Introduction and Objective. The use of cannabinoids, particularly Δ9-tetrahydrocannabinol (THC) and cannabidiol (CBD), in dentistry has gained attention due to their therapeutic potential and complex pharmacokinetics. These cannabinoids interact with CB1 and CB2 receptors, which are crucial for managing pain, inflammation, and immune responses in oral health. This review examines the impact of cannabinoids on oral health, focusing on their therapeutic benefits and potential adverse effects in treating oral diseases. Review Methods. The literature review utilized databases such as PubMed, ResearchGate, and Google Scholar, covering sources from 2003 – 2024. Benign metastasizing leiomyoma (BML) is a rare condition predominantly affecting women of late reproductive age, characterized by metastasis of the uterine leiomyomas to distant organs. The case is presented of a 48-year-old woman with BML who initially presented with non-specific abdominal pain. She was diagnosed with a uterine tumour and multiple pulmonary metastases. Therefore, uterine sarcoma was suspected. However, pathologic examination of core needle biopsy of the uterine tumour revealed leiomyoma cells. Due to inconsistency between pathologic examination and imaging, hysterectomy was performed. The final pathologic diagnosis confirmed leiomyoma in the uterus. To exclude another malignancy, thoracotomy with open lung biopsy was performed. Histopathological examination showed the same cells as in the uterine leiomyoma, resulting in the diagnosis of BML. Gonadotropin-releasing hormone (GnRH) analog was administered. After 15 months, the patient remained stable without disease progression.

Introduction. According to 2023 statistics in the United States, penetrating eye injuries are not infrequent. Mishandling a penetrating eye injury can lead to irreversible blindness of the the patient. Therefore, proper response from doctors and prompt surgical intervention with a well-planned procedure is necessary. Two cases of penetrating eye injuries that occurred in young men are presented. Due to the implemented treatment, both patients regained full visual acuity. Case Reports. In the first case, a 27-year-old male presented with a penetrating injury to his left eyeball. treatment involved vitrectomy, laser coagulation of the retina, and injection of gas into the vitreous chamber. In the next case, a 32-year-old male was admitted to hospital with a penetrating injury to his right eyeball and vitreous haemorrhage. An emergency posterior vitrectomy was performed with removal of the foreign body. Conclusion

Sciatica is a clinical condition characterized by pain radiating along the roots of the sciatic nerve, extending from the lumbosacral spine and buttock, through the posterior surface of the thigh and lower leg down to the foot. One of the frequent cause is piriformis syndrome where the sciatic nerve is compressed by the piriformis muscle. There are many diagnostic tests and imaging studies available, but the most crucial aspect is to exclude other causes of sciatica. The aim of the review is to present the literature describing causes, diagnosis and treatment of the piriformis syndrome. Review Methods. The review is based on 46 papers found in PubMed and PubMedCentral databases using key words: ‘piriformis syndrome’ and ‘anatomical causes/diagnosis/treatment’, published between 1947–2023. Brief description of the state of knowledge. The variability in the course of the sciatic nerve and anatomical variations of the piriformis muscle have different impacts on the onset of piriformis syndrome. The diagnosis of this condition primarily aims to exclude other causes of sciatica, as there are no precisely confirmed diagnostic criteria. The preferred treatment is non-operative. When this is unsuccessful, surgical intervention may be considered. Summary. There are numerous causes of piriformis syndrome. There is a discrepancy in the results of various studies regarding the impact of different anatomical variations of the sciatic nerve on the onset of symptoms of this condition. The primary goal of research should now be to conduct a large, representative study on patients diagnosed with sciatica, focusing on the presence of anatomical variations.

Introduction and Objective. Chronic kidney disease (CKD) is a major health issue worldwide affecting 10–14% of the globe’s population, with a climbing trend. CKD is associated with a variety of factors, including obesity, which has grown increasingly prevalent and adds to the pathogenesis of the disease. The aim of the review is to summarise pathomechanism and current treatment methods of chronic kidney disease. Additionally, using this knowledge, treatment suggestions are provided for different patient groups. Review Methods. A literature search was conducted in the PubMed database with inclusion criteria ‘English language’ and ‘free full texts’, and publications date between 2018 – 2023. A total of 1,172 results were found; 28 publications were ultimately included in the review. Brief description of the state of knowledge. The pathomechanisms include haemodynamic changes, oxidative stress, adipokine dysregulation, and correlations to insulin resistance and diabetes. Weight loss by proper nutrition and exercise is a beneficial approach to manage CKD in obese patients. Bariatric surgery has proven a realistic alternative due to improvements in kidney function and obesity-related comorbidities. There are several options for pharmacotherapy in CKD and obesity, including SGLT2 inhibitors, GLP-1 analogues, agents targeting the renin-angiotensin-aldosterone system and bardoxolone methyl. Addressing obesity plays an essential role in alleviating CKD risk and progression. Conclusions. Early intervention involving lifestyle changes, weight reduction strategies, surgical procedures, and medication provides a holistic approach to addressing CKD in obese people. Recognizing the complex interplay of variables that contribute to CKD enables targeted therapy approaches and better patient outcomes.

Introduction and Objective. The Ketogenic Diet (KD) is a diet consisting of the restriction of protein, carbohydrates, and fluids. KD is used in the treatment of epilepsy and is currently the foundation for a therapeutic approach for drug-resistant epilepsy. The aim of the study was to review the current literature, and provide information about the application of the Ketogenic Diet in children with treatment-resistant epilepsy. Review Methods. Literature in English and Polish was reviewed via PubMed, Google Scholar and The Wiley Library. 250 publications were taken into consideration from which 50 were selected: meta-analysis publications, review articles, randomised controlled trials, and research articles, with emphasis on the most recent information on the topic. About 88% of the publications selected were published in 2017 or later. Brief description of the state of knowledge. Taking into consideration randomised controlled trials of 472 children with drug-resistant epilepsy, the results indicate a statistically significant reduction in seizure frequency (SFR ≥ 50%) in the KD-treated group, compared to the control group. The greatest improvement was observed in patients following a KD with a ketogenic ratio of 2.5/3:1 and the optimal time to initiate KD being before the age of two years. The highest chance of success was noted in the infant population in whom a complete elimination of epileptic seizures is possible. Summary. Randomised controlled trials indicate that the Ketogenic Diet has a positive effect on reducing the occurrence of seizures in paediatric patients with drug-resistant epilepsy.

Introduction. Clostridium difficile infection most commonly manifests in patients with antibiotic-associated diarrhea, ranging from very mild to severe pseudomembranous colitis. Recurrent C. difficile infections remain a serious clinical problem, occurring in approximately one in five patients. Recurrence of infection despite antibiotic therapy is often due to disruption of the intestinal microbiota. Objective The aim of this review is to summarize knowledge of current treatments and explore new therapies for Clostridium difficile infection, excluding vaccines under development and bacteriophage therapy, with a particular focus on patients with recurrent infections. Review Methods. The review is based on 59 articles on the pathophysiology, epidemiology and treatment of clostridium difficile infections found in PubMed databases published between 2009–2024. Brief description of the state of knowledge. Current treatments are mainly based on antibiotic therapy, or, for severe antibiotic-resistant forms, the faecal microbiota transplantation (FMT) method. In patients with recurrent infections, prolonged antibiotic therapy or sequential therapy with vancomycin and rifaximin is used. FMT is suggested for second or subsequent recurrent infections. Hopes are pinned on oral microbiome preparations, antibodies, new antibiotics, non-toxic strains or antibiotic degraders. Summary. The treatment of recurrent infections is a difficult problem that requires a broader view. Emerging therapies with promising results focus both on antibiotic therapy that eliminates toxin-producing bacteria, and on modifying the microbiota and reducing the conversion of spore forms of these bacteria into toxin-producing forms.

Clostridium difficile infection most commonly manifests in patients with antibiotic-associated diarrhea, ranging from very mild to severe pseudomembranous colitis. Recurrent C. difficile infections remain a serious clinical problem, occurring in approximately one in five patients. Recurrence of infection despite antibiotic therapy is often due to disruption of the intestinal microbiota. Objective The aim of this review is to summarize knowledge of current treatments and explore new therapies for Clostridium difficile infection, excluding vaccines under development and bacteriophage therapy, with a particular focus on patients with recurrent infections. Review Methods. The review is based on 59 articles on the pathophysiology, epidemiology and treatment of clostridium difficile infections found in PubMed databases published between 2009–2024. Brief description of the state of knowledge. Current treatments are mainly based on antibiotic therapy, or, for severe antibiotic-resistant forms, the faecal microbiota transplantation (FMT) method. In patients with recurrent infections, prolonged antibiotic therapy or sequential therapy with vancomycin and rifaximin is used. FMT is suggested for second or subsequent recurrent infections. Hopes are pinned on oral microbiome preparations, antibodies, new antibiotics, non-toxic strains or antibiotic degraders. Summary. The treatment of recurrent infections is a difficult problem that requires a broader view.
