RESULTS ANALYSIS OF PERIODIC MEDICAL EXAMINATION BY DERMATOVENEREOLOGISTOF WORKERS EMPLOYED AT THE METALLURGICAL ENTERPRISESOF THE SVERDLOVSK REGION 斯维尔德洛夫斯克地区冶金企业职工皮肤性病定期体检结果分析

We estimated the prevalence and structure of skin diseases among workers employed in metallurgicalenterprises of the Sverdlovsk region for the production of titanium and copper. The prevalence andnature of skin diseases in 528 workers employed at two enterprises of the Sverdlovsk region in theelectrolytic production of copper and titanium were studied by means of periodic medical examinations.Skin diseases of various etiologies and pathogenesis were detected in 27.3 % of workers. Mycoses of feetand onychomycosis had the highest prevalence (19.3 % of the examined), and the prevalence amongcopper production workers was higher than among those employed in titanium production enterprises.Titanium production workers have higher morbidity of allergic dermatosis (1.7 % of those surveyed).Vascular skin lesions in the form of vascular spots, the etiology, pathogenesis and clinical significance ofwhich remain unknown are detected in production workers previously involved in aluminumproduction at other enterprises. The presented data emphasize the importance of the development andimplementation of preventive measures, early detection and treatment of skin diseases, improvement ofthe clinical examination system for workers in non-ferrous metallurgy enterprises.

USING THEMETHODOLOGY OF CARCINOGENIC RISKS ASSESSMENT IN COPPERPYROMETALLURGY 用热铜冶炼致癌风险评价方法

We conducted a hygienic assessment of the chemical factors contribution of the production environmentto the formation of carcinogenic risk of workers employed in the reverberatory and blast smelting ofcopper-containing ores, compared with autogenous processes. Inorganic arsenic compounds have beenshown to be a major factor forming a carcinogenic risk in copper smelting. The greatest prognosticvalues of carcinogenic risk for similar professions of metallurgical shops are observed duringreverberatory and blast smelting, rather than during bath smelting due to the difference in theequipment used. The main measure to reduce the carcinogenic risk of blister copper production should bethe technical reequipment of smelters with the introduction of autogenous processes.

Современные аспекты профпатологической помощи работникам сельского хозяйства

На основе анализа актуализированных данных об условиях труда в аграрном секторе экономики, региональных уровнях профессиональной заболеваемости, доступности медицинских организа- ций жителям села и профессиональной заболеваемости работников сельского хозяйства выявле- но снижение числа работающих во вредных условиях труда в период с 2011 по 2017 г. на 19,2 %, которое не сопровождалось соответствующим падением удельного веса этой когорты в общей структуре занятых в аграрном секторе, доля которых уменьшилась на 4,4 %. Одновременно с ростом обеспеченности учреждений сельского здравоохранения врачами-профпатологами с 43,3 до 60,0 % от уровня потребности охват работников сельского хозяйства периодическими меди- цинскими осмотрами увеличился с 75,6 % в 2011 г. до 82,7 % в 2017 г. За период с 2011 по 2017 г. число субъектов РФ, на территории которых были зарегистрированы случаи профес- сиональных заболеваний, сократилось на 25,6 %: с 39 до 29 регионов. При этом уровень профес- сиональной заболеваемости вырос на 32,3 % с 1,27 в 2015 г. до 1,68 в 2017 г. на 10 000 работаю- щих в отрасли. По разным годам наблюдения в 43,7−57,2 % случаев профессиональные заболева- ния диагностировались при самообращении в центры профпатологии. Большинство случаев профзаболеваний выявлено центрами профпатологии, функционирующими на базе научных ор- ганизаций Роспотребнадзора, НИИ гигиенического профиля федерального подчинения и учреж- дений высшего профессионального образования Минздрава РФ.

SCHOOL-AGED CHILDREN MORBIDITY DYNAMICSOF ALIMENTARY-DEPENDENT DISEASES IN MEZHDURECHЕNSK DURING THE2010–2016 PERIOD 2010 - 2016年mezhdurechЕnsk地区学龄儿童食源性疾病发病动态

We analyzed the prevalence of alimentary-dependent diseases of the gastrointestinal tract, endocrineorgans, blood and hematopoietic organs and certain disorders involving the immune mechanism, inparticular anemia, obesity in schoolchildren aged 7–17 years. An increase in the number of thesediseases was found in schoolchildren, associated with malnutrition and nutritional status disorders overa 6-year period. There has been a significant increase in obesity, the prevalence of which has increasedover the past six years, among children 5 times. Among schoolchildren living in the city, diseases of thedigestive organs associated with malnutrition account for 27.2 % of all alimentary-dependent diseases.We found that disease prevalence rates caused by the alimentary factor are influenced by the schoolenvironment factors. Formation of eating disorders in high school associated with the intensity of theeducational process and training of senior schoolchildren during the school year only from the secondshift create prerequisites for eating disorders and is a factor leading to an increase in the number ofchildren with inadequate nutritional status.

We gave practical aspects of epidemiological monitoring organization for brucellosis in the area unaffectedby this infection. The positive experience of interagency cooperation during the organization ofpreventive and anti-epidemic measures in focus of brucellosis in the territory of the Lipetsk region isdescribed. We presented the genetic analysis results of brucella cultures isolated in epizootic foci of cattlebrucellosis and identified the most probable regions of the Russian Federation from which brucellosisinfection could be brought to the region.

We evaluated diets for students of secondary vocational education organizations in Chelyabinsk.Students of secondary vocational education organizations are provided with one- or two-time organizedmeals at the expense of the regional budget, as well as additional food in the form of pantry products. Allorganizations have identified any deficiencies in diets formation. The results indicate the need tooptimize diets in secondary vocational education organizations, improve the quality of staff training inorganizations providing nutrition services, and strengthen production and state sanitary andepidemiological monitoring of nutrition in these organizations.

FEATURES OFCHILDREN'S MORBIDITY LIVING IN AREA OF INFLUENCE ENTERPRISES FORALUMINA PRODUCTION 影响氧化铝生产企业地区儿童发病特点

The article presents the study results conducted to study the level and structure of children morbidity ofpreschool age living in the territories in zone of influence enterprises for metallurgical aluminaproduction. We obtained data indicating the formation of residential areas where the multiplicityexceeding of MPCad on aluminum, formaldehyde, fluoride compounds, phenol, benzo (a) pyrene,suspended matter in atmospheric air is 1.16-43.6 times and on aluminum, nickel and manganese indrinking water is 1.25-42.2 times. We found that the content of chemical compounds (aluminum,manganese, chromium, nickel, phenol, formaldehyde, and ion fluoride) in the biological media of childrenof the observation groups was up to 4.2 times higher than those of the children of the comparison group.The morbidity of respiratory diseases, diseases of the blood, hematopoietic organs and disordersinvolving the immune mechanism, diseases of digestive, musculoskeletal and nervous systems inchildren living in the area of anthropogenic influences, up to 8.0 times higher than those of childrenliving in the territory in the absence of enterprises influence. We proved the dependence of theregistration frequency of the revealed pathology on the concentration in biomedia of chemicals(aluminum, copper, manganese, chromium, nickel, phenol, formaldehyde and ion fluoride), tropic to theorgans of the respiratory, digestive, nervous and musculoskeletal systems.

NEW HEARING LOSS PROBLEMSOLUTIONS OF FLIGHT CREWS MEMBERS IN CIVIL AVIATION 民航机组人员听力损失新问题的解决方法

The flight crews members of Civil Aviation aircraft were object of study. The article discusses issuesaffecting the official indicators of occupational hearing loss among flight crews members of Russian CivilAviation aircraft. We presented the main expert and diagnostic errors in addressing the correlation betweenthe hearing organ diseases and the profession, showed new solutions to reduce hearing loss indicators,which consist in improving the methodology for assessing the acoustic load and improving the medicalstaff skills in the development of hearing impairment in persons of flight professions of Civil Aviation.

ENSURINGBIOLOGICAL SAFETY DURING LYOPHILIZATION FOR MICROORGANISMS OFI–II PATHOGENICITY GROUPS 确保i - ii致病性微生物冻干过程中的生物安全性

The article shows the presence of pathogen single cells of a dried sample in swabs taken from the camerasurfaces of sublimation machines after lyophilization is completed, which may entail the possibility ofrelease in the form of aerosol of viable cells of lyophilized microorganisms and lead to the creation ofdangerous to personnel emergency situation. We proposed the use of additional isolation equipment -laminar box equipped with H14 class HEPA filters, the purpose of which is to protect the operator andthe environment when working with pathogenic biological agents. The swabs taken from the internalsurfaces of the laminar box and lyophilic machines at the end of the process gave negative results, whichminimized the aerosol formation risk in room of the bacteriological box. We selected personal protectiveequipment, disinfectants for processing equipment and the final product, developed an algorithm forpersonnel actions, which made it possible to significantly increase the level of protection againstpathogens of laboratory staff and the environment.
