Introduction: Surgery and anaesthesia may affect the outcomes of cancer. The aim of the study was to evaluate the effect of propofol or sevoflurane on cancer biomarkers such as interleukins, adhesion molecules, and EGFR. Material and methods: Eighty patients scheduled for colorectal cancer surgery were randomised to either propofol or sevoflurane anaesthesia. Blood samples for interleukin (IL)-6, IL-8, IL-10, tumor necrosis factor α (TNF-α), interferon α (IFN-α), soluble intracellular adhesion molecule-1 (sICAM-1), soluble vascular cell adhesion molecule-1 (sVCAM-1), sE-selectin, and sEGFR measurements were obtained before induction of anaesthesia, at the end of surgery, and 72 h postoperatively. Results: Complete samples were obtained from 71 patients. Demographic data and anaesthesia/surgery-related data were similar between the two groups. There were significant differences produced by sevoflurane vs. propofol on the sE-selectin (median (IQR) 57.1 (59.2) vs. 42.7 (22.9) ng/ml, p = 0.011) and sEGFR (median (IQR) 49905.7 (22673.5) vs. 25.657.2 (13842.1) ng/ml, p < 0.001) concentrations postoperatively, while sEGFR plasma levels also showed a significant difference during surgery (median (IQR) 32964.5 (14402.5) vs. 25567.0 (13315.4) ng/ml, p = 0.04). IL-10 levels were significantly higher in the propofol group postoperatively (median (IQR) 13.7 (18.5) vs. 14.9 (66.6) pg/ml, p = 0.05). Conclusions: Given the role of EGFR and adhesion molecules on tumour progression and the generation of metastases, the inhibitory effect of propofol observed in this study might prove useful in the future. Further studies in larger populations investigating the effect of anaesthetic agents on these biomarkers are warranted.

Introduction: Hepatitis C virus is a blood-borne infection and one of the major global problems. In Egypt, the prevalence is reported to be the highest. Infection through unsafe medical procedures is considered now the primary cause of infection. The study aims to assess the knowledge, attitude and practice of doctors regarding infection control guidelines in Ismailia governorate. Material and methods: A cross sectional study was conducted on a sample of physicians (355) in Ismailia governorate using a questionnaire to collect data on their knowledge and attitude concerning the control measures related to their years of experiences. Results: Most of the physicians (89.6%) regardless of their years of experience have had contact with blood products. Those who had a history of needle stick injury accounted for 56.3%, with a statistically significant difference as to the highest number being middle career physicians. Concerning knowledge about infection control policies in their hospitals, it was relatively low among all groups. Conclusions: Despite all efforts exerted by the infection control professionals, infections still remain a major unpleasant side effect of health care, often causing harm to patients.. Although Egypt has made great progress in implementing the infection control procedures and policies, there is still a huge problem as the practices of physicians do not really reflect their knowledge and they still do not follow the infection control policies. Education, monitoring, improved availability of resources and disciplinary measures are necessary to improve infection control in health facilities that should be applied to all physicians.

Introduction: Photodynamic therapy is a minimally invasive clinical treatment modality for a variety of premalignant and malignant conditions combining a photosensitizing drug, oxygen and light irradiation. Hematoporphyrin is an organic photosensitizer, which mediates inhibition of endothelial cell proliferation and induces apoptosis. Human serum albumin is an endogenous drug carrier for hematoporphyrin. The present study aimed to investigate the hematoporphyrin binding to human serum albumin, which is its transport protein. Material and methods: The chemical reagents were hematoporphyrin (Hp), human serum albumin (HSA) and bovine serum albumin (BSA). In the experiment two techniques were used: spectrofluorimetry and UV-Vis absorption spectrophotometry. Results: The binding sites for Hp were identified in the tertiary structure of HSA by fluorescence quenching technique. The experiment with BSA delivered additional data on Hp-albumin interactions close to Trp135. The participation of tyrosyl residues apart from tryptophanyl ones was discussed. A decrease of the polarity in the binding sites, testifying to possible hydrogen bonding in the binding sites, was also described. The binding and quenching constants Hp-HSA and Hp-BSA were determined as well as the number of binding sites. Conclusions: Hp locates in subdomain IIA in the tertiary structure of HSA. The location in subdomain I close to Trp135 is also possible. Hp is also able to interact within tyrosyl residues.

Introduction: Hematoporphyrin is a photosensitizer used in photodynamic therapy of various malignant diseases. It is carried to the cancer tissue by serum albumins. Spectrofluorimetric spectra of hematoporphyrin–serum albumin complexes were examined in vitro. Material and methods: The chemicals were: hematoporphyrin, human serum albumin and bovine serum albumin. The spectra were recorded on a Kontron SFM-25 Instrument AG at two excitation wavelengths: λex = 280 nm and λex = 295 nm. The spectra of hematoporphyrin 1.5 × 10 –5 M as well as spectra of complexes of hematoporphyrin–human serum albumin (1.5 × 10–5 M Hp – 1.25 × 10–6 M HSA) and hematoporphyrin–bovine serum albumin (1.5 × 10–5 M Hp – 3.5 × 10–7 M BSA) were recorded repetitively for 8 days and compared to the initial spectrum. Results: Formation of a complex with human serum albumin extends the stability of the hematoporphyrin spectrum. This extension is greater at excitation λex = 295 nm. Different stability of complexes with bovine and human serum albumins most likely does not result from an actual lower stability of bovine serum albumin complexes, but from the fact that dissimilarity in the structure of both albumins enables additional spectroscopic observations within subdomain IB in the bovine serum albumin molecule. Conclusions: Spectrofluorimetric spectra are stable longer when hematoporphyrin forms a complex with human serum albumin. The present data may be important for understanding the mechanism of hematoporphyrin transportation to the target cancer tissue and effectiveness of photodynamic therapy.
