Photodynamic Diagnosis-guided Ureteroscopic Surgery for Upper Urinary Tract Tumors 光动力诊断引导输尿管镜下手术治疗上尿路肿瘤

For upper urinary tract urothelial carcinoma (UTUC), the European Association of Urology and the National Comprehensive Cancer Network guidelines have recommended ureteroscopic surgery (URS) for cases with low-risk tumors (low-grade biopsy and cytology, <1.5–2 cm, unifocal, no invasion on imaging), and cases with bilateral tumors, a solitary kidney, or severe renal dysfunction. However, the 2-year recurrence rate after surgery is remarkably high, leading to the need for salvage radical nephroureterectomy. Furthermore, unlike robotic or laparoscopic surgery, the surgical procedure for URS has not been standardized due to the technical specificity of laser devices and ureteroscopes. Therefore, this procedure is performed only in limited facilities with extensive experience internationally. In this review, we introduce the current issues of URS for UTUC and the use of the photodynamic diagnosis-guided URS procedure, which we are implementing to overcome these challenges.

Current Status and Problems of Photodynamic Diagnosis in Bladder Cancer 膀胱癌光动力学诊断的现状及问题

アラグリオ顆粒剤による5-アミノレブリン酸塩酸塩(5-aminolevulinic acid: ALA)を用いた光力学診断(Photodynamic diagnosis: PDD)は,2017年より本邦において実臨床で使用可能となった.ALA-PDDにより診断精度の向上および治療成績の改善を認めた.同時に副作用として低血圧を認める場合があり,念頭に置いて実施する必要がある.

Experience of Kidney-preserving Surgery for Upper Tract Urothelial Carcinoma and Evaluation of Biopsy Proven Pathological Features 保肾手术治疗上尿路上皮癌的经验及活检病理特征的评价

上部尿路上皮がん(upper urothelial carcinoma: UTUC)に対する温存療法は1 cm未満の小さい孤立性の低グレードかつ低リスクのUTUCなら腎盂尿管鏡(ureteropyeloscope: URS)にレーザーアブレーションを併用した低侵襲治療で腎温存療法の成績が比較的良好であることが最近報告されている.加えてURSの利点として,治療前に病理組織診断が可能であることが挙げられる.我々の施設では低リスクの腫瘍に限らず臓器温存としてlaser resection with vaporization(LRV)に取り組んでいる.我々はLRVを施行した37症例(男性30例・女性7例)患者の生検病理と再発との関連を調査した.lymphovascular invasion(LVI)に関しては陰性が35症例(94.6%)と最も多かった.生検病理で判定が可能であったグレード・浸潤様式・LVI・同時に存在するcarcinoma in situ(CIS)病変・生検pTステージを多変量解析で検討したところINFとLVIが独立した再発予測因子となることが判明した.症例数が限られるためさらなる検討が必要であるが生検病理でのLVI陽性あるいは浸潤性の発育様式と判明した症例には厳重なフォローが必要となることが示唆された.

Comparative Evaluation of Photodynamic Therapeutic Probes for Peripheral Lung Cancer Based on Light Propagation Simulation 基于光传播模拟的外周肺癌光动力治疗探针的比较评价

This study evaluates photodynamic therapeutic probes for peripheral lung cancer by a preclinical trial based on computational simulation. By light propagation simulation based on a Monte Carlo method, light distributions in tissues by a direct transmission light irradiation probe and a diffused light irradiation probe were calculated to compare each irradiated volume. The simulated results demonstrated quantitatively that the diffused light irradiation probe can irradiate the tumor tissue more efficiently than the direct transmission light irradiation probe for peripheral lung cancer.
