To do a prospective study on foreign bodies(FB) in the oropharyngeal (OP) / nasopharyngeal (NP) pathways done in the ENT department of a tertiary care private medical college hospital in the year 2022.To analyse, categorize, correlate the demographic data of patients presenting with FB in OP/ NP pathways with the nature of FB, clinical features, method of removal, duration of hospital stay in the ENT department of a tertiary care private medical college hospital in the year 2022.Study method: Prospective studyStudy centre: Saveetha Medical College, ThandalamStudy population: patients with nasopharyngeal foreign body attending the ENT departmentStudy duration: 8 months from January to August 2022Sample size: 50 1. Age more than 6 months 2. Both male and female subjects 3. Patients presenting to ENT department with oropharyngeal/nasopharyngeal foreign body. 1. Age less than 6 months 2. Patients presenting to ENT department with diagnosis other than FB in the OP/NP pathways.OP/NP FB has been seen frequently in the males than females. Most of them were single in number in the OP pathway. Highest number of patients are seen in the 30-39yrs age group of LIG, MIG coming from urban and sub-urban population in our study when compared to other studies which showed highest frequency in children. Fishbone was the frequently encountered FB. Almost all of them are acute cases presenting within 24 to 48hrs. With regards to symptoms, all of them except the persons with NP FB presented with dysphagia. In children upto 4yrs the symptoms were incessant cry, refusal to feed and excess salivation were seen. All OP FB were treated with either IDL or DL scopy. All NP FB treated with FOB. Anaesthesia was either local or short GA depending on the merits of each case. The duration of hospital stay was mostly 24 to 48hrs, except 3 cases who were geriatric people with co-morbidities. In our study Foreign bodies in the OP pathway were common in the adult male population in the middle age unlike children in other studies. Fish and chicken bones were common and in the low and middle income group coming from urban and sub-urban locality. Further studies are needed with larger sample size and longer duration to generalize the results to wider population.

The Vogt-Koyanagi-Harada syndrome (VKHS) is a rare, multisystemic, granulomatous inflammatory and autoimmune disease that affects the pigmented structures such as eyes, ears, skin, and hair. The syndrome mainly affects the eyes, followed by bilateral chronic panuveitis. A 34 years old female came with complaints of reduced vision and reduced hearing sensitivity to our hospital. She was diagnosed with VKHS in the ophthalmology department. Throughout the audiologist assessment, she presented bilateral sensorineural hearing loss, absent otoacoustic evoked emissions, and complaints about postural vertigo and acute tinnitus. The specific case reported presented sudden sensorineural hearing loss, vertigo, tinnitus, and bilateral ocular disease. Even though drug treatment was performed, the hearing loss remained.

Otological conditions range from a wide spectrum of congenital anomalies to inflammations, infections and complicated tumours that often not much is said about simple conditions such as myringitis. Bullous myringitis is not just a very unique condition but it also associated with a much-dreaded condition in the form of sudden SNHL and needs immediate diagnosis and timely treatment. Mucoceles are known to occur as a result of chronic accumulation of mucoid secretions within a sinus as a result of long-standing outflow obstruction secondary to inflammation. This can lead to extension into the orbital and intracranial cavities. It can also get infected to cause mucopyocele. The frontal sinus is the most common site for mucocele. Long-standing symptoms of chronic rhinosinusitis, if ignored, can lead to such complications. Clinical features include frontal pain and swelling. It may displace the orbit inferiorly and laterally. If neglected, it can progress to cause osteomyelitis and erode the posterior wall of the sinus to cause intracranial complications. Imaging is paramount in assessing the lesion's type and extent. Computed tomography can be used to delineate the bony erosions, whereas Magnetic Resonance Imaging helps characterize the lesion’s nature. Surgery remains the mainstay of treatment with the aim of draining the mucocele and removing the mucosa to prevent a recurrence. Surgical approaches can be both endoscopic and open, with the advantages and disadvantages of each.

Despite an alarmingly rise in prevalence of allergic rhinitis in the last 2 decades, yet till date it remains underdiagnosed, undertreated, or mistreated in India. The distinguishing features of allergic rhinitis are commonly misinterpreted with the common cold and thus early detection of allergic rhinitis is often delayed in primary care. Poorly controlled allergic rhinitis adversely impairs overall long-term productivity at work or school, quality of life (QoL) and can also trigger underlying comorbidities like asthma. Thus, early diagnosis and timely referral is the need of the hour for more effective management which will reduce the overall burden of allergic rhinitis in India. As allergic rhinitis needs long-term management, it is also paramount to prescribe highly efficacious, cost-effective treatment with minimal side effects like drowsiness to prevent disruption of patient’s QoL. However, current therapies including intranasal corticosteroids have adverse long-term complications and antihistamines like cetirizine, levocetirizine, and fexofenadine have varying degrees of sedation.With these concerns, a management algorithm was developed to aid for early diagnosis in primary care which provides cost effective option of right medication for a proper duration of time to optimize overall patient care. After a thorough discussion of key opinion leaders and evidence-based recommendations, it was highlighted that loratadine deserves to be considered as a first-line antihistamine therapy due to its rapid and long-lasting symptom relief, non drowsy, and low complication rate for long-term management of allergic rhinitis.

Congenital absence of stapedius is an exquisite entity, with an incidence of 0.5% and very few cases reported in live patients. We herein report a case of a middle-aged female presented with bilateral ear discharge and decreased hearing for more than a year, which, on examination, showed a large central perforation bilaterally, and an audiogram preoperatively revealed an average hearing loss of 45 dB and air-bone gap of 35dB bilaterally. Right endoscopic type I tympanoplasty was done, which unveiled absent stapedius muscle, tendon, and pyramid on the right side. The absent stapedius should circumspect the operating surgeon to look for other associated anomalies such as tympanosclerosis, fixed footplate and aberrant course of the facial nerve.

Neck dissection (ND) is a surgical procedure performed in treating head and neck cancer patients with or without cervical nodal neck metastasis. The aim of neck dissection is to achieve loco-regional control thus optimizing the cancer cure rate. Various complications may potentially occur following this surgery. Previous studies done have documented the complication rate following neck dissection surgery to range between 5% to 15%. Most of the complications are inevitable and various efforts have been made to try and reduce complications by modifying the aggressive surgical procedure. The objective of this study is to report the various complications, its rate in various neck dissection procedures, their management and to seek improved patient care and outcome. : A systematic review of 73 patients who underwent neck dissection for various head and neck cancers from August 2017 to July 2020. These patients were studied for post-operative complications and their management.Out of 73 patients, Vascular injury observed in the current study was internal jugular vein (IJV) and was seen in 22.7%) cases and was repaired intraoperatively. Throracic duct injury was identified in 3(4.1%) and it was identified and ligated intraoperatively. Immediate post-operative complication being hemorrhage was seen among 2 cases, both the cases were re-opened. Marginal mandibular nerve and spinal accessory nerve injury was observed in 3 cases and they both came under the delayed type of complications.: A careful preoperative assessment, meticulous surgical technique, high quality postoperative care and appropriate rehabilitation are the cornerstone of preventing and managing complications.

Research Approach: This study adopted a non experimental descriptive and comparative survey approach.The study was conducted at the cardio thoracic surgical out patient department of one of the selected hospital at Sahdol.The population comprised of all patients with coronary artery bypass graft attending the cardio thoracic surgical outpatient department of the selected hospital at Sahdol at the time of the study.The sample consisted of 75 patients with coronary artery bypass graft based on the criteria for sample selection. Non probability convenient sampling technique was adopted for the selection of sample. 64 samples (85.3%) were able to carry out the household activities such as cooking, washing the cloth, sweeping, grinding, ironing, gardening, minor repairing work at home moderately well and 11(14.7%) were able to perform these activities very well before surgery. However after the surgery the percentage of samples who performed the household activities very well increased by 5.3%.cBefore surgery 59 samples (78.7%) showed good interest in reading newspaper and watching TV. However after surgery this number reduced to 51, a reduction of 10.7%. that majority of the samples (84-94.7%) had good social well being before surgery. However after surgery the samples showed moderate quality of life in social wellbeing. Statistically there was no significant difference between the mean score of personal and household activities ( t-0.239 and 0.054,df=74,p<0.0 ) before and after surgery and there was significant difference between the mean score of outdoor and other activities (t-3.17and3.8,df=74,p<0.01 ) before and after surgery. The study concludes that the quality of life of patients who had undergone coronary artery bypass graft was better before surgery but after surgery the quality of life was better in physical well being whereas psychological and social wellbeing was reduced.

: Imaging of temporal bone helps the Otologists in preoperative surgical planning in ear surgery. Cone Beam Computed Tomography (CBCT) has much higher spatial resolution and clarity as compared to conventional X-Ray and much lower radiation exposure as compared to HRCT Temporal Bones.: To evaluate the change in management plan of Chronic Otitis Media (Mucosal) after CBCT. Also to determine the sensitivity and specificity of the CBCT findings by comparing with intra operative findings in the study participants. We conducted an observational descriptive study in the ENT Department of an academic Tertiary care hospital from October 2017 to October 2019. We included 95 patients whose management plan before CBCT was compared with the plan after CBCT and the percentage of cases undergoing a change in the management after CBCT was determined. The correlation of CBCT findings and intra operative findings in terms of specificity and sensitivity was also statistically calculated. : Subsequent to imaging, the management plan was changed in only 4 patients which was statistically non-significant. CBCT showed a 100% sensitivity, specificity, positive predictive value and negative predictive value to detect pneumatisation of the Mastoid, high jugular bulb and low-lying dura. The sensitivity to detect ossicular erosion was 98.9%. : Thorough clinical examination is the most important factor in decision making in patients with Chronic Otitis Media (Mucosal) and management plan remains unchanged after imaging. Though thorough clinical examination remains the gold standard in making a management plan of patients with Chronic Otitis Media (Mucosal), CBCT with its available details and minimal radiation helped to minimise intraoperative inadvertent injuries which might result due to normal anatomic variations like low lying dura, forward lying sinus, contracted mastoid cavity etc.
