Simultaneous determination of five pesticides residues in Anoectochilus roxburghii by HPLC-MS/MS method HPLC-MS/MS法同时测定金线莲中5种农药残留

Objective: To establish a HPLC-MS/MS method for simultaneous determination of five pesticides residues including tebuconazole,metalaxyl-M,thiophanate-methyl,oxytetracycline and abamectin in Anoectochilus roxburghii.Methods: The samples were extracted by acetonitrile solution and purified by QuEChERS method.Separation was performed on Agilent Eclipse Plus C18 column(50 mm×2.1 mm,1.8 μm) with the mobile phase consisting of 0.1% formic acid(A) and acetonitrile(B) by gradient elution(0-1.0 min,90% A;1.0-3.0 min,90%-70% A;3.0-5.0 min,70%-30% A;5.0-8.0 min,30%-10% A;8.0-14.0 min,10% A;14.0-15.0 min,10%-90% A;15.0-17.0 min,90% A).Detection was performed on a triple quadrupole tandem mass spectrometry with electrospray ionization source operating in positive ion mode under multiple reaction monitoring(MRM) mode.Results: The calibration curves of five pesticides showed good linearity coefficients(r>0.99).The average recoveries ranged from 72.7% to 94.9%(n=6),and the relative standard derivations(RSDs) were within 1.8%-10.3%(n=6).The detection limits of tebuconazole,metalaxyl-M,thiophanate-methyl,oxytetracycline and abamectin were 0.6,0.2,0.3,0.2,5.0 μg/kg,and the quantitation limits of 5 pesticides were 2.0,0.5,1.0,0.6 and 10.0 μg/kg respectively.Conclusion: The established method was proved to be sensitive,simple and reliable,and could be applied for the determination of these five pesticides in Anoectochilus roxburghii and quality control in the planting process.
目的:建立同时测定金线莲中戊唑醇、甲霜灵-M、甲基托布津、土霉素和阿维菌素5种农药残留量的HPLC-MS/MS方法。方法:样品用乙腈溶液提取,用QuEChERS法纯化。在Agilent Eclipse Plus C18柱(50 mm×2.1 mm,1.8μm)上进行分离,流动相由0.1%甲酸(A)和乙腈(B)组成,梯度洗脱(0-1.0分钟,90%A;1.0-3.0分钟,90%-70%A;3.0-5.0分钟,70%-30%A;5.0-8.0分钟,30%-10%A;8.0-14.0分钟,10%A;14.0-15.0分钟,10%-90%A;15.0-17.0分钟,90%A)在多重反应监测(MRM)模式下以正离子模式操作的具有电喷雾电离源的四极串联质谱法。结果:5种农药的线性关系良好(r>0.99),平均回收率为72.7%~94.9%(n=6),相对标准偏差在1.8%~10.3%(n=65种农药的定量限分别为2.0、0.5、1.0、0.6和10.0μg/kg。结论:建立的方法灵敏、简便、可靠,可用于金线莲中这5种农药的含量测定和种植过程中的质量控制。

Investigation and analysis of the status of clinical research coordinators 临床研究协调员现状调查与分析

Objective: To investigate the current status of clinical research coordinators(CRCs) in Fudan University Shanghai Cancer Center,so as to optimize the current management mode and improve the quality of drug clinical trials.Methods: A survey was made among 104 CRCs in the hospital by using a self-made questionnaire,which included professional background,problems facing CRCs and their opinions or suggestions concerning the current management mode.Finally,the study made a descriptive statistical analysis of the obtained results.Results and Conclusion: Most of the CRCs in the hospital had an educational background of above undergraduate course in related medical science,over 80% of them had service years of less than 3 years,and less than 50% of them used to undertake clinical trials independently before they took up their posts in the hospital.The majority of the CRCs had a love for their profession and a sense of accomplishment,but only 20% of them had a sense of home-coming and approval.The feelings of heavy workload and dissatisfaction with salary widely existed among CRCs.Poor cooperation between subjects and pharmacy intravenous admixture service(PIVAS) and CRCs occasionally occurred in work.CRCs put forward a series of suggestions on the current CRCs management mode in the hospital.Such measures as setting up CRCs access threshold,raising salary,enriching training contents,reforming the work system of PIVAS,enhancing the sense of responsibility of clinical researchers,and holding regular informal discussions should be taken to intensify the construction of the CRCs team both professionally and from the aspect of team stability in the long run.
目的:了解复旦大学上海肿瘤中心临床研究协调员(CRCs)的现状,以优化现有的管理模式,提高药物临床试验的质量。方法:采用自制问卷对我院104名中心进行调查,问卷内容包括中心的专业背景、面临的问题以及对当前管理模式的意见或建议。最后,对所得结果进行描述性统计分析。结果与结论:该院CRCs大部分具有相关医学本科以上学历,80%以上的CRCs工作年限在3年以下,不到50%的CRCs在上岗前曾独立从事过临床试验。绝大多数人对自己的职业有热爱和成就感,但只有20%的人有回家的感觉和认同。工作压力大、对薪酬不满意的情绪普遍存在。在工作中,偶尔会出现受试者与静脉药物调配中心(PIVAS)、中心配合不佳的情况。对医院目前的中心管理模式提出了一系列建议。应采取设置CRCs准入门槛、提高薪酬、丰富培训内容、改革PIVAS工作制度、增强临床研究人员的责任感、定期举行非正式讨论等措施,从专业和长期稳定两方面加强CRCs队伍建设。

Study on the relationship between adverse drug reactions of carbamazepine and HLA-B*1502 genetic polymorphism and blood concentration 卡马西平不良反应与HLA-B*1502基因多态性及血药浓度关系的研究

Objective: To study the relationship between genetic polymorphism,blood concentration and adverse drug reactions(ADRs) of carbamazepine(CBZ),so as to promote clinical safety and reasonable use of CBZ.Methods: Clinical medical data of the inpatients treated in Ruijin Hospital Affiliated to School of Medicine,Shanghai Jiao Tong University from 2017 to 2019 were collected retrospectively.Data concerning HLA-B*1502 gene detection and serum concentration of CBZ were collected to analyze the relationship between HLA-B*1502 genotype and ADRs of CBZ in the patients.Results: A total of 148 patients were enrolled for study,of whom 101 patients received HLA-B*1502 genotypes detection,with the genotype of 34 patients being positive,accounting for 33.66%.In the 148 patients,78 were treated with CBZ(53.79%),among whom 60 patients(86 case times) received serum drug concentration detection of CBZ.The serum drug concentration in 53 case times was within the recommended concentration range,accounting for 61.63% of the detected case times,and the serum drug concentration in 32 case times(37.21%) was lower than that of the recommended data.CBZ-related ADRs occurred in 12 cases(15.38%),of which one patient with HLA-B*1502 homozygous mutation developed severe skin ADRs after taking CBZ.Conclusion: There is close association between HLA-B*1502 genotype and skin ADRs induced by CBZ.The treatment window of CBZ is narrow,and the rate of ADRs might be associated with blood concentration of CBZ.Gene detection and blood concentration detection are of great significance to the improvement of the safety of CBZ in clinical practice.
目的:研究卡马西平(CBZ)的基因多态性、血药浓度与不良反应的关系,以促进临床安全和合理使用。方法:收集上海交通大学医学院附属瑞金医院2017~2019年住院患者的临床医学资料回顾性地。收集HLA-B*1502基因检测及血清CBZ浓度资料,分析HLA-B*15022基因型与患者CBZ不良反应的关系。结果:共有148例患者被纳入研究,其中101例患者接受了HLA-B*1502基因型检测,其中34例基因型阳性,占33.66%。148例患者中,78例接受了CBZ治疗(53.79%),其中60例(86例次)接受了CBZ-血清药物浓度检测在推荐浓度范围内,占检出病例次的61.63%,32例次(37.21%)血清药物浓度低于推荐数据。12例(15.38%)出现CBZ相关的不良反应,其中1例HLA-B*1502纯合子突变患者服用CBZ后出现严重的皮肤不良反应浓度检测对提高CBZ临床应用的安全性具有重要意义。

A case of complex multisite carbapenem-resistant Klebsiella pneumoniae infection and literature analysis 复杂多位点耐碳青霉烯肺炎克雷伯菌感染1例及文献分析

Objective: To analyze a case of complex multisite carbapenems-resistant Klebsiella pneumoniae(CRKP) infection,and to evaluate the rationality of the treatment scheme,so as to provide reference for rational use of drugs.Methods:The clinical and laboratory data of the patient were collected,and the clinical efficacy was observed,laboratory indexes and the results of etiological examination were compared,treatment effect was evaluated and relevant literature was reviewed.Results and Conclusion: In the treatment of the patient,meropenem,amikacin combined with fosfomycin were used.Literature retrieval revealed that there were many kinds of antimicrobial therapy options for CRKP infection,but cure rate was not clear.The prevalence of CRKP was more feasible in the intensive care unit(ICU).Reducing irrational use of broad-spectrum antibacterials and unnecessary invasive manipulation were effective strategies for the control of CRKP prevalence and reduction of economic burden on patients.
目的:分析1例多部位碳青霉烯类复杂耐药肺炎克雷伯菌(CRKP)感染病例,评价治疗方案的合理性,为合理用药提供参考。方法:收集患者的临床和实验室资料,观察临床疗效,比较实验室指标和病原学检查结果,评价治疗效果,并查阅相关文献。结果与结论:本组采用美罗培南、阿米卡星联合磷霉素治疗。文献检索显示,CRKP感染有多种抗菌治疗选择,但治愈率尚不清楚。CRKP的患病率在重症监护室(ICU)更为可行。减少广谱抗菌药物的不合理使用和不必要的侵入性操作是控制CRKP患病率和减轻患者经济负担的有效策略。

Establishment and application of intelligent pre-review platform of Chinese drug pieces prescriptions 中药饮片处方智能预审平台的建立与应用

Objective: To realize pre-review of Chinese drug pieces prescriptions and improve the accuracy and efficiency of prescription review,so as to ensure the rationality of Chinese drug pieces prescriptions.Methods: The Chinese drug clinical prescription review system was developed,the pre-review rules and database of Chinese drug pieces prescriptions were established,and finally the smart pre-review of Chinese drug pieces prescriptions was realized.Results: The Chinese drug clinical prescription review system made the prescriptions review standards unified,fast and accurate.100% pre-review of Chinese drug pieces prescriptions was realized,and the rationality and standardization of Chinese drug pieces prescriptions obviously improved.Finally,the Chinese drug pieces clinical prescription pre-review system obtained the copyright of national computer software(No.2019SR0757904).Conclusion: The Chinese drug clinical prescription pre-review system has realized the intelligent pre-review of Chinese drug pieces prescriptions,with good practicability.For this reason,it is worth further popularization.
目的:实现中药饮片处方的预审核,提高处方审核的准确性和效率,确保中药饮片处方合理性。方法:开发中药临床处方审核系统,建立中药饮片处方预审规则和数据库,实现中药饮片处方的智能预审。结果:中药临床处方审核系统实现了处方审核标准的统一、快速、准确,实现了中药饮片处方100%预审,中药饮片处方的合理性和规范性明显提高。最终,中药饮片临床处方预审系统获得了国家计算机软件版权(编号:2019SR0757904)。因此,值得进一步推广。
