Primary fungal epiglotitis is the isolated fungal infection of the epiglottis, without affecting the other body parts such as the larynx, lungs, pharynx, and oral cavity. It is an extremely rare clinical entity. Very few cases could be found in world literature. Pain throat and/or odynophagia are common clinical presentations. It can be secondary to inhaled steroid therapy which is usually mild.: In this article, we introduced a rare case of fungal epiglotitis in a 66 years old immunocompetent male presented with odynophagia without any other symptom or sign. In endoscopic laryngoscopy, using a 70 scope, a white lesion on swollen epiglottis was seen and rest of larynx was normal. No cervical lymphadenopathy was seen. Laryngeal area was tender on deep palpation. Patient was put on antibiotics and anti-inflammatory agents. There was no response even after 24 hers of starting treatment. Patient was put on empirically oral antifungal treatment and he improved in 6-8 hrs. Diagnosis of fungal epiglotitis was clinical, based on patient’s history, signs and sypmtoms. Signs included the inflammatory changes of the epiglotis and candida whitish discoloration of it. There was rapid and obvious improvement in condition of the patient, once started on oral antifungal treatment.

Allergic rhinitis is the most common form of rhinitis, worldwide, affecting close to 10% to 30% of the global population. Heredity and environmental exposures may contribute to a predisposition to allergies. Pharmacotherapy along with effective allergen avoidance measures forms the mainstay of management of Allergic Rhinitis. The evidence indicates that intranasal corticosteroids are more efficacious than are antihistamines in the control of Allergic Rhinitis symptoms. This "superiority" is principally related to their effect on nasal obstruction. The numbers of studies done to know the efficacy of combination sprays over steroidal sprays alone are very few and provide insufficient insight.The purpose of this study was to ascertain whether the combination nasal sprays should be prescribed as a standard medical management for allergic rhinitis or not, as to achieve better quality of life after treatment with maximum improvement in Total Nasal Symptom Score (TNSS). 100 subjects were taken up for this study, 50 in each group after fulfilling the inclusion and exclusion criteria. Subjects were allocated in two groups according to consultant’s preference. Subjects were evaluated for Total Nasal Symptom Score (TNSS) and score of Mini Rhino conjunctivitis Quality of Life Questionnaire (MiniRQLQ) before and after the treatment initiation The study was conducted in two groups based on drug given and both the groups were evaluated after 2 weeks and 4 weeks of medication on the basis of TNSS and MiniRQLQ score. Allergic Rhinitis was found to be more common in younger age group. Both the drugs were effective in reducing the Total nasal symptom scores (TNSS) and score of MiniRQLQ significantly (p<0.001). However, Combination Nasal Sprays (Intra-nasal corticosteroids with intranasal antihistamines) were found to be statistically superior when compared to only intranasal steroid sprays.Combination Nasal Sprays are highly efficacious and statistically better to steroid nasal sprays hence this study confirms the superiority of combination nasal sprays and recommends combination nasal sprays as first line of treatment for allergic rhinitis.

One rare complication of the same is fracture and aspiration of distal portion into the lower airway, mainly seen in metallic tubes. We report a very rare case, where aspiration of fractured PVC tracheostomy tube occurred, which was successfully removed by rigid bronchoscopy. The etiology and prevention measures are also being reviewed.

Cut neck injuries are sharp incised injuries in the neck which if not treated promptly may lead to lifetime morbidity or even death of the patient. To study the socio-demographical pattern, various modes of injury and the management of cut neck injuries. This study was conducted in Tezpur Medical College from the time period of 01/01/20 to 31/12/21. A total of 28 cases of cut neck injuries were included in the study after obtaining proper consent from the patient party. The socio- demographic pattern, modes of injury and the treatment provided were recorded and analysed. Out of 28 cases, 18 were males and 10 females. Majority of the cases were suicidal (15), followed by homicidal (10) and accidental (3). Patients from rural areas were 19 whereas 3 cases were from urban population. Tracheostomy was needed in 10 cases. Cut neck injuries are serious injuries that need prompt treatment, failing to which can be life threatening to the patient. In our study we found that most of the patients were from lower socioeconomic background from rural areas. We also found that male were more commonly affected than females with a male to female ratio of 1.8:1. Most of the injuries were suicidal in nature.

Adenoid cystic carcinoma is a slow growing and relentless salivary gland malignancy composed of epithelial and myoepithelial neoplastic cells that form various patterns, including tubular, cribriform, and solid forms. It is a rare tumor constitutes for less than 1% of head and neck malignancies and 10% of all salivary gland tumors. Palate is the most common site to be involved in the oral cavity followed by parotid gland and submandibular gland. They are usually asymptomatic, characteristically shows infiltrative growth and perineural invasion. In this paper a case of adenoid cystic carcinoma of soft palate in 52 year old male is presented.

Otalgia has a spectrum of causes and one among them is a foreign body in the EAC. Insects are not uncommon, but pose a threat by virtue of carrying infections that affect the patient locally as well as systemically.A 42-year female patient presented with acute onset otalgia, examination by otoendoscopy showed a foreign body over the posterior superior canal wall and it was removed, further microscopy then diagnosed it as Ambylomma Testudinarium and patient was treated accordingly.In tropical and developing countries such as India otologists must be aware and watchful of EAC infestations by arthropod vectors. Farmers are more prone to this zoonotic infection. Ticks consume a blood meal and hatch eggs, causing local irritation, perforations and hematogenic spread of infection.Tick infestations though rare are perilous due to the risk of tick-borne infections, which have grave complications. The management must be holistic including the otological perspective with systemic treatment.

: Basal cell carcinoma (BCC) is a slow growing, locally destructive, malignant tumor of the skin. It is derived from non- keratinizing cells that originate from the basal layer of the epidermis. After excision nasolabial flap, median forehead dorsal nasal flap, glabellar flaps, bilobed flaps, cheek and craniofacial flaps are used for nasal reconstruction. The Zitelli’s bilobed flap is one of the most useful flaps for nasal reconstruction. It is a simple double transposition flap and is designed to move more skin, without deformation. A 60-years old male patient reported to our department with complains of circular raised and reddish discoloration lesion on the left side of the nose. Surgical excision and reconstruction of defect using bilobed flap was planned under general anaesthesia. The flap was designed in such a manner that the flap could be turned over the defect on the ala of the nose and closure of the donor site could be done primarily without deformity. Surgical excision of the nose lesion was done and reconstruction was performed using bilobed flap. The patient has been followed up for 1year with no evidence of recurrence. The bilobed flap is a versatile and reliable flap for coverage of small skin and soft tissue defects of the lower third of the nose. It gives a successful outcome if it is designed well and performed properly. In this case it helped us to achieve a defect free cosmetically fine nose after excision of basal cell carcinoma nose.

This is a retrospective study analysing the demographics, clinical characteristics, radiological findings and treatment of the 54 patients of rhino-sino-orbital or cerebral mucormycosis in a tertiary health care centre in Jharkhand. All patients with probable or proven mucormycosis were included in the study. A significant number (45%) of patients belonged to rural areas. About 20% of patients did not give history of COVID infection denoting association of mucormycosis in mild or asymptomatic COVID patients. There was a delay of approximately 11 days in seeking medical care leading to poorer outcome in patients. Diabetes was a major risk factor associated with 83% of the patients. Steroid was given as treatment in 80%of the patients who had COVID - 19. Extensive disease was seen in 37% of the patients suggesting that the mucormycosis was more devastating during the outbreak as compared to pre- covid times.

Amongst the various cystic lesions of oral cavity, Lymphoepithelial cyst (LEC) is a rare cystic lesion that presents as an asymptomatic submucosal mass and is usually discovered during routine oral examinations. Most common site of occurence is the floor of the mouth but tonsillar involvement seems to be very rare. Pathogenesis involves the excessive accumulation of desquamated epithelial lining in the tonsillar crypt. This results in a dilated obstructed crypt of the palatine tonsil region which presents as a cystic lesion. Here we are reporting a case of 17 year old female with lymphoepithelial cyst of tonsil.
