Background: Cognitive dysfunctions are considered as a poor prognostic factor that influence health-related quality of life in multiple sclerosis. Objective: The aim of the study was to evaluate the impact of cognitive impairment on the quality of life in multiple sclerosis patients. Methods: This study included 60 MS patients treated at the Department of Neurology, Clinical Center University of Sarajevo. Inclusion criteria were clinically definite diagnosis of multiple sclerosis, 18 years of age or older and were able to give written informed consent. Cognitivefunction was evaluated by the Montreal Cognitive Assessment (MoCa) screening test. Quality of life was evaluated by SF36 questionnaire. Results: 88.33% of patients had cognitive impairment with 68.33% with mild cognitive impairment. Abstraction (60,83%), language (56,66%), executive functions (53.66%) and delayed recall (28.33%) were rated the worst. The median value of SF-36 score was 54.1 (27.7-70.01). The lowest results were achieved in the QOL domains of psycial limitation with a median value of 12.5 (0-75) and emotional limitation 33.3 (0-100). It is found statistically significant correlation of the MoCa score with social functioning, energy, vitality and general health (p<0.05) and physical functioning (p<0.001) domains of quality of life, as well as with SF -36 total scores (p<0.05). Among group of patients with cognitive impairment, statistically significant positive correlation between cognitive status mental health HRQOL domain (rho=0.427; p<0.001) was found. Conclusion: Cognitive impairment is very often presented in patients with multiple sclerosis with significant contribution to a poorer quality of life. It is associated with physical and emotional limitations, as well as poorer mental health. Further studies are needed, especially when we take into account very important clinical and prognostic role of cognition in multiple sclerosis.

Background: A significant number of patients who underwent surgical myocardial revascularization (CABG) have various comorbidities, including left ventricular diastolic dysfunction (LVDD). Objective: The aim of this paper is to show that patients with preoperative LVDD are at an increased risk of postoperative adverse events and have a worse prognosis compared to patients without LVDD. Methods: The study included 116 patients who underwent myocardial revascularization (CABG) performed at Medical Institute Bayer in Tuzla over a period of 1 year. None of the patients had previously undergone CABG or valvular surgery, or undergone stent implantation (PCI). The presence of LV diastolic dysfunction was detected by echocardiography, and the patients were divided into two groups: with and without LVDD, and further classified into three degrees of LVDD. Results: Of the total number of subjects who underwent surgical revascularization of the myocardium without echocardiographically detected LVDD, 24.1% were patients, and 75.9% were patients with LVDD. In the group with LVDD, the range of days spent in the intensive care unit (ICU) is significantly higher compared to patients without LVDD. Also, patients with LVDD had a significantly higher number of hours of inotropic support, a significantly higher number of cases of tachycardia, predominantly atrial fibrillation (POAF). In this group of patients, a greater number of other adverse events were observed, such as myocardial infarction, cardiac or renal failure, TIA or CVI. Of the total number of patients in the study, 107 patients survived surgical myocardial revascularization and had a satisfactory recovery while 9 (7.8%) survived, all from the group with LVDD. Conclusion: Patients with preoperative diastolic dysfunction of the left ventricle, especially with III degree of dysfunction, are exposed to a higher postoperative risk of adverse events such as: atrial fibrillation, prolonged stay in the intensive care unit, the need for longer-term support with inotropes, and the fatal outcome was recorded exclusively in patients with LV diastolic dysfunction.

Background: preeclampsia, a pregnancy-specific condition, is characterized by hypertension and proteinuria after 20 weeks of gestation. In addition, preeclampsia may have an impact on women’s health beyond their pregnancies, and has been associated with increased risks for future hypertension and cardiovascular disease, such as coronary heart disease and stroke. Objective: the aim of this paper was to present a case of preeclampsia with letal outcomes of mother, but child is saved by Cesarea section. Methods: presentation of a case of preeclampsia in a patient who was hospitalized at the Clinic for Gynecology and Obstetrics of the Sarajevo Canton. Cas report: the patient underwent continuous non-invasive monitoring of vital parameters. On admission TA 209/141 mmHg, HR 99/ min, SpO2 95 %, with oxygen support on a nasal catheter 4L/min. Ebrantyl 25 mg iv is prescribed immediately (measured pressure TA 191/148 mmHg), and then Ebrantyl in continuous infusion, 0.9% NaCl iv to maintain the venous path, laboratory findings are taken and the internist and neurologist are called in consultation. Upon arrival at the intensive care unit at 3:57 PM, the competent physician for the consular internal examination finds the patient unconscious, with no pressure or pulse, resuscitation in progress, guided by anesthesia. Conclusion: the conducted study proved that high blood pressure leads to preeclampsia, which in this case resulted in the death of the mother, while the baby survived.

Background: Spinal hydatid cyst represents a rare form of parasitic infection and is a serious clinical condition that can cause severe consequences such as paraplegia, tetraplegia, or radiculopathy. Objective: In this article, we report a case of a rare lumbar hydatid cyst in a 50-year-old man. Case presentation: We report a rare case of primary lumbar hydatid cyst. Recurrence was diagnosed by magnetic resonance imaging (MRI), although the patient had surgery and was taking albendazole regularly. Conclusion: MRI imaging should be used to diagnose the disease. Complete resection is the only treatment that can prevent the lesion from invading adjacent tissues. Regardless of all measures, the disease has a high recurrence rate.
