Effectiveness Of Simulation- Based Learning In Term Of Competency Regarding Pre-Term Newborn Care Among Nursing Students: An Quasi Experimental Study 模拟学习对护生期前新生儿护理能力的影响:一项准实验研究

Background: Premature birth is a sudden event of birth below the 37th week of pregnancy. In the third trimester, that is, 27-40 weeks of gestation when the baby’s major stages of development take place, the baby receives a wonderful blood supply to its respiratory system that enables it to breathe for the first time. Material And Method: A Quasi- experimental Non- equivalent control group pre-test post-test design was adopted to assess the effectiveness of simulation based learning in terms of competency of nursing students. Data was collected from 114 students studying BSc. 3rd Year in M.M. College of Nursing, and M.M. Institute of Nursing, Mullana, Ambala, Haryana. Study participants were randomly allotted in experimental group (n=55) and comparison group (n=59) by using purposive sampling technique. The data was collected by using selected variables, Structured Knowledge Questionnaires, Objective Structured Clinical Examination (OSCE) and Clinical Decision Making Ability Questionnaires. Results: The study findings showed that majority of the nursing students (80%) were female in experimental group and (79.66%) in comparison group. The mean post-test knowledge score (26.22±5.24) of nursing students in experimental group was higher than the mean post -test knowledge score (22.46±6.54) of comparison group. The mean post-test skills score (45.40±7.45) of nursing students in experimental group was lower than the mean post -test skills score (45.51±8.36) of comparison group. The mean post-test clinical decision making ability score (17.84±2.84) of nursing students in experimental group was higher than the mean post-test clinical decision making ability score (14.39±5.47) of comparison group. The calculated ‘t’ value of post-test knowledge scores in experimental and comparison group (p=0.00*) and the calculated ‘t’ value of post-test skills scores in experimental and comparison group  (p=0.94*) The calculated t value of post-test  clinical decision making ability scores in experimental and comparison group(p=0.00*) was found to be statistically significant at 0.05 level of significance. Conclusion: The study concluded that Simulation based learning was effective in enhancing the competency in terms of knowledge, skills and clinical decision making ability regarding Preterm Newborn Care among Nursing Students.

Synthesis, Characterization, Antibacterial Activity And Molecular Docking Studies Of Novel Nitrogen Based Indole-2-One Schiff’s Bases 新型氮基吲哚-2-酮席夫碱的合成、表征、抗菌活性及分子对接研究

Series of some novel nitrogen based Indole-2-one Schiff’s bases (4a-4j) were synthesized by a conventional method via Schiff’s bases mechanism. The Istain Schiff’s bases was made by combining substituted isatin with hydrazine hydrate in ethanol to generate a compounds 3a-3d, which was then reacted with terephthaldehyde and 5-amino pyrimidine to obtained final compounds. All the synthesized compounds gave a good yield between 76-88% and structure was confirmed by FT-IR, LC-MASS and 1H-NMR spectral analysis. The novel nitrogen based Indole-2-one Schiff’s bases are screened for antimicrobial activity using standard agar diffusion method and using four bacterial stains (Staphylococcus aureus, Bacillus subtilis, Escherichia coli, Salmonella paratyphi). The most of the compounds such as 4a, 4d, 4e, 4h and 4j shows good activity against gram positive and gram negative bacteria. Finally, molecular docking studies were carried out by using AUTODOCK suite of MGL Tools by using Fgb1 receptor with PDB ID (3K4P). Among the docked ligands, compound 4a, 4d and 4h reported highest docking score (-9.8, -9.7 and -9.7).

Introduction: Therapeutic drug monitoring (TDM) measures the level of medications within the biological fluids. Valproic acid (VPA) is a narrow therapeutic index anti-epileptic. Measurement of its level is essential for the evaluation of safety and efficacy. Aim was to evaluate the benefit of TDM services in patients taking Valproic acid with respect to outcome improvement and prevention of adverse events linked to dosage errors, Methodology The cohort was designed to take random samples of patients taking Valproic acid, measuring their VAP blood levels at the correct time. Patient demographic data were collected through face-to-face and phone interviews and from patient charts. Results Results indicated that most patients were male and below 18 years of age. No toxicity was observed in the patient. Very few patients experienced side effects and attacks per week. Patients had no knowledge of medical history, Renal Function Test (RFT), Liver Function Test (LFT), Protein Level, and other medications. Discussion  The study found poor education of patients about therapeutic drug monitoring, and TDM is available in private facilities only. The patient had no education about Renal and liver function test protein levels and other medications being used by them. The study also found that side effects and frequency of attacks per week were not effects by valproic acid blood levels.

The Effect Of Safety Climate On Safety Behavior With Safety Motivation As An Intervening Variable On Contractor Workers In Pt X 以安全动机为中介变量的安全气候对承包者安全行为的影响

Safety climate in the workplace and safety behavior of workers are concerned in order to assess what factors are the causes of safe action on workers.  The purpose of this study was to analyze the effect of safety climate on safety behavior with safety motivation as an intervening variable in contractor workers at PT X. This type of research is analytical observational with a cross sectional design. The questionnaire was given to 380 respondents.  The data were analyzed using path analysis to see the direct and indirect influence of safety climate on safety behavior. Result research shows, as many as 361 respondents (95%) with good safety climate, 367 respondents (96.9%) with good safety motivation, and as many as 348 respondents (91%) with good safety behavior. From the results of the path analysis, it was found that there was no direct influence of safety climate on safety behavior on contractor workers at PT X (p value >0.05). However, indirectly safety climate affects safety behavior through safety motivation (p value = 0.001). It is recommended to workers to continue to increase safety motivation so as   to create safe and safe work behaviors.

Background: Every year, over half a million women continue to lose their lives during pregnancy, childbirth and postpartum. Although, pregnancy and childbirth are a normal process Pregnancy can sometimes present life-threatening health problems for a mother and her baby. These problems are called obstetric emergencies. An obstetric emergency may arise at any time during pregnancy, labor and birth. This study aimed to determine the emergency obstetric complications which occurred during labor and delivery, and evaluate their associations with socio-demographic, obstetrical characteristics and fetal complications. Methods: A descriptive cross-sectional study was conducted on 200 women who attended the delivery room of Maternity Teaching Hospital. The tool that used was an examiners-made questionnaire, containing 3 parts: demographic characteristics, obstetrical history, and emergency obstetrical complications which occurred during labor and delivery. The data were analyzed through frequency, percentage and Chi-Square test using the SPSS statistical program.                                                                                                                                                                                                            Results: Mean age of respondents was 30.15 years. We have found that the 39% of cases developed complication. Among the complications, most common problems:  high blood pressure, postdated delivery, bleeding before delivery, difficult labor, premature rupture of membranes and prolonged labor respectively. Conclusions:  The burden of emergency obstetric complications during labor and delivery is high. Efforts should be made at both the community and hospital levels to increase awareness regarding emergency obstetrical problems and reduce its associated morbidity and mortality.

A steady DC-Link Voltage, a CV voltage control and a reactive power control on the basis of the previous reviewers' analysis are the goals of the VSC-HVDC connecting controller. In order to achieve these objectives, the appropriate controller is required at both VSC-HVDC terminals. This paper presents a non-linear control approach to improve the VSC-HVDC relationship's control robustness. A sliding mode control (SMC) and an integrated proportional control (PI) were used for the configuration of a hybrid controller. The hybrid controller will provide mathematical simulation. This hybrid driver provides malfunction driving and the transient reliability of the VSC-HVDC link.  This paper presents a non-linear approach to control to improve the robustness of the VSC-HVDC controller. The hybrid controller configuration is a control of the sliding mode (SMC) and an integrated proportional control (PI) for a hybrid controller design. Fault ride the stability of this hybrid control unit is provided through and transient VSC-HVDC links. MATLAB and Simulink simulate the hybrid controller and increases the performance of the controller. The simulation induces a symmetrical grid-side malfunction to maintain transient stability.

CLINICAL OUTCOME OF HIGH DOSE RADIOTHERAPY WITH OR WITHOUT CONCURRENT CHEMOTHERAPY IN ESOPHAGEAL CANCER (EC): A STUDY FROM NORTH – EAST INDIA 食管癌高剂量放疗合并或不合并化疗的临床结果:一项印度东北部的研究

OBJECTIVE: The objective of this study is to evaluate the clinical outcomes of high dose radiotherapy 63 Gray (Gy) with or without concurrent chemotherapy in Esophageal Cancer (EC) patients of North-East, India. MATERIAL AND METHODS: All reported EC patients belonging to different states of North-East and treated with radical intent with high dose radiotherapy with or without concurrent chemotherapy during the period 2017-2018 (2 years) at State Cancer Institute (SCI),Gauhati Medical College (GMC), Assam, India were included in the study. This study was retrospective in nature and two years retrospective data were analyzed for clinical outcome. Statistical analysis of data was done using Statistical Packages for Social Sciences (SPSS) and Disease Free Survival (DFS) was calculated using Kaplan-Meier method. RESULTS: Total 105 patients were treated during the period. All patients received a total dose of 63 Gy radiation. Out of 105 patients, 76 patients (72.4%) received concurrent chemotherapy and 29 patients (27.6%) not received chemotherapy only due to co-morbidities. Here, at first follow up, 89 (84.8%) patients showed complete response, 10 (9.5%) showed partial response and 6 (5.7%) showed progressive disease. Treatment related toxicities were assessed. None of the patient developed > Grade 2 toxicity. Further, Median Disease Free Survival (DFS) was 13 months with estimated mean was 16.98 months. Also, Standard Error (SE) =1.683, Confidence Interval (CI) =13.682-20.279. CONCLUSION: The high dose radiation (63 Gy) with concurrent chemotherapy as weekly regimen of Cisplatin or Paclitaxel plus Carboplatin is an effective definitive treatment of Stage I – IVA EC with acceptable toxicity. Median DFS is minimal in EC irrespective of the treatment. The rate of survival is decreasing rapidly before the median DFS and the rate of survival is constant after the Median DFS.

Personalized nutrition is emerging as a promising approach to optimize health outcomes by accounting for individual genetic and metabolic differences. This review examines the current methods and approaches for integrating genetics and metabolomics into personalized nutrition strategies. We discuss the importance of understanding an individual's genetic makeup, including single nucleotide polymorphisms (SNPs), and their effects on nutrient metabolism and health outcomes. Additionally, we explore the role of metabolomics in elucidating individual metabolic profiles and identifying biomarkers for personalized dietary interventions. We also discuss the application of computational and machine learning techniques in data integration and analysis, as well as the ethical and regulatory considerations surrounding personalized nutrition. The ultimate goal is to provide a comprehensive overview of the current state of the field and identify future research opportunities to improve the implementation of personalized nutrition for disease prevention and overall health promotion.
