Calcaneal bursitis secondary to intrathecal gastrocnemius and superficial digital flexor tendon tears in two horses 两匹马腓肠肌鞘内及指屈肌腱浅表撕裂继发于跟骨滑囊炎

Two horses were referred for acute severe hindlimb lameness that persisted despite rest and nonsteroidal anti-inflammatory drugs. Both cases presented a moderate effusion of the calcaneal bursa (CB) on the medial aspect of the hock of the affected limb. The source of lameness was localised to the CB by intrathecal analgesia. Ultrasonographic examination revealed synovitis of the CB and tearing of the gastrocnemius tendon (GT) in both horses. Additionally, desmitis of the medial retinaculum and a focal area of radiolucency on the plantar border of the calcaneus were also identified in Case 2. Calcaneal bursoscopy was performed under general anaesthesia with the horse in dorsal recumbency. The surgical procedure confirmed the radiographic and ultrasonographic findings and, in addition, revealed the presence of a longitudinal tear of the superficial digital flexor tendon (SDFT) in both cases. During the calcaneal bursoscopy, surgical debridement of the intrathecally torn fibres of the GT and SDFT tendons was performed. Long-term outcome (>24 months) was excellent in both cases. Although uncommon, nonseptic calcaneal bursitis may occur secondary to intrathecal tearing of the GT and SDFT. In these cases, calcaneal bursoscopy may be necessary to obtain a definitive diagnosis and debride torn fibres contributing to the bursitis.

Synovial osteochondromatosis in the antebrachiocarpal joint of a pony 小马腕前关节滑膜骨软骨瘤病

Synovial chondromatosis is the formation of multiple benign cartilaginous nodules within a synovial lined structure. When the abnormal cartilage nodules undergo endochondral ossification to form ossified bodies, the condition is termed synovial osteochondromatosis. This case report documents an instance of synovial osteochondromatosis in the left antebrachiocarpal joint of a 12-year-old Connemara pony gelding following trauma to the area. The pony presented with a mild left forelimb lameness, positive to carpal flexion and a severe antebrachiocarpal joint effusion with a firm swelling on the dorsomedial aspect of this joint. Radiographs, ultrasonography, and magnetic resonance imaging (MRI) revealed multiple irregularly shaped but very smoothly marginated mineral opacities within the antebrachiocarpal joint. Arthroscopic removal of the nodules and subsequent histopathology confirmed the diagnosis. At 10 months, the pony was sound in all gaits and had returned to its previous level of work, with no effusion of the left antebrachiocarpal joint.

Bone oedema-like lesions at the enthesis of DIPJ collateral ligaments: MRI findings, management and outcome DIPJ副韧带末端骨水肿样病变:MRI表现、处理和结果

Bone oedema-like lesion is characterised by hyperintense signal in short tau inversion recovery (STIR) sequences on magnetic resonance imaging (MRI). Bone oedema-like lesions involving the enthesis of the collateral ligament (CL) of the distal interphalangeal joint (DIPJ) have not been investigated as a cause of lameness. This pattern was detected using low field MRI in nine forelimbs of eight horses. Lesions were found in the medial and the lateral collateral fossae of the distal phalanx in five and four feet; the lateral fossa of the middle phalanx was also abnormal in one foot. The STIR signal was graded as ‘severe’, ‘moderate’ and ‘mild’ in two, two and five feet, respectively, predominantly involved the dorsal two-thirds of the fossa. The lesion was the only pathology in six feet, while in three, the ipsilateral CL presented abnormalities. Follow-up MRI was available for two horses revealing a reduction in STIR signal. Five horses resumed a lower level of athletic activity, while three returned to the same or a higher level. Bone oedema-like lesion at the CL enthesis is a potential cause of lameness even if the CL appears normal. The prognosis is good, although further studies with a larger number of horses are required.

Assessment of serum pancreatic (DGGR-) lipase concentrations in equids with gastrointestinal disease 胃肠道疾病马科动物血清胰(DGGR-)脂肪酶浓度的评估

Background

Pancreatitis is a poorly understood condition in the horse. The DGGR-lipase assay has recently been validated for horses.

Objectives

To evaluate serum DGGR-lipase concentrations in equids presented to an equine hospital in the United Kingdom with gastrointestinal disease.

Study design

Prospective descriptive.

Methods

Blood samples were obtained by convenience sampling of horses and donkeys presented for evaluation of gastrointestinal disease.

Results

Serum pancreatic (DGGR-) lipase concentrations were measured in 205 equids with gastrointestinal disease, of which 147 survived, 47 were euthanised and 11 died. The median serum pancreatic lipase concentration in all animals was 17 U/L (IQR 14–27; range 1–3484). The lipase concentration was categorised as normal in 124 animals (60.5%) and elevated in 81 (39.5%). There was a statistically significant difference in the disease category and pancreatic lipase concentration (p < 0.001), with colic cases having higher lipase concentrations than colitis and peritonitis cases. There was strong evidence (p = 0.01) of an association between pain severity and lipase values, with higher lipase concentrations in horses with more severe pain. Of 12 horses with severely increased pancreatic lipase concentration (>200 U/L), 3/12 had spontaneous nasogastric reflux and 6/10 had distended and/or thickened small intestine on abdominal ultrasonography; 7/12 survived to hospital discharge and 5/12 died or were euthanised.

Main limitations

We were unable to confirm the presence of pancreatitis in any of the horses with elevated serum DGGR-lipase concentrations by post-mortem examination or histopathology.

Conclusions

Some equids with gastrointestinal disease have increased serum pancreatic (DGGR-) lipase concentrations, especially those with colic. This suggests that a degree of pancreatitis may be present in many colic cases, although this does not indicate causation.

Patellar fragmentation following variation of medial patellar ligament splitting: Two cases 髌骨内侧韧带断裂变异致髌骨断裂2例

Medial patellar desmotomy is one of the management strategies for upward fixation of the patella but has been associated with fragmentation of the patella. Consequently, medial patellar ligament splitting is now often considered a superior surgical technique, without reports of this complication. However, this case report documents two cases of this complication in a variation of this surgical technique.

Subchondral lucencies (SCLs) of the medial femoral condyle (MFC) are a cause of lameness in young horses, potentially affecting future athletic performance. Due to the widespread use of pre-sales screening radiographs for Thoroughbred weanlings and yearlings, these lesions are frequently identified at various stages of progression without the manifestation of clinical signs. Treatment for horses without lameness or effusion is not recommended as most do not develop clinical signs. Conversely, conservative treatments, such as reduced exercise and the use of non-steroidal anti-inflammatory medications or intra-articular therapies, typically yield suboptimal outcomes in clinically significant SCLs, and these will frequently be presented for surgical intervention. This report documents five yearling horses with MFC SCLs that had received speculative intra-articular (IA) injections of 10 mg triamcinolone acetonide (TCA) and 40 mg of gentamicin sulphate (GS) into the medial femorotibial joint (MFTJ). Treatments had been initiated by the attending veterinarians based on their own circumstantial impressions that treatment might help improve some radiographically observed MFC SCLs detected on weanling screening radiography. All cases in this series became acutely lame on the treated limbs, developing marked effusion of both the MFTJ and the femoropatellar joint (FPJ). Clinical assessment and synovial fluid analysis confirmed that all cases were non-septic. Due to the development of clinical signs, surgical management was advised. Before surgery, affected joints did not show additional radiographic changes compared to initial radiographs. While examining the MFTJ and FPJ using standard arthroscopic techniques, large areas of detached and partially detached cartilage were present on the caudal aspect of the distal patella, the lateral and medial trochlear ridges and the central articular surface of the MFC. These lesions had the appearance of acute delamination of the articular cartilage, exposing an underlying, pale, avascular subchondral bone plate below. This case series is the first to report iatrogenic articular cartilage damage in the MFTJ and FPJ of thoroughbred yearlings treated for early MFC SCLs with recommended therapeutic doses of IA TCA and GS. This case series demonstrated that IA injections of TCA and GS in juvenile horses with MFC SCLs can have negative effects on articular cartilage. We propose that IA corticosteroids should not be used in young horses with MFC SCLs on radiographs but without clinical signs and owners should be apprised of this potential complication. Further research into the potentially negative effects of IA corticosteroids on the cartilage of juvenile horses would be appropriate.

Intermittent functional pheochromocytoma in a 26-year-old pony 26岁小马间歇性功能性嗜铬细胞瘤

Pheochromocytoma is a tumour of the chromaffin cells of the adrenal medulla. These tumours can be asymptomatic or functional, due to the secretion of catecholamines. Clinical signs associated with functional pheochromocytomas are mainly tachycardia, hyperlactataemia, hyperglycaemia and haemoconcentration. In this case report, we describe a 26-year-old pony gelding admitted to the Emmeloord Equine Clinic with multiple adrenergic crises, epistaxis, oesophageal obstruction and endocrinopathic laminitis with confirmed insulin resistance. In between these admissions, the pony competed successfully at national level. The final and fatal adrenergic crisis started with a behavioural change followed by profuse sweating. The pony showed haemoconcentration, hyperlactataemia and hyperglycaemia. Treatment with balanced isotonic fluids, pergolide, insulin and acepromazine failed to stop the worsening clinical state of the pony, and a decision for euthanasia was made. On post-mortem examination, the diagnosis of pheochromocytoma was confirmed. Due to the intermittent presence of clinical signs, the diagnosis intermittent functional pheochromocytoma was made.

Background

Left dorsal displacement of the large colon (LDDLC) is a common type of colic in horses. Different risk factors for developing LDDLC have already been established. To prevent recurrence of the pathology, multiple laparoscopic nephrosplenic space closing techniques have been developed. However, the correlation between the configuration of the nephrosplenic space (NSS), which is visible through laparoscopy, and LDDLC has not been examined.

Objectives

To examine whether the NSS configuration, among other possible risk factors, is correlated with the development of LDDLC.

Study design

Retrospective case–control study.

Methods

Horses that underwent a laparoscopic procedure were divided into cases (treated with NSS closure due to putative LDDLC) and controls (without history of LDDLC and laparoscopically treated for another indication). The shape and size of the NSS were evaluated via laparoscopic images and/or videos by three evaluators who were blinded to the origin of the images. These results, together with data on other possible risk factors, were compared between the two groups by multivariable statistical analysis.

Results

Eighty-two horses were included in the study: 28 cases and 54 controls. The multivariable analysis revealed that cases had significantly more cribbing behaviour [p = 0.032; odds ratio (OR) = 0.070 (0.006–0.798)] and taller heights at the withers [p = 0.014; OR = 0.856 (0.756–0.969)] than the controls. No significant differences in terms of NSS configuration were found between the two groups.

Main limitations

Retrospective nature of the study and the diagnosis of LDDLC being putative.

Conclusions

Results suggest that cribbing behaviour and height at the withers are contributing factors to the development of LDDLC. However, NSS configuration could not be linked to LDDLC.
