Dry Suspension Formulation of Ethanol Extract from Wuluh Starfruit (Averrhoa bilimbi L.) with Arenga Gum as Suspending Agent 以Arenga Gum为悬浮剂的乌卢海杨果乙醇提取物干悬浮液配方研究

Wuluh starfruit (Averrhoa bilimbi L.) has a sour taste and high water content which makes the fruit easily damaged and has a short shelf life. It is necessary to process wuluh starfruit in order to obtain a product that has a longer shelf life and tastes better without reducing its benefits. The purpose of this study was to determine the wuluh starfruit in the form of ethanol extract can be formulated into a dry suspension dosage form with arenga gum as a suspension which at a certain concentration can be an optimal suspending agent. Based on the results showed that the ethanol extract of wuluh starfruit (Averrhoa bilimbi L.) could be formulated into a dry suspension. The preparation with 0.5% arenga gum as a suspending agent is the best preparation which has a moisture content of 0.46%, stationary angle of 26.48º, flow time of 4.77 seconds, compressibility index of 18%, particle size distribution 152.133μm, viscosity 106.00 cP, pH 4.16 and sedimentation volume 0.55. Data were analyzed using SPSS 26.0 using one-way analysis of variance (ANOVA) test and post hoc Multiple Comprasions Tukey HSD test.
乌拉星果(Averrhoa bilimbi L.)口感偏酸,水分含量高,易损坏,保质期短。为了在不降低其营养价值的前提下获得保质期更长、口感更好的产品,有必要对乌拉杨桃进行加工。本研究的目的是确定在一定浓度下,以arenga胶为悬浮剂,以乙醇提取物的形式配制成干悬浮剂的效果。实验结果表明,枸杞杨桃乙醇提取物可配制成干悬浮液。以0.5% arenga胶为悬浮剂的最佳制备工艺为:含水率0.46%,静置角26.48º,流动时间4.77 s,压缩指数18%,粒径分布152.133μm,粘度106.00 cP, pH 4.16,沉降体积0.55。数据分析采用SPSS 26.0,采用单因素方差分析(ANOVA)检验和事后多重比较Tukey HSD检验。

Patient Satisfaction on Covid-19 Handling in Primary Centers Medan 棉兰基层医疗机构新冠肺炎处理患者满意度分析

Primary health centers play important roles in preventing, detecting, and helping people to cope with the problems associated with the COVID-19 infection. Therefore, it is necessary to strengthen and optimize the role of primary health center, including pharmaceutical care. This prospective descriptive study aimed to evaluate the level of patient satisfaction on COVID-19 handling in seven primary health centers Medan, Sumatera Utara, Indonesia  period January to April 2021. A self-designed questionnaire was distributed to a total of 617 patients admitted to the 7 primary health centers to assess the characteristics of the patients and the required data about patient satisfaction about nine-item pharmaceutical services provided, supporting facilities, and infrastructures. The patient satisfaction was assessed using a 4-point Likert scale categorized into very dissatisfied, dissatisfied, satisfied and very satisfied. The obtained data were analyzed using descriptive and Chi-Square analyses. Most (67%) of the respondents were females. The present study indicated that most respondents satisfied with the services provided.

Formulation and Evaluation of Cream Turmeric Extract Preparations from Turmeric Rhizomes (Curcuma domestica Val.) 姜黄根茎乳状姜黄提取物制剂的配方及评价

The use of plants as a method of herbal treatment is a step that is currently being developed by many researchers to produce effective and minimal side effects. Turmeric is one of the most widely used cooking spices by the people of Indonesia. The main ingredients possessed by turmeric include curcumin (77%), demethoxy (17%) and bisdemethoxy (3%). One of the pharmaceutical preparations with a topical delivery system is a cream preparation which is a semi-solid dosage form, containing one or more drug ingredients dissolved or dispersed in an appropriate base material. Turmeric can be formulated in various preparations, one of which is a cream dosage form. The advantage of using cream preparations is that they are practical, easy to wash and clean. The purpose of this study was to formulate turmeric extract in a cream preparation. The results of the evaluation test showed that the formula produced a cream with a yellow-orange color, a characteristic odor of turmeric, and a preparation with a semisolid cream texture, had good homogeneity, good dispersing ability, the pH of the preparation was 5.84; 6.32; 6.24; 5,94;5,94 and the type of emulsion is O/W (Oil in Water).

Antibiotic Use in Cesarean Section and Hysterectomy Procedure: Qualitative and Quantitative Assessment 剖宫产和子宫切除术中抗生素的使用:定性和定量评估

Antibiotic resistance is global issue and the incident is increasing with the use of unwise antibiotic. This study is to describe and evaluate appropriateness of antibiotic prophylaxis selection on Cesarean Section and Hysterectomy procedure. Purpose of study is to evaluate appropiateness of antibiotic selection on cesarean section and hysterectomy. A retrospective study was performed during September 2016 - January 2017 in Private Hospital, Semarang. Data were analized quantitatively using ATC-DDD methodology. Evaluation of antibiotic prophylaxis will be assessed based on The guideline of The American College of Obstetricians and Gynecologists (ACOG) : Antibiotic Prophylaxis for Gynecologic Procedures and The American Society of Health-System Pharmacists (ASHP) : Clinical Practice Guidelines for Antimicrobial Prophylaxis in Surgery. Overall, 90 of 95 patients were given antibiotic prophylaxis. The most frequently used antibiotics were ceftriaxone (69.51 DDD/Patient-days) and ceftazidime (14.95 DDD/Patient-days). Appropriate antibiotic selection based on the guidelines for cesarean section procedure was 1.75% and 6.1% for hysterectomy procedure. The use of antibiotic prophylaxis on cesarean section and hysterectomy procedure was very high and less appropriate in the selection. Almost patient received third-generation cephalosporin instead of first-generation cephalosporin such as cefazolin. cefazolin is more effective and narrow spectrum for infection-causing bacteria of these procedures.

Antibacterial Activity of Patch Silver Nanoparticles and Chitosan with Cellulose Nanofibers Carriers against Staphylococcus aureus and Escherichia coli 纳米纤维素纤维载体贴片纳米银和壳聚糖对金黄色葡萄球菌和大肠杆菌的抑菌活性

One of the medical needs whose demand continues to increase is wound dressings. The wound cover must also be non-toxic, non-allergenic, made of widely available biomaterials, and have antibacterial properties that can prevent infection of the wound. Chitosan is known to have wound healing activity by acting as a blood-clotting agent and stimulating the formation of new tissue, and silver nanoparticles have good antibacterial activity. Silver Nanoparticles and Chitosan with Cellulose Nanofibers carriers (SNCCN) are made in the form of patches with the ratio formula between cellulose nanofibers and chitosan/silver nanoparticles is 1:9, 2:8, 3:8, 4:7, 5:5, 6:4, 7:3, 8:2, 9:1, and 10:0. Then the antibacterial activity was tested against Staphylococcus aureus and Escherichia coli to find the best formula for antibacterial activity. The analysis showed that the SNCCN patch with a ratio of 9:1 had the best antibacterial activity against Staphylococcus aureus (13.8±0.05 mm) and Escherichia coli (12.5±0.05 mm). It can be concluded that patch Silver Nanoparticles and Chitosan with Cellulose Nanofibers carriers (SNCCN) have good antibacterial activity at a concentration of 9:1 in the category of strong inhibition (10-20 mm).

Plasmodium Resistance to Artemisinin Derivates due to Kelch-3 Gene Mutation Kelch-3基因突变引起的疟原虫对青蒿素衍生物的耐药性

Artemisinin class of antimalarial drugs play an important role in controlling falciparum malaria after the emergence of resistance of Plasmodium falciparum to other antimalarial drugs such as chloroquine, sulfadoxine-pyrimethamine and mefloquine. Therefore, the presence of Plasmodium falciparum resistance to this class of drugs is threat to global efforts to eliminate this disease. Resistance of Plasmodium falciparum to artemisinin recently known to be associated with mutations in the propeller domain of the kelch-13 (K13) Plasmodium falciparum gene. The incidence of Plasmodium falciparum resistance due to mutations in the K13 gene, among others, can be found in Cambodia, Laos, Vietnam, China, Myanmar, Thailand and Africa. The presence of mutations in this gene will change the response of Plasmodium falciparum against oxidative stress induced by artemisinin by involving the proteasome-ubiquitin pathway. In addition, mutation K13 will also change the levels of PI3K and PI3P in the body of Plasmodium falciparum. PI3K and PI3P are lipids that essential for the development of Plasmodium falciparum from ring stage to schizont. Resistance to artemisinin will also provide phenotypic changes in the life cycle of Plasmodium falciparum in the form of elongation at the stage ring and transient shortening in trophozoite development. This resistance incident can be overcome, among others by prolonging the duration of treatment (from a 3-day regimen to a 4-day regimen) and combining artemisinin with proteasome inhibitors.

Resistance of Plasmodium falciparum to artemisinin recently known to be associated with mutations in the propeller domain of the kelch-13 (K13) Plasmodium falciparum gene. The incidence of Plasmodium falciparum resistance due to mutations in the K13 gene, among others, can be found in Cambodia, Laos, Vietnam, China, Myanmar, Thailand and Africa. The presence of mutations in this gene will change the response of Plasmodium falciparum against oxidative stress induced by artemisinin by involving the proteasome-ubiquitin pathway. In addition, mutation K13 will also change the levels of PI3K and PI3P in the body of Plasmodium falciparum. PI3K and PI3P are lipids that essential for the development of Plasmodium falciparum from ring stage to schizont. Resistance to artemisinin will also provide phenotypic changes in the life cycle of Plasmodium falciparum in the form of elongation at the stage ring and transient shortening in trophozoite development. Antifungal Activity of Patch Silver Nanoparticles and Chitosan with Cellulose Nanofibers Carriers against Trichophyton rubrum and Pitysporum ovale 纳米纤维素载体贴片纳米银和壳聚糖对红毛霉和卵形Pitysporum的抑菌活性研究

Abstract. Wounds that are not treated and are kept open will provide an entrance for microorganisms from outside that can cause infection. One of the medical needs whose demand continues to increase is wound dressings. Chitosan is known to have wound healing activity by stimulating the formation of new tissue, and silver nanoparticles have good antimicrobial activity. Silver nanoparticles and chitosan with cellulose nanofibers carrier are made in the form of patches with the ratio formula between cellulose nanofibers and chitosan/silver nanoparticles are 1:9, 2:8, 3:8, 4:7, 5:5, 6:4, 7:3, 8:2, 9:1, and 10:0 then tested the antifungal activity against Trichophyton rubrum and Pitysporum ovale for finding the best formula for antifungal activity. The analysis showed that the patch with a ratio of 6:4 had the best antifungal activity against Trichophyton rubrum (14.7±0.1 mm) and a 9:1 patch on the Pitysporum ovale (6.9±0.05 mm) gave a significant difference to negative control (p<0.05). It can be concluded that the Patches Silver Nanoparticle and Chitosan with Cellulose Nanofibers (SNCCN) carriers have good antifungal activity in the inhibitory category.

Good drug management including planning, procurement, storage, distribution, destruction, recording and reporting is one of the tasks of the North Sumatra Provincial Health Office and HIV Services in the Regency/City. The purpose of this study was to analyze the drug management of the Antiretroviral program drugs in 2019 and 2020 at the Pharmacy Warehouse of the Health Office of North Sumatra Province, the Pharmacy Installation of Pirngadi Hospital Medan, and Pharmacy Installation at HKBP Balige Hospital. The research was conducted in January-April 2021. This research is descriptive with quantitative data obtained retrospectively and concurrently, qualitative data obtained through observation and interviews with informants. The data obtained were analyzed using indicators and compared to standard research results. The results showed in the North Sumatra Provincial Health Office 5 indicators have not met the standards and 9 indicators have met the standards, in Pirngadi Hospital Medan 4 indicators have not met the standards and 8 indicators have met the standards, but have not done the process of planning drug needs, and in HKBP Balige Hospital there are no indicators that meet the standards and do not do the planning of drug needs. The conclusion of this study is drug planning at the North Sumatra Provincial Health Office using the consumption method of the previous period. Pirngadi Hospital Medan and HKBP Balige Hospital did not plan for drug needs. Drug distribution at the North Sumatra Provincial Health Office uses a decentralized system starting in 2020 from the previous centralized system. In Pirngadi Hospital Medan the system is decentralized, while at HKBP Balige Hospital the distribution system is centralized.

The Identification of Mercury in The Urine of Night Cream Users in Midwifery Program Abdurrab University Abdurrab大学助产项目晚霜使用者尿液中汞的鉴定

White and bright skin is everyone's dream. Many women who do facial skin care in order to obtain healthy facial skin and attractive appearance. Facial skin care that is mostly done is the use of facial whitening creams. Face whitening cream is believed to be able to remove or lighten black spots on facial skin. Whitening cream that is often used is a night cream. Some of the night creams sold have not received permission from National Agency of Drug and Food Control of Indonesia. Usually these creams contain heavy metals, such as mercury. Mercury is toxic which when exposed and absorbed by the body can cause loss of enzyme ability in the body to cancer. The purpose of this study was to determine the presence or absence of mercury in the night cream and urine of night cream users at Abdurrab University Midwifery Study Program students. The method of this research is a qualitative examination on samples of night cream and urine used KI reagent. The results showed that the night cream sampled gave a positive test result and the urine of night cream users gave a negative test result. The conclusion of this study is that the night cream sampled contained mercury and the urine of night cream users did not contain mercury.

Anti-Inflammatory Activity Test of Ethanol Extract of Ambarella Fruit Leaves (Spondias dulcis Frost) Against Male Rats Induced Carrageenan 伞叶乙醇提取物对雄性大鼠卡拉胶诱导的抗炎活性试验

In the treatment of inflammation, there are several chemical drugs, namely steroidal and non-steroidal anti-inflammatory drugs, but because they have many side effects, anti-inflammatory drugs are developed from natural ingredients, especially plants. Plants are scientifically proven to have anti-inflammatory properties, one of which is ambarella fruit leaves because it leaves contain flavonoid secondary metabolites as anti-inflammatory compounds. The objective of this study to determine the effect of giving ambarella fruit leaves on anti-inflammatory activity in male rats induced by carrageenan. The method used in this study was paw edema, namely by induction of carrageenan 0.1 ml as an irritant on the soles of the rat's feet for 360 minutes with the parameter observed was the inhibition of edema in the feet of rats using a plethysmometer. This study was an experimental study that was divided into 5 groups, namely the control group positive (diclofenac sodium 9 mg/kgBW), negative control (Na-Cmc 0.55), and three doses tested, namely 100 mg/kgBW, 300 mg/kgBW and 500 mg/kgBW. The results of this study showed a significant difference (p<0.05) where flavonoid compounds from ambarella fruit leaves affect anti-inflammatory activity. The conclusion that the three doses is tested, namely 100 mg/kgBW, 300 mg/kgBW and 500 mg/kgBW has an anti-inflammatory effect where the more effective dose was 300 mg/kg kgBW.
