving the management of haemodialysis patients, possibly in addition to bioimpedance metry data. Method: To assess the interest and feasibility of lung ultrasound in the evaluation of dry weight overload in comparison with clinical examination in hemodialysis patients. We conducted a study in our dialysis unit at the MOHAMMED V military training hospital. The patients were all dialysed on a NIKKISSO machine. Each patient had a pulmonary ultrasound 30 minutes after the HD session, with measurement by B-line counting on 8 zones. Cardiac ultrasound and chest X-ray were performed to assess volume overload. Clinical parameters scheduled and performed UF were also recorded. Results: Between March and May 2022, 37 patients were included with a mean age of 57.4 ± 16.7 years [26; 83] and a female predominance of 54.1%. Subclinical overload was detected on lung ultrasound in 45.9% versus 18.9% on clinical examination. A significant correlation between the Kerley B line and pre-dialytic hypertension and overload was found, with a correlation coefficient of 0.038 (p < 0.05). No significant correlation was found between B-lines and clinical examination (oedema, NYHA stage, crepitus). Conclusion: Pulmonary ultrasound appears to be easy to use in routine and effective for dry weight assessment, in addition to other assessment methods.

Candida auris remain a major healthcare problem in intensive care units (ICU) as it is multidrug resistant and complicates an immunocompromised patient Our case is a 56-year-old gentleman admitted for H1N1 pneumonia and intubated, with prolonged antibiotics and imunocompromised status the patient develops Candida auris in blood and treated with anidulafungin. Conclusion the Candida auris is rare, which must be considered when treating patients with predisposing risk factors such as long hospital stays, and prolonged antibiotic therapy used prompt antifungal therapy should be considered.
