Gastric accommodation: pathogenetic role and clinical significance 胃容积:致病作用和临床意义

The article presents a literature review of modern Russian and foreign publications devoted to the study of gastric accommodation. It has been shown that accommodation of the proximal stomach plays an important role in its adaptation to food intake. This process is carried out in two stages with the participation of parasympathetic and paracrine regulation; the effects of nutrients when they enter the duodenum also play an important role. Methods for assessing gastric accommodation include gastric barostat, gastric scintigraphy with food load, drinking tests, single-photon emission computed tomography, magnetic resonance imaging and ultrasound examination of the stomach, intragastric manometry. Violation of gastric accommodation leads to an excessive increase in intragastric pressure and the occurrence of symptoms most often found in functional dyspepsia, gastroesophageal reflux disease and diabetic gastropathy. Approaches to the treatment of gastric accommodation disorders are at the development stage, the possibilities of diet therapy, pharmacological and non-drug correction are being discussed.

Malnutrition, dysfunction of the gastrointestinal tract and sarcopenia - features of combined pathology 营养不良、胃肠道功能障碍和肌肉疏松症--综合病症的特征

One of the risk factors for the development of sarcopenia is malnutrition. Impairment of the energy value of the diet, the balance of proteins, fats and carbohydrates, a decrease in the protein-amino acid component of the nutritional diet - all this can lead to the development of sarcopenia. The intestinal microflora plays an important role in the process of growth and development of muscle tissue, in preventing its degradation with age. Fatty hepatosis and cirrhosis of the liver can also cause the development of sarcopenia. The problem is urgent and modern, and needs to be studied, which is what this literature review is dedicated to.

This article highlights the pharmacological activity of licorice metabolites of significant interest in the context of the treatment of cancer, cardiovascular, and respiratory diseases. The review details the diversity and efficacy of lichalocone, isoliquiritigenin, liquiritigenin, and other licorice metabolites that exhibit anti-inflammatory, antimicrobial, and antioxidant effects. These effects are based on the ability of licorice metabolites to regulate molecular cascades responsible for apoptosis, cell cycle, cell proliferation and many other processes. The article discusses in detail the prospects for the use of licorice metabolites in oncology, cardiology, their potential in the development of new drugs and the need for further research to determine their full therapeutic potential.

Dynamics of the level of hormones-regulators of gastric motility when drinking low-mineralized water 饮用低矿物质水时胃动力调节激素水平的动态变化

Purpose of the study: To assess the effect of low-mineralized sulfate-chloride-sodium mineral water (MW) on the level of motilin and cholecystokinin in the blood in functional dyspepsia and in healthy people. Materials and methods. 180 people aged 22.3±0.21 years were examined, divided into 3 groups: patients with postprandial distress syndrome (PDS) (62 people), patients with epigastric pain syndrome (EPS) (62 people), practically healthy (56 people). The severity of gastroenterological symptoms was determined using the GSRS questionnaire. In all examined patients, the concentrations of motilin and cholecystokinin in the blood were determined initially and after a single dose of 200 ml of MW. Results. The initial concentration of motilin in the blood of patients with EPS (8584,0 [7951,0; 9807,0] pg/ml) was lower than in patients with PDS (9876,0 [9340,0; 10219,0] pg/ml, p=0,000) and healthy (9779,5 [9230,0; 10027,0] pg/ml, p=0,000). The intake of MW in healthy people did not cause changes in the concentration of motilin in the blood (p=0,352), in patients with PDS it led to a decrease in its level (p=0,000), and to an increase in EPS (p=0,000). The initial level of cholecystokinin in the blood of patients with EPS (187,7 [124,0; 230,1] pg/ml) was higher (p=0,008), and in those examined with PDS (123,3 [83,5; 143,2 ] pg/ml) is lower (p=0,000) than in healthy people (145,3 [107,1; 186,2] pg/ml). MW intake led to an increase in its concentration in all three groups: healthy (p=0,014), patients with PDS (p=0,000) and EPS (p=0,000). Conclusion. Peculiarities of the dynamics of the level of motilin and cholecystokinin in the blood of patients with PDS, EPS and healthy people when taking low-mineralized sulfate-chloride-sodium MW, which must be taken into account when developing schemes for drinking balneotherapy for functional pathology of the stomach, were revealed.

The number of patients with chronic liver disease (CLD) is steadily increasing. According to the 2023 update published by the European Association for the Study of the Liver, liver diseases account for two million deaths annually and account for 4% of all deaths (1 in every 25 deaths worldwide). This review emphasizes the need for early detection and control of these diseases course, as a factor that improves the patient's prognosis. It is proposed to use a new scale of the CLivD indicator (Chronic Liver Disease indicator), based on the assessment of risk factors widely available in physician practice, to predict the risk of developing progressive liver disease in the general population.

Matrix metalloproteinases and morphological features in chronic liver diseases 基质金属蛋白酶与慢性肝病的形态特征

Aim of investigation. To study the relationship of matrix metalloproteinases (MMPs) and their tissue inhibitors (TIMPs) with a morphological features of chronic liver diseases (CLD). Materials and methods. 76 patients with CLD of viral or alcoholic etiology aged from 18 to 64 years were examined. Chronic hepatitis was diagnosed in 59 patients, liver cirrhosis (class A according Child-Pugh score) was detected in 17 cases. The control group consisted of 72 practically healthy people. The blood levels of MMP-1, MMP-9, TIMP-1 were determined by enzyme immunoassay, and the ratio of TIMP-1/MMP-1, TIMP-1/MMP-9 was calculated. Results. There was an increase in the concentration of TIMP-1 and MMP-1 in the blood, the ratio of TIMP-1/MMP-9, a decrease in the ratio of TIMP-1/MMP-1 in CLD. In moderate and severe histological activity of CLD the levels of TIMP-1 and MMP-9 were higher, and the ratio of TIMP-1/MMP-9 was lower than in patients with histological activity index values less than 9 points. In patients with liver cirrhosis (fibrosis F4), the maximum values of TIMP-1, TIMP-1/MMP-1 and TIMP-1/MMP-9 were determined, which differed from the corresponding values for fibrosis F0-1 and F2. The blood levels of MMP-9 more than 410 ng/ml predicted severe inflammation in CLD with an accuracy of 82.9 %. The blood levels of TIMP-1 above 624 ng/ml, TIMP-1/MMP-1 more than 37.1, TIMP-1/MMP-9 more than 7.33 had high accuracy (82.9, 80.3, 80.3 %, respectively) in the prediction of liver cirrhosis (fibrosis F4). Conclusion. The imbalance in the matrix metalloproteinase system is associated with the morphological features of CLD and is characterized by hyperexpression of MMP-9 in cases of severe inflammation and increased activity of TIMP-1 in severe stages of liver fibrosis.

Systematic analysis of the relationship between non-alcoholic fatty liver disease and tissue iron overload: promising areas for the use of polypeptide therapy 非酒精性脂肪肝与组织铁超载之间关系的系统分析:使用多肽疗法的前景广阔领域

Iron overload in non-alcoholic fatty liver disease (NAFLD) is a fairly common phenomenon that receives very little attention in clinical practice. However, iron overload, leading to hemosiderosis (deposition of “indigestible” nanodispersed iron oxides in various tissues) significantly aggravates NAFLD, stimulating increased chronic inflammation, insulin resistance and hemosiderosis of other organs. As a result, ferroptosis of hepatocytes occurs (apoptosis caused by iron overload and hemosiderosis), which accelerates the transformation of non-alcoholic steatosis into non-alcoholic steatohepatitis (NASH) and, subsequently, into liver cirrhosis. Iron overload is aggravated by micronutrient deficiencies and pathogenic intestinal microbiota. The paper presents the results of a systematic analysis of this issue, describes the prospects for therapy using micronutrients and human placenta hydrolysates (HPP), which contribute not only to the regeneration of liver tissue, but also to the normalization of iron homeostasis.
