Medical interventions and their poor scientific backup: A threat to Evidence-Based Medicine 医疗干预及其缺乏科学依据:对循证医学的威胁

In order to support Evidence-Based Medicine, critically evaluating the scientific basis of contemporary medical interventions is crucial. The significant scarcity of scientific backup for most medical interventions, however, poses a marked threat to Evidence-Based Medicine. This article explores the extent of this issue and its effects on healthcare. This article suggests that most contemporary medical interventions lack a scientific backup. Although there is counterevidence suggesting that most medical interventions are backed with solid evidence, the results of this counterevidence (study) can only be inferred in the settings where this study was conducted, questioning its generalisability. Furthermore, this article delves into the information-seeking behaviours of doctors. Over journal articles, which are the cornerstone of Evidence-Based Medicine, doctors favour textbooks and personal contacts. Relying on textbooks may not be the best choice as they may not necessarily be updated with the latest research, leading to outdated recommendations. Personal opinions obtained through personal contacts are also unreliable sources of information. When doctors search for health information inefficiently and choose treatments unsupported by evidence, it can have severe consequences for public health and put patients at risk. Therefore, to encourage doctors to root their interventions in the best available evidence, urgent measures are required to enhance the research paper reading literacy skills of doctors. This article suggests that encouraging research-related training and education in the medical syllabus and continuing education activities is vital. Improving doctors' skills in reading and evaluating research papers can lead to increased interest in medical literature and more reliable decision-making based on the most trustworthy evidence. Moreover, this article suggests that, even for experienced researchers, reading a research paper can be difficult due to the complex language and terminologies. Therefore, journal article authors should consider using more straightforward language, providing plain language summaries, visual aids like graphs and videos, and graphical abstracts. This way, doctors can understand research findings and easily incorporate them into practice. At last, this article provides an important note that collaboration among interdisciplinary professionals (researchers, doctors, allied health professionals, and educators) and other relevant stakeholders (policymakers, syllabus designers, and decision-makers) is crucial for promoting Evidence-Based Medicine.

Need for psychological counseling for pregnancy during COVID-19 pandemic: A mini review COVID-19大流行期间妊娠心理咨询需求综述

Pregnant women during the COVID-19 pandemic were more vulnerable which had more psychological impact on them because of reduced social interactions and daily routines. The impacts of COVID-19 on the mental health of pregnant women were remarkable due to their prone to psychological disorders during the pregnancy period. There are some recommendations to reduce the likelihood of psychological disorders during the COVID-19 pandemic including "attention to previous mental problems of pregnant women", "screening of patients in need of psychological counseling in the first trimester of pregnancy", "providing various psychological programs according to the mothers' request, including booklets, brochures, videos and online sessions", "special contact numbers for psychological consultants for psychological emergencies of mothers" and "training the husbands of pregnant women according to the needs of mothers.

Incidence and severity of vitamin d insufficiency among type diabetes patients and the impact of hyperglycemia on serum vitamin d levels: A case control study 型糖尿病患者维生素d不足的发生率和严重程度以及高血糖对血清维生素d水平的影响:一项病例对照研究

Aim: The aim of the present study was to assess the prevalence and severity of vitamin D deficiency in type 2 DM as well as to record the effect of hyperglycaemia on serum vitamin D level. Methods: The present study was conducted in the Department of General Medicine. Total 200 participants were enrolled in the study out of which 100 healthy people were enrolled as case (Group A) and 100 type 2 diabetic patients as controls (Group B). Controls include age and sex matched healthy individuals. Patients younger than 18 years, patients with chronic kidney disease, patients taking calcium supplements or vitamin D supplements within last 3 months, patients suffering from any known chronic illness were excluded from this study. Results: Frequency Distribution of Participants according to Severity of Vitamin D level noted. Prevalence of low vitamin D level in healthy population was only 20% in the study, while prevalence was 80% in Diabetic group. Among diabetic patients having abnormal Vitamin D level, majority (64%) were having insufficiency, only 20% were having overt vitamin D deficiency in Diabetic patients. In patients with controlled diabetes as per HbA1C criteria, the prevalence of sufficient, Insufficient and Deficient Vitamin D was 24%, 60% and 15% respectively, where in patients with uncontrolled diabetes it was 10%, 68% and 22% respectively. More number of diabetic patients with uncontrolled status (22%) was having overt vitamin D deficiency in comparison to controlled status (16%). There was a significant association between the maintenance of euglycemia and severity of Vitamin D level in diabetic patients, as the p value was less than 0.05.Conclusion: Vitamin D deficiency is highly prevalent in diabetic patients as compared to normal healthy population. All patients with type 2 diabetes mellitus must be screened for vitamin D levels and those found to be having insufficiency or deficiency of vitamin D should be started on vitamin D supplements.

A hospital‑based cross‑sectional study to examine the echocardiographic features of patients with type-2 diabetes 一项以医院为基础的横断面研究,以检查2型糖尿病患者的超声心动图特征

Aim: This study aimed to investigate echocardiographic features in patients with diagnosed T2DM. Material & Methods: A hospital‑based, cross‑sectional observational study included 100 patients with diagnosed type 2 diabetes, who presented to Medicine Department K S Hegde hospital for one year. This study included type 2 diabetes diagnosed as per American diabetes association 2018 criteria. Their ECG findings were noted and correlated with their blood sugar levels. Results: There were 55 males and 45 females. Out of 100 cases, 5 cases (5%) of newly diagnosed type 2 diabetes were in age group 31-40 years, 24 cases (24%) were in age group 41-50 years, 26 cases (26%) were in age group 51-60 years, 30 cases (30%) were in age group 61-70 years, 10 cases (10%) were in age group 71-80 years and 5 cases (5%) were above 81 years. Out of 100 cases of newly diagnosed type 2 diabetes, 54 cases (54%) were smokers and 46 (46%) were non-smokers, 20 (20%) consumed alcohol and 80 (80%) didn’t consume alcohol. Out of 100 cases of newly diagnosed type 2 diabetes, 50 cases (50%) had HTN, 25 (25%) had CAD and 75 (75%) did not have CAD. Out of 100 newly diagnosed type 2 diabetes, 40 cases (40%) had left ventricular diastolic dysfunction, 26 cases (26%) had left ventricular hypertrophy (LVH), 15 cases (15%) had RWMA, 10 cases (10%) had RA/RV dilated, 6 cases (6%) had dilated LV and 3 cases (3%) had dilated LA. Conclusion: Type 2 diabetics without cardiovascular symptoms must be screened for cardiovascular abnormalities so that early interventions can be done to prevent further progression to symptomatic cardiovascular abnormalities. ECG should be done to predict cardiovascular risk in type 2 diabetic patients without cardiovascular symptoms. Echocardiographic abnormalities are very common in outpatients with type 2 diabetes, but neither cardiac symptoms nor clinical characteristics are effective to identify patients with echocardiographic abnormalities.

Out of 100 newly diagnosed type 2 diabetes, 40 cases (40%) had left ventricular diastolic dysfunction, 26 cases (26%) had left ventricular hypertrophy (LVH), 15 cases (15%) had RWMA, 10 cases (10%) had RA/RV dilated, 6 cases (6%) had dilated LV and 3 cases (3%) had dilated LA. Conclusion: Type 2 diabetics without cardiovascular symptoms must be screened for cardiovascular abnormalities so that early interventions can be done to prevent further progression to symptomatic cardiovascular abnormalities. ECG should be done to predict cardiovascular risk in type 2 diabetic patients without cardiovascular symptoms. Clinical profile of patients with acute pancreatitis 急性胰腺炎患者的临床分析

Background: Acute pancreatitis is a serious condition that can involve the surrounding tissues and remote organ systems. The mortality rate for severe acute pancreatitis ranges from 2% to 10%, and the condition can be diagnosed based on abdominal pain consistent with acute pancreatitis, elevated serum lipase or amylase activity, and characteristic findings on imaging tests such as contrast-enhanced computed tomography (CECT) scan. Aims and objectives: The present study aimed to study the clinical profile of acute pancreatitis and complications of acute pancreatitis. Methodology: The study included 200 participants and was conducted retrospectively over a period of one year. Patients of all ages and genders who were clinically diagnosed with acute pancreatitis, and confirmed through ultrasound or CT scans, were eligible for inclusion. However, cases with incomplete documentation, as well as those with chronic pancreatitis or pancreatic malignancy, were excluded from the study. Conclusion: Acute pancreatitis is a significant contributor to increased morbidity and mortality in society. The clinical evaluation of patients, along with their radiological findings, has been shown to be closely associated with morbidity
