Psychotherapy with deaf college students: perception of process. 聋人大学生心理治疗:过程知觉。

Abstract The process of psychotherapy between a hearing therapist and a deaf client has been a largely unexplored area. This paper provides a distillation of the experiences of a hearing therapist who conducted short-term psychotherapy with deaf college students. The focus of the paper is on areas of conflict and important decisions in the beginning phases of therapy that are important for long-term success. The paper consists of two major areas of interest: 1) the communicative process; and 2) early transference and countertransference. Two primary methods of communication were used with deaf clients: 1) oral with lipreading and partial hearing; and 2) use of an interpreter. Major difficulties with the oral method include partial comprehension by therapist and client, the use of communication problems as resistance, and fatigue with resultant withdrawal. Abstract Mitral valve prolapse is the most common heart disease seen in college and university health services. It underlies most arrhythmia and many chest pain complaints. Auscultatory findings vary; only 60% have the click, only 48% have the murmur, and in half of those with murmur, it was heard better at the upper-left sternal border than at the apex. In 15%, neither click nor murmur was heard. Only two patients had major arrhythmia. Endocarditis prophylaxis is indicated. Activity and exercise restrictions are usually unnecessary.

Abstract An apparently healthy six-foot, two-inch, 220-pound football player at the University of South Carolina collapsed and died prior to practice in the training room at Williams-Brice Stadium. There was no previous history of cardiovascular or other major medical abnormality. A discussion of the final autopsy diagnoses, the click-murmur syndrome or mitral valve prolapse, and its apparent association with this case will be discussed.

Abstract The purpose of this paper is to familiarize health professionals with the clinical manifestations of illness encountered by healthy subjects upon acute exposure to high altitude. Acute mountain sickness (AMS) consists of mild (headache, lethargy, anorexia) and severe (pulmonary and cerebral edema) forms which are probably a continuum of the adverse effects of hypoxia. The former occurs usually at more than 6000 feet, and is self-limited with rest and analgesia; the latter occurs at more than 12,000 feet, can be rapidly fatal, and must be treated with descent and/or oxygen. Acetazolamide may provide prophylaxis for mild symptoms; Lasix and morphine should not be used for severe AMS unless close monitoring is available. The etiology of AMS is not clear but may be related to one's ventilatory response or the response of the pulmonary and cerebral vasculature to hypoxia.

Psychiatric symptomatology in a deaf college population affecting diagnosis and treatment. 聋人大学人群的精神症状学影响诊断和治疗。

Abstract As the mental health needs of the deaf receive increased recognition, issues of diagnosis and treatment become vital. Several characteristic behavioral patterns are frequently encountered which often confuse diagnosis, particularly in clinicians inexperienced with hearing-impaired individuals. These behavioral characteristics of suspiciousness, the use of odd or idiosyncratic language, difficulties in expression of affects, and acting out behaviors may create a tendency to overdiagnose serious mental illness. In addition, an understanding of the impact of deafness on an individual's personality development is crucial to an intelligent choice of treatment approach. These difficult issues of differential diagnosis and treatment are discussed in the context of the psychotherapeutic experiences of a hearing and nonsigning clinician working with deaf college students.
