Primary mucosal melanoma is an under diagnosed and rare tumor. Subglottic malignant melanoma is the least common of mucosal melanoma. This paper reports a case of a 60-year-old man who presented to the ENT clinic at King Hussein Medical Center in 2015 complaining of shortness of breath and stridor. Fibro optic endoscopic examination revealed a subglottic mass, microlaryngoscopy and biopsy with immunohistochemical staining revealed the diagnosis of malignant melanoma. The patient was treated by surgical excision followed by radiotherapy.

Introduction: Cord blood gas analysis reflects placental respiratory and metabolic function and is commonly used to assess fetal status at birth. Our aim was to identify the differences in blood gas parameters between the umbilical artery (UA) and umbilical vein (UV) in term and preterm infants. Methods: This was a prospective pilot study. Umbilical cord artery and UV samples were consecutively collected from 225 term and 57 preterm infants at the University Hospital of Padua and at the neonatal unit of Ruesch Hospital in Naples, Italy. Blood gas-analysis and glucose measurement were performed in all samples. Term infants were divided into adequate for gestational age (AGA) and small for gestational age (SGA) according to birth weight percentile. Data were analyzed using Pearson Product-Moment Correlation, independent-samples t-test, and paired-samples t-test. A p-value <0.05 was considered statistically significant. Results: The median gestational age of preterm and term newborns were 34 weeks (±2) and 39 weeks (±1) respectively. The pH, pO2, were lower and pCO2, HCO3 and base excess (BE) were higher in the UA compared with the UV, in both AGA and SGA term infants. In preterm infants, no statistically significant differences were found between the UA and UV regarding pH, while the pO2 was lower and pCO2 was higher in the UA than UV (respectively: 22.5 vs. 32.4 and 53.1 vs. 42.8; p<0.0001). In the UA, pO2, BE and potassium (K+) were lower (respectively: 22.5 vs. 25.1; -2.5 vs. -3.7; 4.8 vs. 5.8; p<0.001) while sodium (Na+) and calcium (Ca++) were higher in preterm than term newborns (respectively: 135.7 vs. 133.6; p<0.001; 1.4 vs. 1.3; p<0.001). Glycaemia was lower in the UA than in the UV in all newborns (respectively: 66 vs. 77 in AGA term newborns; 63 vs. 71 in SGA term newborns; 68 vs. 76 in preterm newborns; p<0.0001) and was related to venous glycaemia measured in the first hour of life (n= 282, r =0.60; p<0.001). Conclusions: Significant differences in blood gas parameters between the UA and UV in term and preterm newborns suggest the importance of the placental barrier and the need for accurate cord blood gas analysis interpretation at birth. UA and UV samples at birth are required to assess with higher accuracy the metabolic status of the newborn in utero. Further studies are needed to evaluate if cord glycemic values could be a noninvasive tool for prediction of glycaemia during the first hour of life.

Hematuria is one of the most important clinical manifestations, dating back in antiquity to more than a thousand years ago. In this paper, we tried to give a brief history of hematuria in Persian medicine and compare its similarities with Modern medicine. The importance of this issue is to give a better cognition of hematuria in the Persian medicine viewpoint in comparison with Modern medicine and also to understand the antiquity of hematuria, using simple and accessible therapies, provide a background for further studies in this field, and ultimately help improve Modern medicine in order to have a better treatment package, along with managing the costs.

Background and aim: Ceftriaxone is a third generation Cephalosporin and is often used to treat infections of the upper gastrointestinal tract and urinary tract infections. Due to some reported side effects of these drugs, such as urinary stone formation which may be related to hypercalciuria, this study aimed to determine the effect of ceftriaxone in the treatment of urinary tract infection with urinary stone formation. Methods: This cross-sectional study was carried out on 120 children from the age of 2 months to 14 years with febrile infections admitted to the Children's Hospital of Bandar Abbas in 2016. Patients were randomly allocated to two groups: The first group received a dose of ceftriaxone 100-50 mg/kg every 12 hours (depending on severity) and in the second group, ampicillin 50 mg/kg every 6 hours and cefotaxime 50 mg/kg every 8 hours were administered. For evaluation of hypercalciuria calcium to creatinine ratio (ca/cr) in random urine was used. Hypercalciuria was defined as Ca/Cr>0.6 in infants<1 years old and Ca/Cr>0.2 in infants>1 year old. Data was analyzed by IBM-SPSS version 21 through descriptive statistics, Chi-square, Fisher's exact test, independent samples t-test, and paired samples t-test. P-value less than 0.05 was considered significant. Results: A total of 120 children were enrolled in the study (78 in the ceftriaxone group vs. 42 in the ampicillin and ceftriaxone group). Both groups were similar in terms of age, gender, creatinine ratio and pre-treatment Ca/Cr ratio, but the difference between the groups was compared after treatment (0.339±0.204 in the ceftriaxone group and 0.236±0.159 in ampicillin and cefotaxime group) (p=0.005). The prevalence of hypercalciuria in the group receiving ceftriaxone, 15 (19.2%) and in the group receiving Ampicillin 2 (4.8%) (p<0.03). Conclusion: According to the information obtained from this study, we found out that ceftriaxone can increase the risk of hypercalciuria. Further studies with different populations could help to raise awareness of this issue.

Psychopathologic factors underlying an individual's personality structure have a major role in his or her tendency to use substances. This comorbidity can have an adverse effect on seeking and complying with treatment. Objective: This review provides the comorbidity of personality disorders and substance use disorders (SUD). Methods: In this report which was conducted in 2016, authors gathered data from different journals issued in print or electronic versions. These journals were indexed in Web of Science (Thomson Reuters), PubMed, Scopus, Google Scholar, ProQuest, Science Direct, Scientific Information Database (SID) and Magiran. A comprehensive search was done using keywords “Comorbidity”, “Personality disorders” and “Substance use disorders”. The research investigated in this review was conducted from January 1995 to 2016. Results: Comorbidity of borderline personality disorder, antisocial personality and diagnostic and therapeutic considerations are the most well-known problems in personality disorder. In dealing with these patients special attention should be paid to their safety, suicidal and homicidal thoughts Conclusion: It should be noted that an early diagnosis of comorbid personality disorder would lead to an adjusted expectancy from treatment, and adoption of long-term treatments for these patients

Background: Bearing a child is one of the most important motivations for human beings with regard to the continuation of life. People's perception of infertility is contingent upon various diverse cultural factors. Objective: This study aims to explore the perceptions of infertility in infertile women in the United Arab Emirates. Methods: This qualitative study was carried out on fourteen infertile women who were referred to medicine and health science clinical services, in Al Ain, in the United Arab Emirates. Data were collected through semistructured interviews. After the interviews were recorded, the data were analyzed using conventional content analysis method. Results: After analyzing the data, four themes including “fulfillment of dreams about being pregnant”, “marital instability”, “loss of self-esteem” and “spirituality is the way for reaching calmness” were extracted. Conclusion: The findings of this study showed that infertile women face many problems that affect their feelings and relationships in everyday life. It is thus recommended that counseling and midwifery centers be established in all infertility clinics, and experienced midwives who are familiar with reproductive health concepts guide and support infertile women as well as respond to their questions.

Introduction: Adenocarcinoma of the third part of the duodenum is a rare type of tumor that has challenges in its diagnosis and management. Segmental resection with pancreatic sparing is a safe and accepted choice for the surgical intervention of this type of cancer. Case Presentation: A 33-year-old male presented with a 3-week history of symptoms of gastric outlet obstruction with anemia and weight loss. Upon investigating the patient with computed tomography and esophagogastroduodenoscopy, he was found to have an adenocarcinoma that involved the third part of the duodenum. The patient underwent segmental resection of the third and fourth part of the duodenum with pancreatic sparing. He had a smooth postoperative course. The pathological evaluation shows a 3 cm moderately differentiated adenocarcinoma with free margins. Six out of thirteen lymph nodes were having metastasis. The patient developed a soft tissue recurrence 18 months after the operation. Take-away Lessons: Adenocarcinoma of the third part of the duodenum should be kept in mind in any patient presented with symptoms of gastric outlet obstruction. It can be managed safely with segmental resection avoiding the morbidity of resection of the pancreas.
