Extra-mammary breast malignancies are rarely the source of a metastatic breast mass. Uterine carcinosarcoma (UCS) is a rare disease with aggressive features and poor survival due to late presentation and advanced stage at diagnosis. Peritoneal seeding and cancer dissemination are not uncommon, but metastasis to the breast is extremely rare. Here we report a case of metastatic carcinosarcoma of uterus with unusual presentation as a breast lump without gynecological symptoms. Tissue biopsy and immunostaining confirmed the diagnosis. Few cases have been reported in the literature, mainly of serous type with the development of metastatic disease at multiple sites. To date, this is the only reported case of uterine carcinosarcoma metastasizing to the breast only, without intrabdominal dissemination, peritoneal carcinomatosis or another distant metastasis. We discuss presentation, diagnosis, methods of differentiation, and the up-to date treatment recommendations.

Polycystic kidney disease is a multisystemic familial inherited disease. A symptomatic treatment approach is the standard modality for autosomal dominant polycystic kidney disease (ADPKD) complications and life-long 50% chronic renal failure risk, as renal transplantation is the only definitive treatment. In this Covid-19 pandemic phase, all teaching and learning process is done by online learning. The COVID-19 pandemic has created the largest disruption of education systems in human history, affecting nearly 1.6 billion learners in more than 200 countries. Closures of schools, institutions and other learning spaces have impacted more than 94% of the world’s student population. There is a fear of losing 2020 academic year or even more in the coming future. The need of the hour is to innovate and implement alternative educational system and assessment strategies. The COVID-19 pandemic has provided us with an opportunity to pave the way for introducing digital learning. This article aims to provide a comprehensive report on the impact of the COVID-19 pandemic on online teaching and learning of various papers and indicate the way forward. Research methodology: This is a review paper. Results: This article present on the psychological impact of online learning on students.

Introduction: The reconstruction of hypopharynx and larynx post ablative surgery has been always a demanding challenge. Not only the restoration of the anatomy and achieving robust wound healing, but also the complexity of function in the area for the airway and upper digestive tract not to be compromised. Moreover, the age of this group of patients and perioperative radiotherapy adds to the existing obstacles in reconstruction. The evolution of microsurgery and free tissue transfer replacing the previous gold standard pedicled flaps revolutionized the head and neck reconstruction. It became a necessity to elicit how far the head and neck reconstruction practice has gone around the globe and what is considered the gold standard technique for reconstruction of each of the defects encountered at the present time. We performed a systematic review appraising the current head and neck reconstruction practice around the world aiming at concluding the gold standard methods of reconstruction of the defects in this area. Methods: Search strategy: Medline using the PubMed interface on 1st of October 2020. Results and search outcome: 236 papers were found. Of which 66 publications in the last 10 years were shortlisted reflecting the most recent practice. 20 of these were identified addressing the reconstructive approach Figure 1. Conclusions: Various reconstructive methods are reliably used in certain indications with comparable results around the globe. We present an unprecedented evidence-based universal algorithm for post ablative surgery reconstruction for the laryngeal and pharyngeal defects.

The first critically ill case with medial soft tissue tuberculosis is recorded for the first time in Cyprus. She is a young patient with a rich medical history. Tuberculosis was detected in the soft tissues of the medial degree in the Intensive Care Unit of the Nicosia General Hospital. Glomus tumors are rare benign mesenchymal tumors with various subclassifications. This is a case of a 71-yearold female with a 1.0 cm painful, firm nodule on her right heel who underwent surgical resection. The tumor demonstrated glomus cells with prominent smooth muscle differentiation that stained positively for SMA and caldesmin. Though glomangiomyomas are defined as having both a smooth muscle and vascular component, they do not account for such extensive smooth muscle differentiation. This case report proposes a new entity to better define these such tumors as a myoglomus tumor.
