CA 18y old female without family history, who presented 6 months before, a right nasal swelling with the notion of right nasal obstruction evolving towards an ulcerated tumor whose biopsy returned in favor of a nasal NK lymphoma. Physical examination showed a right nasal destructive ulcerated tumor, with an infiltrated base, with a necrotic surface with a purulent background and with clear and irregular boundaries making 6cm of the long axis (FIGURE 1). In dermoscopy appearance we had especially an aspect in gaps with separations by whitish septa (FIGURE 2).

1 ISSN 1755-5191 We describe 18F-FDG positon emission tomography-computed tomography findings of a 67 y old male patient, followed since 2014 for poorly differentiated NETs with unknown primary tumor. PET-CT showed cardiac hypermetabolism in the left ventricular as muscular metastasis in addition to other muscular metastases of the two diaphragmatic pillars and the left iliopsoas muscle (FIGURE 1). Cardiac USN and chest-CT (FIGURES 2 & 3) confirmed our diagnostic by showing a small lesion in the cardiac apex measuring 13.7 × 18 mm. Myocardial metastases are very rare with a reported incidence up to 1 to 4% [1]. 18F-FDG is a positron emission tomography tracer for poorly differentiated NETs with KI67>20% [2]. This modality of exploration is useful for the staging and the restaging patients. The maximum detection was correlated to a low somatostatin receptor (SSRT) expression unlike well-differentiated tumors that show great expression of these receptors [3]. Background: Vancomycin, a glycopeptide antibiotic, is commonly used against methicillin-resistant Gram-positive cocci. However, the nephrotoxic side-effects of Vancomycin results in dose restriction and reduces the duration of administration. Objectives: The present study was designed to evaluate the protective effects of aqueous saffron (Crocus sativus) extract on vancomycin-induced nephrotoxicity in rats. Materials and methods: In this study 32 adult male Wistar rats were randomly divided into 4 groups of 8 rats; (i) control (ii) saffron (80 mg/kg, I.P.), (iii) vancomycin (200 mg/kg/BD, I.P.) and (iv) vancomycin plus saffron. The saffron treatment began 24 hours before the administration of the vancomycin therapy and lasted 8 days, while the duration of the vancomycin therapy was 7 days. Serum creatinine and blood urea nitrogen (BUN), renal malondialdehyde (MDA) levels, superoxide dismutase (SOD) activity and histopathological changes of renal tissue were analyzed. Results: Administration of vancomycin caused a significant increase in serum creatinine, BUN and renal MDA levels, whereas, SOD activity was decreased, when compared to the control group. Aqueous saffron extract, on the other hand, decreased serum creatinine, BUN concentration and renal MDA levels but increased the level of renal SOD activity. Substantial histopathological alterations including destruction of kidney tubules, interstitial edema, epithelia vacuolization and epithelial desquamation, were observed with the vancomycin-only treatment group. However, the administration of saffron extract resulted in a significant reduction of damage to the kidneys. Conclusion: The present study demonstrated that oxidative stress plays an important role in vancomycin induced nephrotoxicity and saffron extract has a potent renoprotective effect against vancomycin induced lipid peroxidation and nephrotoxicity.

Introduction: Quantitative magnetic resonance imaging (MRI) methods to quantify muscle tissue fat content are increasingly used in the evaluation of patients with inherited neuromuscular disorders. Recently, these techniques gained importance to detect disease progression and possible treatment responses. In this study, two widely used fat quantification MRI sequences (two- and three- point Dixon) were applied to assess the reproducibility and reliability of the techniques. Methods: For this clinical trial six healthy volunteers were scanned at a 3T clinical MRI scanner a total of six times on two different days (three times each) using two different fat quantification techniques: a 2-point Dixon (2-PD) and a 3-point Dixon (3-PD) technique. Each time axial sequences of both thighs were acquired. For all repeated scans and subjects a total of 660 muscles of the thighs were segmented with regions of interest (ROI) by two different raters. An interclass correlation coefficient (ICC) was used to compare inter-rater and intra-rater agreement of the different acquisition methods, regarding other potential biases such as repetition and day of the scans. Furthermore, the general accuracy of the different methods was obtained by calculating the fat fraction. Results: For quantitative MRI accessing the fat fraction in the thigh muscles, Dixon sequences have been shown to be very stable in reproducibility with ICCs of >0.9 of both inter-rater and intra-rater agreement, as well as low variability. There was no significant difference between the two sequences regarding reproducibility, although 2-PD Dixon showed a little advantage when measuring small muscles. Conclusion: In conclusion both sequences are useful for assessing fatty muscle degeneration showing a high reproducibility and reliability, with low inter- and intra-rater differences. 2-PD showed a small advantage when measuring small muscles.

Imaging is important in distinguishing benign ossifying lipoma from more aggressive tumors such as well-differentiated liposarcomas. Early diagnosis of ossifying lipoma and understanding of its benign etiology prevent unwarranted medical intervention such as biopsies and surgical resection, as well as decrease patient anxiety.

A 63 y old male with long standing history of dyspnea, and recurrent cough and expectoration was initially thought to be suffering from recurrent lower respiratory tract infection (LRTI) and fibrosing alveolitis. The latest plain chest radiograph showed dilated trachea and bronchi in addition to bilateral diffuse interstitial lung disease. The chest CT scan confirmed the abnormally tracheobronchomegaly which was consistent with the diagnosis of the rare Mounier-Kuhn syndrome of an apparently a sporadic form. Such diagnosis should be considered in the differential diagnosis of settings with radiological findings suggestive of LRTI. The case also emphasizes the importance of the size of air passages on the plain chest radiograph.

Idiopathic basal ganglia calcification, or Fahr Syndrome, often presents in middle-aged patients presenting with parkinsonian symptoms. Unique presentation of young patient presenting with a seizure, and without the presence of parkinsonian symptoms. Imaging revealed near complete calcification of bilateral basal ganglia. Most common etiology of basal ganglia calcification is idiopathic or familial, but may also be secondary to metabolic derangements.
