Aim — Infertility is a global health problem. The next-generation sequencing and panel testing are offering new opportunities to further diagnose the reason for male infertility. The aim of this paper is to provide a better insight into the currently available panels for male infertility due to impaired spermatogenesis. Methods — We conducted research in the Genetic testing registry by using the keywords „infertility“, „male infertility“. We also gathered information about the number of tested genes, coverage of the panels, turnaround time, and any additional tests, which could be ordered. Results — As a result there were eleven laboratories, offering panel testing for male infertility, which tested for 230 different genes, but 65 genes (28.26%) from the different panels had an uncertain role for the tested condition. Cystic fibrosis transmembrane conductance regulator was the only gene, suggested by all laboratories. Conclusions — Next-generation sequencing could be extremely helpful in the diagnostic process of male infertility. However, clinicians should be aware that some of the included genes have an uncertain role for male infertility.

Objective — To justify the use of low-intensity transcranial magnetic therapy (TCMT) in combination with electrostimulation (TES) for the correction of neuroendocrine-immune disorders. Material and Methods — Fifty adolescent boys with exogenous constitutional obesity 13-15 years of age were examined. The control group consisted of 30 boys aged 13-15 years without obesity and concomitant pathology. The examination included an assessment of complaints, anamnesis of the disease, life history, objective data, hormonal status, neurotransmitters and cytokines. By random sampling, the group of obese children was divided into two subgroups: subgroup 1a (n=24), who received TCMT in combination with TES, and subgroup 1b – comparison group (n=22), who received placebo therapy with disconnected electrodes. Both groups of children received identical recommendations regarding the nutrition. The effectiveness of the therapy was evaluated after 3 months. The treatment course comprised 10 procedures. Statistical processing of the obtained data was carried out using the statistical software package Statistica 8. Results — An increase in the levels of testosterone and β – endorphin as well as a decrease in estradiol and kisspeptin, which was accompanied by a decrease in body weight was observed in children of subgroup 1a (n=24) under the influence of TCMT in combination with TES. In subgroup 1b (n=22) receiving placebo therapy with disconnected electrodes, there was also a decrease in body weight, but at a much lower level. Conclusion — The use of TCMT in combination with TES led to the normalization of neuroendocrine-immune disorders and contributed to weight loss.

Background — Musculoskeletal pain is one of the most common problem among office workers. The main reason is related to the long time spent sitting and the associated lack of physical activity. Musculoskeletal pain can affect not only their work-related performance, but also their personal performance and quality of life. Research has mainly focused on the effects of physical activity in managing musculoskeletal pain in office workers, while research is sparse pertaining to the possible role of mental imagery on reducing musculoskeletal pain in office workers. While mental imagery has only a minor effect on pain modulation in healthy individuals, its effect appears to be more pronounced in those with chronic pain. Aim — This commentary attempts to present beneficial mental imagery techniques that can be used solely or in combination with physical exercise in an office-based setting to improve musculoskeletal pain to enhance work performance and quality of life among office workers.

Objective — The study objective was to perform a transthoracic echocardiographic study of the remodeling of the left atrium and the maximum and minimum diameters of pulmonary veins, as well as remodeling regression in the process of heart failure treatment, in patients with chronic heart failure (CHF) and different forms of atrial fibrillation (AF). Material and Methods — The prospective study included patients having CHF with paroxysmal AF (n=38) and permanent AF (n=36) and without arrhythmia (n=225); the patients had their maximum and minimum diameters of the pulmonary vein measured, additionally to the standard echocardiographic protocol, before and after six months of standard therapy including a loop diuretic. Results — Structural changes in the left atrium and pulmonary veins of patients with CHF having permanent atrial fibrillation were significantly more distinct than those in patients with paroxysmal AF, in patients without arrhythmia, and in the control group. A decrease in the maximum and minimum diameters of the pulmonary vein followed by termination of AF attacks within a year of observation was detected after CHF treatment with a loop diuretic in patients with paroxysmal AF and permanent AF. Conclusion — Prescribing loop diuretics to patients with pulmonary vein dilatation detected during transthoracic echocardiography in subjects with paroxysmal AF and permanent AF results in a decrease in PV diameters and termination of AF attacks in paroxysmal AF.

Aim — This study aims to evaluate the effect of supportive educative system intervention on Tuberculosis (TB) patient’s family support. Material and Methods — This study was a quantitative study with a quasi-experimental design, conducted in the Public Health Center (PHC) of Kedungkandang and Ciptomulyo with 48 respondents. The respondents were divided into the control group with standard PHC intervention (n=24) and the treatment group with standard PHC intervention added with the supportive educative system (n=24). The data were analyzed by paired t-test and independent t-test. Results — There was a significant difference in family support between treatment and control. The results indicated that a supportive educative system significantly increases family support (p=0.003). Interestingly, PHC standard intervention showed improvement in family support (68.46±73.58) than supportive educative system (74.29±75.83). Supportive educative system interventions were less effective than standard PHC interventions. It could be influenced by sample characteristics, time of the study, control of variables, and other factors. Conclusion — Supportive educative interventions are effective in improving family support but are no better than standard PHC interventions.

Study objective — To evaluate how morphological features of intervertebral disc would affect the outcomes of finite element modeling of axial load in the cervical spine, C3-C5, in order to predict the risk of occurrence and course of dorsopathies. Material and Methods — Three-dimensional models of the cervical spine vertebrae were generated from the computed tomography data of a volunteer (24 years old male without detected pathology of his neck). Intervertebral disc models were developed in two configurations. For each model, we performed a finite element investigation of the stress-strain state with the same loading conditions. The load-displacement curves were compared with the experimental data generated from the results of previously conducted in vitro experiments. Results — The maximum and mean displacement values for the isotropic model were 1.15 mm and 0.73 ± 0.45 mm, respectively. For anisotropic model, maximum and mean displacement values were 0.86 mm and 0.47 ± 0.24 mm, correspondingly. Predicted displacement values for both models matched the experimental data fairly well. Stress profiles of intervertebral discs and stress diagrams of facet joints were calculated. Conclusion — The proposed geometric and constitutive configurations of the intervertebral disc take into account specific morphological features at low computational costs, thereby facilitating the modeling of degenerative disc changes.

The urgency of obesity issue is undeniable. Obesity is now considered the most important risk factor of cardiovascular diseases. Numerous studies have demonstrated the negative effect of excessive adipose tissue on structural and functional changes in the heart that lead to development of left ventricular hypertrophy, arrhythmias and conduction abnormalities, as well as progression of diastolic and systolic heart failure. High prevalence of obesity – so high that it can be called a pandemic – greatly contributes to the increased incidence of cardiovascular diseases. Studying the problem of obesity is a priority area of focus for modern medicine. This article describes hormonal, metabolic and hemodynamic features of obesity impact on cardiovascular system and describes pathogenetic mechanisms of cardiovascular pathology development.

Hypogonadism is a clinical condition comprising symptoms and laboratory evidence of testosterone deficiency and low androgen receptor sensitivity. The importance of hypogonadism in clinical practice is often underestimated. Androgen deficiency habitually occurs in various conditions causing abnormal functioning of many organs and systems, as well as impairing the quality of life in patients. Androgen deficiency often occurs with various somatic diseases. Chronic obstructive pulmonary disease (COPD) is one of the most important medical and social problems of modern medicine, in which severe systemic (including hormonal) disorders occur. This review presents data on androgen deficiency in men with COPD and its potential impact on the patients. We have analyzed literary sources in the eLIBRARY and PubMed databases.

Currently, thread lifting in aesthetic facial rejuvenation is not the top priority, but nevertheless its popularity continues to gain momentum. In connection with increasing popularity, the number of issues related to thread lifting and its undesirable side effects also increases. In this publication, an analysis of Russian and international databases was carried out, including MedLine, PubMed, elibrary.ru, Wiley Online Library, Web of Science, over the period from 2010 to 2020 (10 years). In the search, the following keywords were used both in Russian and English: threads, thread lifting, rhytidoplasty, face rejuvenation, face lift, cog threads, Aptos, suture material. As a result, of all available publications, 51 articles were left for further analysis, on the basis of which the nuances of thread classification, histological changes in tissues in response to thread implantation, along with anatomical basics and features during the procedure of thread lifting were analyzed. The effectiveness and duration of the thread lifting effect is still subject to debate. Hence, sometimes, it is possible to identify the duration of the effect in the same study ranging from 1 month to 2 years. Despite the long history of thread lifting, there are still unresolved issues that need to be covered.

Allergic diseases in children have increased significantly. In recent years, it affects up to 35% of children. The current study aimed to investigate the value of the umbilical cord blood total IgE levels in the prediction of the development of allergic diseases during 8 years of follow-up. Methods — In cross-sectional study included 500 infants who were born in the obstetrics department at Tishreen and Al-Assad University Hospitals during the period 2007-2015. Questionnaires were administered after the birth included gender, gestational age, birth weight, mode and season of delivery, smoking during pregnancy, and family history of allergic diseases. Umbilical cord blood total IgE levels were measured. The infants were followed up for eight years for subsequent development of allergic disorders. Results — 214 (42.8%) of 500 newborns had high umbilical cord blood total IgE levels. We followed 143 of 214 newborns for 8 years; there was an allergic family history in 51.7% of newborns. During the following period, allergic diseases developed in 76.22% of the children with high umbilical cord blood total IgE levels. Allergic symptoms in children varied between nasal allergy (19.6%), skin allergy (eczema and urticarial) (25.2%), childhood asthma (31.5%). The rate of allergic symptoms development in the presence of two factors (family history and high umbilical cord blood total IgE) was 51.7%. Conclusion — We found a high prevalence of allergic diseases in children with high umbilical cord blood total IgE. The current study could be used as a preventative strategy to reduce the risk of allergic diseases by predictive of subsequent.
