What can go wrong with the medication adherence a patient suffers due to diabetes distress? A narrative review 糖尿病患者的服药依从性会出现什么问题?叙述性回顾

Background: Type 2 Diabetes Mellitus (T2DM) is a major public health concern associated with distress. Distress is a rational emotional response to living with T2DM. Diabetes distress has been found to significantly affect the clinical outcome of T2DM. However, little is known regarding how distress may affect medication adherence. Objective: To investigate the relationship between distress and poor medication adherence in people with T2DM. Method: A literature search was conducted to identify relevant research articles published from 2014 to 2022 in three electronic databases (Scopus, PubMed, and SpringerLink) regarding “diabetes distress”, “T2DM management” and “medication adherence”. In addition, a search was also carried out using Google Scholar. Results: A total of seven articles were identified. Amongst these articles, three studies revolved around evaluating the effect of diabetes distress on blood glucose control. All articles mentioned the significant association between distress and poor medication adherence, leading to seemingly comprehensive findings regarding the impact of diabetes distress on T2DM patients. Conclusion: It is important for healthcare providers, particularly pharmacists, to understand the holistic feature of T2DM patients.

Pharmacists' perception about design and content of medicine labels on prescription services in community pharmacies 药师对社区药房处方服务药品标签设计及内容的认知

Background: Differences in perception among pharmacists and the absence of standards or regulations for labelling prescribed medicines in Indonesia make the labels patients receive have different designs and contents. Incomplete information and insufficient medicine label design can lead to medication errors. Objective: This study aimed to determine the pharmacists’ perception of the design and content of prescribed medicine labels. Method: This study was conducted by survey using an online questionnaire developed based on the "FIP Guidelines for The Labels of Prescribed Medicines" and Regulation of the Minister of Health of the Republic of Indonesia number. 73 the year 2016 concerning Pharmaceutical Service Standards in Pharmacies. Questionnaires were distributed through personal chat and social media groups of community pharmacists. Result: A total of 925 pharmacists from 32 provinces of Indonesia participated in this study. Respondents considered the most important information on the label of prescribed drugs namely instructions for drug use (84.6%), patient name (81.6%), pharmacy name (76.1%), special instructions (71.5%), and pharmacist name (63.9%). For the label’s design, most respondents preferred printed and handwritten labels (61.2%). Conclusion: It can be concluded that different pharmacists' perceptions affect the labelling of prescribed medicines. There is a need for regulations or standards of labelling prescribed medicines.

Assessment of the phytochemicals and anthelminthic activities of Adiantum raddianum and Kibatalia arborea extracts 黄菖蒲和木菖蒲提取物的植物化学成分及驱虫活性评价

Background: A limited spectrum and inappropriate use of anthelmintics lead to anthelmintic resistance. Among Indonesian medicinal plants, Adiantum raddianum (Suplir) and Kibatalia arborea (Jelutong Pipit) have shown antihelmintic properties. Objective: To reveal the anthelmintic activity as preliminary screening of A. raddianum and K. arborea leaves. Method: This study used the antihelmintic microdilution test towards methanol extracts at 500 µg/mL. The mortality of Caenorhabditis elegans expresses the presence of anthelmintic activity. Phytochemical screening was determined using thin-layer chromatography. Result: The methanol extracts of both extracts had anthelmintic activity with mortality values of 14.941% and 15.134%, respectively. Both extracts contain polyphenols, flavonoids, terpenoids, and alkaloids found only in K. arborea. Conclusion: This study reveals that the methanol extract of A. raddianum and K. arborea leaves have anthelmintic activity, which could be related to the findings of phytochemical constituents.

The optimisation and standardisation of Indonesian house dust mites allergenic extract as a desensitising agent 印尼屋尘螨致敏提取物脱敏剂的优化与标准化

Background: The different sequences of the allergen protease can induce different immunogenicity for the development of immunotherapy. Dermatophagoides pteronyssinus that has Der p1 as major protein will be developed as local allergen to be immunotherapy for treating rhinitis allergy. Objective: This study aims to optimize and standardize the local Indonesian House Dust Mites Allergenic Extract from the harvesting into the extraction process. Method: The harvesting media used is corn flour placed in certain conditions to produce the mites. After harvesting the flour in different weeks, the flour was extracted and the lipid content and Der p1 assay were measured. Result: The result of the study is that the 3 weeks-harvesting time came higher for those parameters than the other harvesting time (p < 0.05). Conclusion: Thus, it was concluded that the standardized harvesting and extraction process had been developed to produce the standardised Indonesian House Dust Mites Allergenic Extract.

Application of ATC/DDD methodology to analyse antibiotic consumption in internal medicine department: A review ATC/DDD方法在内科抗生素使用分析中的应用综述

Background: Antibiotic resistance has become a significant problem due to its impact such as higher medical costs, prolonged hospital stays, and increased mortality. The Internal Medicine Ward has a high consumption of antibiotics, particularly for gastroenteritis, typhoid, diabetic ulcer, urinary tract infection, and sepsis. The DDD/ATC systems applied in hospitals may provide valid data to evaluate antimicrobial usage as a global standard method. Objective: To explore the antibiotic use pattern in the internal medicine department based on the DDD/ATC system. Method: The authors explored articles from the database in PMC, Research Gate, and Google Scholar from July 11st, 2020, to July 21st, 2020. The authors included original articles/research, case reports, and meta-analyses from 2006 to 2020, which discussed DDD/100 patient-days or DDD/100 bed-days antibiotics in the internal medicine ward. A major criterion was to ensure all subjects in publications were adults aged above 18. Result: Penicillin is the most consumed (127.9 DDD/100 bed-days), followed by Cephalosporin (41.42 DDD/100-bed-days) and Fluoroquinolone (25.87 DDD/100 bed-days). Conclusion: The most widespread antibiotic use in Internal Medicine in many countries showed an improvement in rational antibiotic consumption.

Overview of compliance to taking iron-folic acid supplementation at primary healthcare centre during the Covid-19 pandemic Covid-19大流行期间初级卫生保健中心服用叶酸铁补充剂的依从性概况

Background: One of the causes of anaemia in pregnant women is the insufficient intake of iron-folic acid (IFA) supplementation. However, the COVID-19 pandemic disrupted the mobility of people, including pregnant women who were receiving IFA supplementation. Objective: To assess pregnant women’s compliance in taking IFA supplementation tablets at the primary healthcare centre in the North Region of Kediri City during the COVID-19 pandemic. Methods: This study was observational with a cross-sectional approach that utilised secondary data from the register of pregnant women and the list of IFA supplementation distribution in the North Region of Kediri City primary healthcare centre from January to December 2021. Results: There were 767 visits made by pregnant women throughout 2021, with data collected from 366 pregnant women. Of these, 28% (101 people) of pregnant women took more than 90 tablets of IFA supplementation. In comparison, pregnant women had experienced anaemia during pregnancy were 11% (41 people). Pregnant women who experienced chronic energy deficiency 5% (19 people) are indicated by an upper arm circumference of less than 23.5cm. Conclusion: The compliance of pregnant women who took IFA supplementation tablets at primary healthcare centres was relatively low, so that it could cause anaemia.

Methods: This study was observational with a cross-sectional approach that utilised secondary data from the register of pregnant women and the list of IFA supplementation distribution in the North Region of Kediri City primary healthcare centre from January to December 2021. Results: There were 767 visits made by pregnant women throughout 2021, with data collected from 366 pregnant women. Of these, 28% (101 people) of pregnant women took more than 90 tablets of IFA supplementation. In comparison, pregnant women had experienced anaemia during pregnancy were 11% (41 people). Pregnant women who experienced chronic energy deficiency 5% (19 people) are indicated by an upper arm circumference of less than 23.5cm. Background: Ferulic acid is a phenolic acid compound that has anti-inflammatory and anti-thrombosis activity. However, ferulic acid has the disadvantage of poor absorption. Structural modifications can be made to increase the biological activity of the compound. In this research, the structure of ferulic acid was modified into 4-(4-methoxy)benzoyloxy-3-methoxycinnamic acid. Objective: The purpose of this research is to predict the activity of 4-(4-methoxy)benzoyloxy-3-methoxycinnamic acid. Method: It was carried out using the Pass Online Prediction, Autodock 1.5.7, Discovery Studio and Maestro Schrödinger 2020-1 software. Result: The activity prediction results showed that 4-(4-methoxy)benzoyloxy-3-methoxycinnamic acid had anti-inflammatory and anti-thrombotic activity. The molecular docking results showed that 4-(4-methoxy)benzoyloxy-3-methoxycinnamic acid had lower free bond energy values than ferulic acid, hence, predicted to have greater activity. Molecular dynamics also reveal better stability interactions of the 4-(4-methoxy)benzoyloxy-3-methoxycinnamic acid with COX-1 protein than the ferulic acid. Conclusion: The compund, 4-(4-methoxy)benzoyloxy-3-methoxycinnamic acid is feasible to be synthesised.

Background: Tumor lysis syndrome (TLS) is a life-threatening oncologic emergency because of the release of tumour cell components such as uric acid, which can lead to hyperuricemia and acute kidney injury (AKI). Objective: To analyse serum uric acid, creatinine, and BUN levels pre- and post-allopurinol administration. Method: This study was a prospective observational study conducted from March to July 2020. Inclusion criteria were haematological malignancies in paediatric patients with TLS and high-risk TLS who received allopurinol 10 mg/kg/day in 2-3 divided doses. Collected data were the serum uric acid, creatinine, and BUN levels pre- and post-allopurinol administration. Result: There were 14 sample in total during the study. There was a significant difference in uric acid level on day six after allopurinol administration compared to baseline in patients with high-risk TLS (p < 0.05). Conclusion: Allopurinol was inadequate in reducing uric acid levels in TLS patients but adequate in reducing uric acid levels in patients with high-risk TLS.

Background: Malaria is an infectious disease caused by the Plasmodium parasite and transmitted by the Anopheles mosquito. Plasmodium falciparum is a parasite that causes the most deaths. Currently, there is resistance to various antimalarial drugs. For this reason, it is necessary to search for new antimalarial agents. Medicinal plants have been shown to play an important role in treating malaria for thousands of years. Some of Euphorbiaceae family plants contains terpenoid compounds, which are compounds with antimalarial activity. Objective: The present review aims to provide an overview of the terpenoid compounds from Euphorbiaceae family as an antimalarial. Method: Comprehensive information on Euphorbiaceae family from 2002-2022 was searched for literature relevant to major science-based data, including Scopus, Science, ScienceDirect, Pubmed, and SciFinder, using appropriate keyword combinations. Result: A total of 15 papers were included in this review. The terpenoids isolated from 14 species of Euphorbiaceae family and reported to possess antimalarial activity are presented. Conclusion: Terpenoids are found in almost all parts of Euphorbiaceae family plants and are reported to have moderate to high antimalarial activity. Screening of antimalarial terpenoid activity in Ephorbiaceae family plants can be a key step in the source and development of new antimalarial drugs.

Synthesis of 2-hydroxybenzohydrazide derivatives with microwave irradiation and activity against Escherichia coli 微波辐照合成2-羟基苯并肼衍生物及其抗大肠杆菌活性研究

Background: This study synthesised derivatives of 2-hydroxybenzohydrazide using microwave irradiation (green chemistry) and tested their antibacterial activity against Escherichia coli. Objective: The reaction yielded N'-Benzylidene-2-hydroxybenzohydrazide, N'-(2-Methoxybenzylidene)-2-hydroxybenzohydrazide, and N'-(4-Methoxybenzylidene)-2-hydroxybenzohydrazide compounds. Prior to compound synthesis, docking was performed with the ENR inhibitor receptor (Pdb.1C14), to determine the antibacterial activity test. Method: The synthesis was carried out using a condensation reaction. Identification of compounds was carried out by UV-VIS, FTIR, and 1H-NMR spectroscopies. Antibacterial activity against E. coli was evaluated using the well method. Molecule docking study was performed using Mollegro virtual docker. Results: The percentage yield of the synthesis of 2-hydroxybenzohydrazide derivatives was obtained at 68-81%. Docking with the ENR inhibitor receptor for these compounds resulted in a lower rerank score than the initial compound (methyl salicylate). These compounds exhibited antibacterial activity against E. coli. Conclusion: These compounds can synthesised using microwave irradiation. N'-(4-methoxybenzylidene)-2-hydroxybenzohydrazide has greater activity against E. coli.
